We, the Western Piedmont Council of Governments (WPCOG) as for official and written approval to
have access to the Town of Valdese MS4 Annual Reporting Webpage — also known as The Stormwater
Management Program Assessment (SWMPA). We request this approval to be able to submit any
required documentation and reports to the SWMPA on an annual basis for as long as the WPCOG is
administering Town of Valdese stormwater program. The parties allowed to log in and report to the DEQ
on behalf of the Town of Valdese would be the WPCOG Stormwater Administrator, WPCOG Planning
Director, or other relevant parties as described in the contracts between the WPCOG and municipality.
Before submittal of any Reports, metrics, or other documentation the municipality reserves the right to
comment on said documents before submittal to the SWMPA, and by default will receive said
documents first for review.
By signing this document, the Town of Valdese is allowing the relevant parties from the WPCOG to have
access to, and submit reports or relevant documentation to the SWMPA on behalf of the Town.
