A Division of Energy, Mineral, and Land Resources file review has determined that Stormwater Permit SW7120608 for a stormwater treatment system consisting of a wet detention pond serving the Harvey Point Admin Complex located off Harvey Point Road in Perquimans County expires on June 29, 2020. This is a reminder that permit renewal applications are due 180 days prior to expiration. We do not have a record of receiving a renewal application. Please submit a completed permit renewal application along with a $505.00 fee for permit renewal. Also, it was noted in the file review that a designer's certification that the project was constructed in accordance with the approved plans as required by the permit has not been submitted. Staff review of the plans and specifications has determined that the project, as proposed, will comply with the Stormwater Regulations set forth in Session Law 2008-211 and Title 15A NCAC 2H.1000. This permit is effective only with respect to the nature and volume of stormwater described in the application and other supporting data. 2. The built -upon area percentage for the low density portion of this project must be maintained at 24% per the requirements of Session Law 2008-211 and 15A NCAC 2H..1000. This project is permitted for a maximum of 224,757 square feet of BUA (for both Low Density and High Density portions of this project). 3. The stormwater systems proposed to treat the runoff generated by the designated high density area has been approved for the management of stormwater runoff as described in Section 1.6 on page 3 of this permit. 4. This project will maintain a minimum 50 foot wide vegetated buffers between all impervious areas and surface waters on new development and a minimum 30 foot wide vegetated buffer on all existing and previously permitted impervious areas. 5. Within the low density portions of the permitted project, the only runoff conveyance systems allowed will be vegetated conveyances such as swales with minimum side slopes of 3:1 (H:V) as defined in the stormwater rules and as shown on the approved plan. 6. The permittee is responsible for verifying that the proposed built -upon area for the entire lot does not exceed the maximum allowed by this permit. 3. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. 4. The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface. 5. The runoff from all built -upon area within the drainage area of the high density portion of the project must be directed into the permitted stormwater control system(s). 6. Within the low density portions of the permitted project, all roof drains shall terminate at least 50 feet from the mean high water mark of surface waters. 7. The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. e. Further subdivision, -acquisition, or sale of all or part of the project area. The project area is defined as all property owned by the permittee, for which Sedimentation and Erosion Control Plan approval or a CAMA Major permit was sought. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. 8. The Director may determine that other revisions to the project should require a modification to the permit. 9. All stormwater collection and treatment systems must be located in either dedicated common areas or recorded easements. The final plats for the project must be recorded showing all such required easements, in accordance with the approved plans. 10. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 11. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 12. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility. 13. Permanent seeding requirements for the stormwater control must follow the guidelines established in the North Carolina Erosion and Sediment Control Planning and Design Manual. 14. This permit shall become void unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 15. The permittee shall at all times provide the operation and maintenance necessary to assure that all components of the permitted stormwater system function at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowing and re -vegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of structures, orifice, catch basins and piping. g. Access to all components of the system must be available at all times. 16. Records of maintenance activities must be kept by the permittee. The records will indicate the date, activity, name of person performing the work and what actions were taken. 17. The permittee shall submit to the Division of Water Quality an annual summary report of the maintenance and inspection records for each BMP. The report shall summarize the inspection dates, results of the inspections, and the maintenance work performed at each inspection. 18. Prior to transfer of the permit, the stormwater facilities will be inspected by DWQ personnel. The facility must be in compliance with all. permit conditions. Any items not in compliance must be repaired or replaced to design condition prior to the transfer. Records of maintenance activities performed to date will be required. III. GENERAL CONDITIONS This permit is not transferable to any person or entity except after notice to and approval by the Director. In the event there is either a desire for the facilities to change ownership, or there is a name change of the Permittee, a completed "Name/Ownership Change Form" must be submitted to the Division of Water Quality, signed by the parties involved, along with the applicable documents as . listed on page 2 of the form. The project must be in good standing with DWQ. The approval of this request will be considered on its merits and may or may not be approved. 2. The permittee is responsible for compliance with all of the terms and conditions of this permit until such time as the Director approves the transfer request. 3. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 4. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal), which have jurisdiction. 5. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and re -issuance or termination does not stay any permit condition. 7. Permittee grants permission to staff of the DWQ to access the property for the purposes of inspecting the stormwater facilities during normal business hours. 8. Approved plans and specifications for this project are incorporated by reference and are enforceable parts of the permit. 9. The permittee shall notify the Division in writing of any name, ownership or mailing address changes within 30 days. 10. A copy of the approved plans and specifications shall be maintained on file by the Permittee at all times. 11. The issuance of this. permit does not prohibit the Director from reopening and modifying the permit, revoking and reissuing the permit, or terminating the permit as allowed by the laws, rules and regulations contained in Session Law 2008-211, Title 15A NCAC 2H.1000, and NCGS 143-215.1 et.al. 12. This permit shall be effective from the date of issuance until June 29, 2020. Grass height will not exceed six inches at any time; and grass will not be mowed too close to the ground or "scalped". Swales will be inspected monthly or after every runoff producing rainfall event for sediment build-up, erosion, and trash accumulation. Accumulated sediment and trash will be removed as necessary. Swales will be reseeded or sodded following sediment removal. 