You are required to submit this form, witl'i items I through 10 completed, to the appropriate Division of Environmental Management Regional Water Quality Supervisor (see attached listing). At a minimum, you must include an 8.3" by 11" copy of the portion of a 7.5 minute USGS Topographic Map which shows the subject surface waters. You must identify the Iocation of the facility and the closest downslope surface waters (waters for which you are requesting the classification) on the submitted map copy. Three copies of all calculations, including pump selection, friction calculations, cycle time, pump curves (including system curves applicable with one pump running, two pumps running, three pumps running, etc.), and evaluation of downstream pump stations. These items must be submitted under the signature and seal of the North Carolina Professional Engineer. d. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5). e. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting the wastewater flow from this project is different from the applicant of the project, then a letter must be provided from the owner/authority of the WWTF specifying the volume of flow that will be accepted. The letter should be a recent letter and should refer to the project by the same name as that identified on the application and the plans/specifications. Howes, Secretary Subject: Application No. WQ0007858 Additional Information Request Town of Red Springs Street and Utility Improvements CDBG #92-C-8235 Sewer Extension Robeson County The Permits and Engineering Unit has completed a preliminary engineering review of the subject application. The following items must be addressed before we can complete our review: 1) Please be advised that on February 1, 1993, the Division adopted new regulations that require that the infiltration/exfiltration rate must not exceed 100 GPD per inch of pipe diameter per mile of pipe. All sewer extension permit applications received on or after February 1, 1993, must conform to these new regulations. Please resubmit three revised, signed and sealed copies of page 4 of the application form reflecting these changes. Please be advised that the Division is in the process of revising the application forms to reflect the new regulations. Please also revise page SP-4 of your specifications to reflect this change. If you would like to send three revised copies of this page I would be happy to replace this page in your specifications, granted I have your permission. If you choose to have me replace this page please punch the necessary holes along the margin. 2) The 10 foot minimum horizontal separation is not maintained between the water line and sewer line on sheet 6 between MH #4 and MH #8, on sheet 7 between MH # 9 and MH #11 and MH #9 and MH #12. Please show the water line on the sewer line profile in these areas so the Division can confirm that the 18 inch minimum vertical separation has been maintained. Please show the plan and profile view of all of the force main associated with this project. In addition, the plans show some of a proposed 2 inch force main and the application shows 1,570 feet of 3 inch force main. Please explain this discrepancy. 4) Effective February 1, 1993, 15A NCAC 214.0219 (h) (3) requires that "at least one of the following shall be required for pump stations: (A) dual source or standby power supply on site or, (B) telemetry systems with sufficient numbers of standby generators and personnel for distribution or, (C) approval by the Director that the pump station: i) serves a private water distribution system which has automatic shutoff at power failure and no elevated water storage tanks, and ii) has sufficient storage capacity that no potential for overflow exists, and iii) is connected to facilities that can tolerate septic wastewater due to prolonged detention or, (D) where the waters that would be impacted by a power failure are classified as C, the applicant may be allowed to show a history of power reliability that would demonstrate that an alternative power source or other reliability measures would not be needed. Refer to the subject permit application number when providing the requested information. Please submit three copies of all information to my attention at the address below. Also, please note that failure to provide this additional information on or before June 6, 1993 will subject your application to being returned as incomplete, in accordance with 15A NCAC 2H .0208.
