Paul Mays was responsible for the compliance evaluation of the facility. Brad Grosser, Edward Hemilright, Nash Patel, Jason Hassell and Ginger Dawson were also present during the compliance evaluation and technical assistance. Brad Grosser and Edward Hemiright were very helpful during the inspection. Ginger Dawson and Jason Hassel were present for training purposes. Overall, the site and units were well maintained. The records and required paperwork provided by Brad Grosser were well maintained and neatly kept in binders. No discrepancies were found between monitoring reports and lab data. The 11/2019 NDMR and NDAR1 monitoring reports and corresponding lab data were reviewed with no discrepancies The lagoon was found to be in overall, good condition. The berms were free of tree growth, grass around the berm was freshly cut, there was no sign of burrowing animals and no signs of berm failure to be found. The liner showed no signs of damage and the freeboard was measured at 3.7ft. The only concern found was a small portion of fencing surrounding the lagoon was bent by a moderate amount. The Influent Pump Station visual alarm was working during inspection and the audio alarm was not functional. Effluent meter was calibrated by McCrometer on 2/7/2019. ORC is having the effluent meter calibrated next month. ORC did not use site #4 because the site stayed wet and due to this site remaining wet, other sites were used due to being more suitable. Other than site 4 which was not being utilized due to wetness, no issues were found with sites. The cover crop (Sweet Gums) looked good. MW-2 was looked at and held representative as other wells on the site. The well was well maintained and undamaged. The other monitoring wells will be looked at in the next inspection during the cooler winter months. The facility was found to be complaint with permit WQ0000265 Page 2 of 6 Permit: WQ0000265 Owner - Facility: North Carolina Department of Public Safety Inspection Date: 06/30/2020 Inspection Type : Compliance Evaluation Reason for Visit: Routine Type Yes No NA NE Reuse (Quality) ❑ Activated Sludge Spray, LR ❑ Single Family Drip ❑ Activated Sludge Spray, HR ❑ Infiltration System ❑ Activated Sludge Drip, LR ❑ Recycle/Reuse ❑ Single Family Spray, LR ❑ Lagoon Spray, LR Treatment Yes No NA NE Are Treatment facilities consistent with those outlined in the current permit? 0 ❑ ❑ ❑ Do all treatment units appear to be operational?
