Dear Mr.. Ducote:
A letter of request for Authorization to Construct was received September
9, 1988, by the Division and final. plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is
hereby granted for the construction of .04 MGD wastewater treatment facility
consisting of a bar screen, a 40,000 gallon aeration tank, a 6,670 gallon
clarifier, a 6,000 gallon sludge holding tank, two (2) 200 CFM blowers, a tablet
chlorinator, a 834 gallon chlorine contact tank and a continuous, recording
effluent flow meter.
This Authorization to Construct is issued in accordance with Part III,
paragraph B of NPDES Permit No. NC0073743. issued August 16, 1988, and shall be
subject to revocation unless the wastewater treatment facilities are constructed
in accordance with the conditions and limitations specified in Permit No.
NC0073741.
The Permittee must employ a certified wastewater operator in accordance
with Part III paragraph C of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the
North Carolina Division of Environmental Management.
In the event the facilities fail to perform satisfactorily in meeting its
NPDES permit effluent limits, the Permittee shall take such immediate corrective
action as may be required by this Division, including the -construction of
additional wastewater treatment and disposal. .facilities.

Within six (6) months of construction of these wastewater treatment
facilities an application shall be made by the Permittee for a sludge land
application permit.
The Operational Agreement between the Permittee and the Environmental
Management Commission is incorporated herein by reference and is a condition of
this Permit. Noncompliance with the terms of the Operational Agreement shall
subject the Permittee to all sanctions provided by G. S. 143-215.6 for violation
of or failure to act in accordance with the terms and conditions of this Permit.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer
certifying that the permitted facility has been installed in accordance with the
approved plans and specifications. Mail the certification to the Permits and
Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687.
