GENERAL NOTES:
1. The Stormwater pond shall be permanently vegetated and
operational prior to construction of any impervious surface.
2. Swale construction along Northern property boundary extending
off -site through the property of Gene A. Jackson to the
highway ditch shall be coordinated with the owner and Mr.
Jackson.
3. All storm drainage piping shall be reinforced concrete pipe
except pipe connecting forebay and pond and pipe exiting pond
which can be corrugated aluminum or metal pipe.
4. Subgrade preparation shall be done in accordance with
recommendation of Soils Engineer, Geotechnologies, Inc., in
report dated May 12, 1995. Contact owner for copies of this
report.
5. Boundary and topographic survey data prepared for Bailey and
Associates by James C. Simmons and Associates, Registered Land
Surveyors.
6, Upon recommendation of Soils Engineer, Geotechnologies, Inc.,
parking lot section shall consist of 2-inches type 1-2
asphalt, 6-inches coarse aggregate type base course, over
compacted subgrade. Heavy paving section for primary drives
and rear of center shell consist of 2-inches type 1-2 asphalt,
8-inches coarse aggregate type base course, over compacted
subgrade.
7. Concrete for curb and gutter shall be 3000 psi.

EROSION CONTROL PLAN NOTES:
1. Construction Sequence:
a. Notify North Carolina Department of Environment, Health
and Natural Resources office in Washington, North
Carolina for notification of pre -construction conference
prior to beginning construction.
b. Install stabilized construction entrances at Greenfield
Boulevard. Northernmost driveway entrance shall not be
used for construction access to site.
C. Install silt fence paralleling roadside ditch along
Greenfield Boulevard as indicated on plan.
d. Excavate area for stormwater pond. Contractor should
over excavate by 1 foot to elevation 19.2 to allow for
sedimentation during construction. During site grading
operations all site drainage shall be diverted to pond
area until permanent drainage features are installed.
Suitable materials from pond excavation shall be used for
building pod in accordance with recommendation from Soils
Engineer, Geotechnologies, Inc.
e. Rough grade project site and install storm drainage
piping and catch basins. Catch basins shall be
constructed with sediment barrier in accordance with plan
detail.
f. Install swale at rear of building along with sediment
trap at entrance to Pipe "C"•
g. Install water and sewer utilities.
h. Install stone base course.
i. Fine grade stone base course and place asphalt surface
course.
j. Seed, fertilize, and mulch all disturbed area within 30
days of completing grading work.
2. Seeding Specifications
Apply lime and fertilizer and work into seed bed. Seed
mixture shall be distributed uniformly and covered with a
clean straw mulch. Where slopes exceed 4%, asphalt tack shall
be applied. Material and application rates follow:

1. All water and sewer construction shall be completed in
accordance with the standard specifications of the City of
Havelock's Public Utilities Department.
2. Contractor shall notify Utilities Engineer 48 hours in advance
of beginning construction. Coordinate tie—in to all existing
utilities with Utilities Engineer.
3. Location of existing water and sewer mains are approximate
only and shall be field verified prior to commencing work.
Any discrepancies should immediately be pointed out to the
Engineer and Utilities Department.
4. As of January 1, 1986, anyone digging with mechanized
equipment in highway right—of—ways, private utility easements,
or public spaces will be required by North Carolina law to
give notice of the proposed excavation to all existing are
utilities at least 2 working days before starting to dig. The
utility owner is to locate Its facilities in the area of the
proposed excavation. Utility member of ULOCO can be contacted
at 1-800-632-4949.
5. 4—inch through 12—inch water mains shall conform to AWWA
Specifications C-900-75, Class 150.
6. Water mains shall be buried to a depth of 30 inches, unless
otherwise noted.
7. Contractor shall maintain comprehensive (depth and horizontal
locations) field "as--builts" for all installations and submit
this to the Engineer.

Construction Specifications
1. Stone size - Use MSHA size No. 2 (2-1/2" to 1") or AASHTQ
designation M43, size No. 2 ( 2-1/2" to 1-1/2"). Use
crushed stone.
2. Length - As effective, but not less than 50 feet.
3. Thickness - Not less than six (6) inches.
4. Width - Not less than full width of all points of
ingress or egress. Minimum of 12'.
5. Washing - When necessary, wheels shall be cleaned to
remove sediment prior to entrance onto public right-of-
way. When washing is required, it shall be done on an
area stabilized with crushed stone which drains Into an
approved sediment trap or sediment basin. All sediment
shall be prevented from entering any storm drain, ditch,
or watercourse through use of sandbags, gravel, boards
or other approved methods.
6. Maintenance - The entrance shall be maintained in a
condition which will prevent tracking or flowing of
sediment onto public rights -of -way. This may require
periodic top dressing with additional stone as conditions
demand and repair and/or cleanout of any measures used to
trap sediment. All sediment, spilled, dropped. washed or
tracked onto public rights -of -way must be removed
immediately.
