Division of Water Resources' Central and Regional staff has reviewed the application package
received August 13, 2019 and additional information received November 22, 2019. However, additional
information is required before the review may be completed. Please address the items on the attached pages
no later than the close of business on February 21, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through P, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

G. Hvdrogeologic Report:
The hydrogeologic report provided for this application is insufficient because it does not provide
data for the newly proposed irrigation area and the report was performed too long ago. A
hydrogeologic report shall be prepared by a Licensed Geologist, Licensed Soil Scientist, or
Professional Engineer and shall be provided to the Division by the applicant for systems treating
industrial waste. This report shall include items listed in the WWIS 06-16 application, at a
minimum. [15A NCAC 02T .0504(e)]
Note: Industrial facilities with a design flow less than 25,000 gallons per day of wastewater that
demonstrate that the effluent will be of quality similar to domestic wastewater, including effluent
requirements established in 15A NCAC 02T .0505(b)(1), shall, upon request, be exempted from
this requirement. If the Permittee wishes to be exempt from this requirement, provide this effluent
data.
2. Provide an analysis on the current monitoring well locations at this site in relation to the proposed
irrigation area in the hydrogeologic report. If the monitoring wells do not sufficiently capture the
groundwater up -gradient and downgradient of the wastewater application site, propose new well
location(s). [15A NCAC 02T .0504(e)]