4. Eroded areas of the swales will be repaired and reseeded. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. 4. The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface. 5. The runoff from all built -upon area within the drainage area of the high density portion of the project must be directed into the permitted stormwater control system(s). 6, Within the low density portions of the permitted project, all roof drains shall terminate at least 50 feet from the mean high water mark of surface waters. 7. The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. The project area is defined as all property owned by the permittee, for which Sedimentation and Erosion Control Plan approval or a CAMA Major permit was sought. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. 8. The Director may determine that other revisions to the project should require a modification to the permit. 9. All stormwater collection and treatment systems must be located in either dedicated common areas or recorded easements. The final plats for the project must be recorded showing all such required easements, in accordance with the approved plans. 10. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 11. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 12. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility. 13. Permanent seeding requirements.for the stormwater control must follow the guidelines established in the North Carolina Erosion and Sediment Control Planning and Design Manual. 14. This permit shall become void unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 15. The permittee shall at all times provide the operation and maintenance necessary to assure that all components of the permitted stormwater system function at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semi-annual scheduled inspections (every 6 months). b. Sediment removal. c. Mowing and re -vegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of structures, orifice, catch basins and piping. g. Access to all components of the system must be available at all times. 16. Records of maintenance activities must be kept by the permittee. The permittee shall submit to the Division an annual summary report'of the maintenance and inspection records for each BMP. The report shall summarize the inspection dates, results of the inspections, and the maintenance work performed at each inspection. 18. Prior to transfer of the permit, the stormwater facilities will be inspected by Division personnel. The facility must be in compliance with all permit conditions. Any items not in compliance must be repaired or replaced to design condition prior to the transfer. Records of maintenance activities performed to date will be required. III. GENERAL CONDITIONS This permit is not transferable to any person or entity except after notice to and approval by the Director. In the event, there is either a desire for the facilities to change ownership, or there is a name change of the Permittee, a completed "Name/Ownership Change Form" must be submitted to the Division, signed by the parties involved, along with the applicable documents as listed on page 2 of the form. The project must be in good standing with DEMLR. The approval of this request will be considered on its merits and may or may not be approved. 2. The permittee is responsible for compliance with all of the terms and conditions of this permit until such time as the Director approves the transfer request. 3. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division, in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 4. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal), which have jurisdiction. 5. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and re -issuance or termination does not stay any permit condition. 7. Permittee grants permission to Division staff to access the property for the purposes of inspecting the stormwater facilities during normal business hours. 8. Approved plans and specifications for this project are incorporated by reference and are enforceable parts of the permit. 9. The permittee shall notify the Division in writing of any name, ownership or mailing address changes within 30 days. 10. A copy of the approved plans and specifications shall be maintained on file by the Permittee at all times. 11. HPDTA is now planning to install additional asphalt pavement and small concrete pads within the high density drainage area as well as two small concrete pads within the low density area on the site. In order to accommodate this new impervious area, HPDTA proposes modifying the existing stormwater management permit to demonstrate compliance with NCDEQ's rules. That Permit Modification is the subject of this narrative and associated plans and Stormwater Permit Modification Application. Original Permit Conditions The subject permit defined a 23.65 acre project area which included a 13.16 high -density drainage area. Overall impervious coverage was proposed to be 4.41 acres (192,100 sf, 18.6%) and within the high density drainage area, impervious coverage was proposed to be 2.83 acres (123,275 sf, 21.5% of the drainage area). Runoff outside of the high density drainage area was proposed to sheet flow from paved areas into grassed swales flanking the building and parking and conveying runoff around the high density drainage area to an existing outfall to the north. Runoff within the high density area was to be collected via a series of storm sewer, paved flumes, and grassed swales and conveyed to a Wet Detention Basin located in the north end of the project area. Current Field Conditions The buildings, parking, and stormwater improvements associated with the original permit have been installed and appear to be functioning in accordance with the permit requirements. As -built measurements show that the Wet Basin was constructed with a slightly larger footprint than originally proposed, while maintaining the general geometric ratios originally proposed. The storage available above the permanent pool and below the 1-yr storm weir within the basin is 21,962 ft3 (equivalent to runoff generated by 1.75 inch storm). A new Wet Basin Supplement has also been filled out to reflect these new conditions and is included with this submission. Conclusions This proposed Modification does not propose to alter the existing Stormwater Wet Basin or Stormwater Collection system in any way. The proposed impervious coverage additions are diffuse throughout the drainage area and will drain via overland flow to existing conveyances. The attached calculations and new Wet Basin Supplement demonstrate compliance of the existing Stormwater Management system, even with the addition of the newly proposed impervious coverage. The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. e. Further subdivision, acquisition, or sale of all or part of the project area. The project area is defined as all property owned by the permittee, for which Sedimentation and Erosion Control Plan approval or a CAMA Major permit was sought. