ADDITIONAL INFORMATION
1. As required by 15A NCAC 02C .0105(f)(5), attach a scaled map of the site showing the locations of the following:

I am/We are the owner(s) of the parcels of property listed above, located at or near the
incident in question, and hereby permit the NC Department of Environmental Quality (NCDEQ),
formerly the Department of Environment and Natural Resources (NCDENR) or its contractor to
enter upon said property for the purpose of conducting an investigation of the groundwaters under
the authority of G.S. 143-215.3(a)2.
I am/We are granting permission with the understanding that:
1. The investigation shall be conducted by the UST Section of the Department's Division of
Waste Management or its contractor.
2. The costs of construction and maintenance of the site and access shall be borne by the
Department or its contractor. The Department or its contractor shall protect and prevent
damage to the surrounding lands.
3. Unless otherwise agreed, the Department or its contractor shall have access to the site by
the shortest feasible route to the nearest public road. The Department or its contractor may
enter upon the land at reasonable times and have full right of access during the period of the
investigation.
4. Any claims which may arise against the Department or its contractor shall be governed by
Article 31 of Chapter 143 of the North Carolina General Statutes, Tort Claims Against State
Departments and Agencies, and as otherwise provided by law.

request and is a public record, in accordance with G.S. 132-1.
6. The activities to be carried out by the Department or its contractor are for the primary
benefit of the Department and of the State of North Carolina. Any benefits accruing to the
owner are incidental. The Department or its contractor is not and shall not be construed to
be an agent, employee, or contractor of the land owner.

Dear Mr. & Mrs. Sasser,
WithersRavenel (WR) is under contract with the North Carolina Department of Environmental
Quality (NCDEQ), Underground Storage Tank (UST) Section, Federal Trust Fund (FTF) Branch for
the assessment of leaking underground storage tanks (LUSTS). NCDEQ has tasked WR with
completing assessment and monitoring activities at the Former Nobles Property, located at 4713
New Britton Highway East in Lak-

near the intersection of Pleasant Plains Church Road and New Britton Highway East, immediately
adjacent to your property. 00 r l ('8
Completed assessment activities relating to the site have indicated the presence of targeted
petroleum compounds in two previously -installed temporary monitoring wells located on the
adjacent property. In an effort to monitor the presence of petroleum compounds on the property,
the NCDEQ incident manager (Mr. Scott Ryals) has instructed WR to petition you to allow for the
installation and sampling of a permanent groundwater monitoring well, to be followed by well
sampling on an on -going basis. As such, WR is respectfully requesting your authorization to allow
for the access to your property to complete the requested tasks. Prior to well installation and any
sampling of the well, a WR representative will contact you to advise you of the projected dates of
activity and for collection of samples.
NCDEQ has requested that we approach you for the purposes of obtaining an access agreement
which will allow us to complete the monitoring and assessment work on the property. Any and all
fees for this monitoring and assessment are the responsibility of NCDEQ, and you Irvin and Betty
Sasser will incur no charges of any kind for this work. I have included an access agreement for
your consideration, and request that you complete and return the agreement in the enclosed self-
addressed stamped envelope. I have also included a copy of the agreement for your records.
