VAN DER VAART Sacrerary TRACY DAVIS Otrec(nr RECEIVED/NCDENR/DWR MAR 4 2016 Water Quality Regional Operations Section Washington Regional Office This is in response to your submission dated February 11, 2016, and received on February 12, 2016, of engineering data and analysis for use of alternative embankment fill for construction of a portion of the subject dam. The submitted information describes materials testing and stability analyses to support use of shell bed material as embankment fill for the second pass between Stations 107+00 and 150+00 of the embankment currently under construction. The analyses demonstrate that the shell bed material may be used as suitable structural fill when placed in the manner described in the geotechnical report prepared by Mr. Brian E. Ladd, PE. This letter constitutes approval of the proposed design changes described in the submittal received on February 12, 2016, subject to all stipulations of the Certificate of Approval dated October 31, 2011, and the additional following stipulations: After placement of the shell bed material in the embankment, regular density testing of the material shall be performed at the frequencies specified and using the appropriate measuring techniques identified in the approved Technical Specification. These tests shall show that the material exhibits a dry density of 100 pounds per cubic foot or greater. Documentation of the successful tests shall be submitted with record drawing submittal at completion of construction (reference Stipulation No. 3 of Certificate of Approval dated October 31, 2011). 2. Regular visual inspections of the shell bed section of the dike and documentation of its performance shall be conducted by qualified personnel to insure maintenance needs are identified and maintenance activities performed as necessary to insure safe operation. Documentation associated with these inspections shall be submitted with record drawing submittal at completion of construction (reference Stipulation No. 3 of Certificate of Approval dated October 31, 2011).
