ADAMS — PUBLIC UTILITIES DIRECTOR CITY OF DUNN POST OFFICE BOX 1065 DUNN, NORTH CAROLINA 28335 Subject: Permit No. WQ0006101 City of Dunn RLAP Attachment B Certification Harnett County Dear Ms. Adams: In accordance with your permit major modification request received April 15, 2020, we are forwarding herewith a modified Attachment B certified July 8, 2020, to the City of Dunn for the subject residuals management program. Modifications to the subject Attachment B are as follows: Addition of six new fields HA-307-4. 11A-307-5, 11A-307-6, IA-307-7A, SA-19-01, and SA-19-02 totaling 123.0 acres. The modified Attachment B shall be effective from the date of certification until September 30, 2021 (i.e., expiration date of the current permit), shall void Attachment B of Permit No. WQ0006101 issued October 13, 2016, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace Attachment B of Permit No. WQ0006101 that was issued October 13, 2016 with the modified Attachment B. Please note that the Permittee shall submit a renewal application no later than April 3, 2021. Please note that only the maps for new land application sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements or limitations contained in this certification are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
