subject application.
The following items must be addressed before we can complete our review:
1) Please provide a topographic map showing site location and adjacent bodies of
water (USGS Quad map suggested).
2) Provide plan/profile of proposed gravity sewer, if it is to be included in above
permit.
Please note that failure to provide this additional information on or before May 27, 1990 will
subject your application to being returned as incomplete in accordance with 15 NCAC 2H .0208.
