Division of Water Resources' Central and Regional staff has reviewed the application package
received August 13, 2019 and additional information received November 22, 2019 and February 28, 2020.
However, additional information is required before the review may be completed. Please address the items
on the attached pages no later than the close of business on May 25, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through P, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.

1. A hydrogeologic report shall be prepared by a Licensed Geologist, Licensed Soil Scientist, or
Professional Engineer and shall be provided to the Division by the applicant for systems treating
industrial waste. This report shall include items listed in the WWIS 06-16 application, at a
minimum. [15A NCAC 02T .0504(e)]
Note: Industrial facilities with a design flow less than 25,000 gallons per day of wastewater that
demonstrate that the effluent will be of quality similar to domestic wastewater, including effluent
requirements established in 15A NCAC 02T .0505(b)(1), shall, upon request, be exempted from
this requirement. If the Permittee wishes to be exempt from this requirement, provide this effluent
data. The effluent data provided on April 15, 2020 did not comply with this exemption
because effluent standards were much higher than domestic wastewater standards listed in
15A NCAC 02T .0505(b)(1).
