We are submitting this change at the request of Ms. Cindy Bale, DEHNR. Please find attached two sketches, a profile and plan view of Causeway "B", depicting the changes. We propose to increase the size of the existing Causeway "B", as shown, to accommodate the construction of two (2) 1219 mm diameter drilled shafts. The revised causeway will extend to Station 1+260 and be positioned between the existing bridge and Causeway "B", as shown on the attached Plan View. Box 25201 Raleigh, NC 27611 Dear Mr. Vick: You have our approval to place temporary fill in Lake Lee/Richardson Creek for the purpose of replacing bridge No. 147 on SR 2102, as you described in your application dated 8 January, 1996 . After reviewing your application, we have decided that this fill is covered by General Water Quality Certification Number 2727.1 This certification allows you to use Nationwide Permit Number 33 when it is issued by the Corps of Engineers. This approval is only valid for the purpose and design that you described in your application. If you change your project, you must notify us and you may be required to send us a new application. For this approval to be valid, you must follow the conditions listed in the attached certification. The abandoned approaches shall be revegetated with native tree species on 10 foot centers to control water temperature. In addition, you should get any other federal, state or local permits before you go ahead with your project. If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must act within 60 days of the date that you receive this letter. To ask for a hearing, send a written petition which conforms to Chapter 150B of the North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This certification and its conditions are final and binding unless you ask for a hearing. This letter completes the review of the Division of Environmental Management under Section 401 of the Clean Water Act. No. 199401628 The referenced project was processed as a Categorical Exclusion, which was approved by the U.S. Army Corps of Engineers on March 11, 1994. The existing one-lane bridge is to be replaced east of its existing location with a two-lane structure, along with associated roadway improvements. It has been determined that the new bridge cannot be constructed without the placement of two temporary causeways in Lake Lee/Richardson Creek to provide access for equipment. The attached cross-section and overhead drawings illustrate the location and dimensions of the two proposed causeways in relation to the new bridge. These causeways may need to be placed in Lake Lee/Richardson Creek simultaneously throughout the construction period. Each causeway will extend 15 meters into the creek from the banks. The causeways will be needed for placement of bridge bents near the shoreline, while a. barge will be used to place bents in the deeper areas of Lake Lee. This will allow over 75 meters clearance to maintain flow of Lake Lee into Richardson Creek. No fill in wetlands will be involved. Placement of Class B rip rap above the banks will be required on both sides of the bridge. Washed stone material obtained from a local rock quarry will be used to prevent the flow of solids into the stream. Vehicular traffic will be maintained on the existing bridge throug out the construction period. Upon completion of the new bridge, the old bridge will be removed and the old approach roadways will be abandoned. No temporary causeways will be required for removal of the old bridge. January 8, 1996 ? Page 2 Prior to construction of the causeways, sediment control devices, including silt fences and a stilling basin, will be installed. Turbidity curtains will be placed around each bent. Upon removal of the temporary causeways and following permanent stabilization of all disturbed areas, all temporary sediment control devices will be removed. Construction is scheduled to extend from May 1996 through May 1998.
