Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The project area consists of an earthen and gravel access road and existing culverted crossing of a channelized stream. The access road is located in a cultivated field and is adjacent to the NCDOT ROW for US Hwy 17 Bypass to the north. The vicinity consists of a mixture of agricultural land, wooded areas and scattered residential/commercial development. 4b. Explain the purpose of the proposed project,* The purpose of the project is to replace a damaged 36-inch diameter reinforced concrete pipe (RCP) and wooden headwalls with a new RCP and concrete headwalls that are compatible with current safety standards. The existing road and crossing are used to provide vehicular access to Piedmont's Line 243 Launcher Site. 41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* The project Will first entail establishment of a darn -and -pump system around the crossing to create a dry work area. Coffer dams will be constructed within 15 feet of the upstream and downstream extents of the existing crossing. A pump will then by used to transfer clean water around the work area. The old 36-inch RCP and wooden headwalls Will then be removed and replaced with the new RCP and concrete headwalls. The new RCP will be 42 inches in diameter so that the bottom of the pipe can be appropriately embedded per conditions of the permit. The footprint of the existing stream crossing is 24 linear feet. The new headwalls will extend beyond the footprint of the existing headwalls by 3 feet at each end, resulting in a new footprint of 30 linear feet. The work will be performed by industry standard earth moving equipment, and in it's entirety, will be conducted within a 60-If portion of the channelized stream. 4j. The increase in diameter from 36 inches to 42 inches is necessary to appropriately bury the bottom 20 percent of the new pipe. The additional 6 feet is necessary to accommodate new headwalls that are compatible with current safety standards. 1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: The work will be conducted "in -the -dry" by constructing a dam -and -pump system such that flowing water does not come into contact With the work area. Appropriate erosion and sediment control measures will be employed as necessary to prevent undue siltation of the channelized stream. Any temporarily disturbed streambanks Will be returned to pre -construction condition and stabilized in accordance with the project's E&SC Plan. 2.