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. 8. The Director may determine that other revisions to the project should require a modification to the permit. 9. All stormwater collection and treatment systems must be located in either dedicated common areas or recorded easements. The final plats for the project must be recorded showing all such required easements, in accordance with the approved plans. 10. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 11. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 12. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility. 13. Permanent seeding requirements for'the stormwater control must follow the guidelines established in the North Carolina Erosion and Sediment Control Planning and Design Manual. 14. This permit shall become void unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 15. The permittee shall at all times provide the operation and maintenance necessary. to -assure that all components of the permitted stormwater system function at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. c. Mowing and re -vegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of structures, orifice, catch basins and piping. g. Records of maintenance activities must be kept by the permittee. The records will indicate the date, activity, name of person performing the work and what actions were taken. 17. The permittee shall submit to the Division of Water Quality an annual summary report of the maintenance and inspection records for each BMP. The report shall summarize the inspection dates, results of the inspections, and the maintenance work performed at each inspection. 18. Prior to transfer of the permit, the stormwater facilities will be inspected by DWQ personnel. The facility must be in compliance with all permit conditions. Any items not in compliance must be repaired or replaced to design condition prior to the transfer. Records of maintenance activities performed to date will be required. lll. GENERAL CONDITIONS 1. This permit is not transferable to any person or entity except after notice to and approval by the Director. In the event there is either a desire for the facilities to change ownership, or there is a name change of the Permittee, a completed "Name/Ownership Change Form" must be submitted to the Division of Water Quality, signed by the parties involved, along with the applicable documents as listed on page 2 of the form. The project must be in good standing with DWQ. The approval of this request will be considered on its merits and may or may not be approved. 2. The permittee is responsible for compliance with all of the terms and conditions of this permit until such time as the Director approves the transfer request. 3. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 1437215.6A to 143-215.6C. 4. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal), which have jurisdiction. 5. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management•systems. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and re -issuance or termination does not stay any permit condition. 7. Permittee grants permission to staff of the DWQ to access the property for the purposes of inspecting the stormwater facilities during normal business hours. 8. Important maintenance procedures: — Immediately after the wet detention basin is established, the plants on the vegetated shelf and perimeter of the basin should be watered twice weekly if needed, until the plants become established (commonly six weeks). — No portion of the wet detention pond should be fertilized after the first initial fertilization that is required to establish the plants on the vegetated shelf. -- Stable groundcover should be maintained in the drainage area to reduce the sediment load to the wet detention basin. — If the basin must be drained for an emergency or to perform maintenance, the flushing of sediment through the emergency drain should be minimized to the maximum extent practical. — Once a year, a dam safety expert should inspect the embankment. After the wet detention pond is established, it should be inspected once a month and within 24 hours after every storm event greater than 1.0 inches (or 1.5 inches if in a Coastal County). Records of operation and maintenance should be kept in a known set location and must be available upon request. Inspection activities shall be performed as follows. The site is very flat with elevations ranging from around elevation 5 at the low point to a high of about 8 close to Towers Boulevard. The drainage ditch on the west side of the site and as it turns along River Road ranges from elevation 3.0 to 1.2. The site drains to this ditch. The ditch crosses River Road through a 30-inch culvert then continues approximately 750 feet to the Perquimans River. The southern portion of the site along Towers Boulevard is covered with mature pine trees. The remainder of the site is covered with smaller dense pines and hardwoods. Twenty-nine soil test borings were drilled at the proposed buildings and parking lot. Groundwater levels ranged from 5 to 8 feet below existing grade. Groundwater levels were approaching their seasonal high at the time the borings were drilled. The site is covered with topsoil that ranges in depth from 2 to 6 inches. Stratum I with an average depth of 0.3 to 12.7 feet consisted of very stiff to medium stiff, moist to wet, brown and gray, silty, fine sandy clay (CL, CL-ML). Stratum 2 with an average depth from 12.7 to 40.5 feet consisted of loose to compact, wet, grayish brown -to -brown, silty, fine sand (SP-SM, SM). Maximum boring depth was 40.5 feet. Scope of the project: The "HPDTA Administration Complex" includes 21,056 square foot, 2-story BOQ building; 20,990 square foot, one-story Dining Hall; 5,636 square foot, one-story Fitness Center and future 6,744 square foot, one-story Conference Center. The site has ingress and egress access to Second Street from the paved road and from the paved parking for 138 cars. Outside of the above -described boundary, a sidewalk will be constructed to the existing Administration Building. The new sidewalk will be installed from the existing Administration Building sidewalk to the existing entrance drive and then will continue on site to the new Dining Hall. The offsite sidewalk is approximately 350 feet long. The total disturbed area for this project, including utility installation, approximately 16.26 acres. HDPTA is bounded to the north by Perquimans River and to the east and south by Albemarle Sound. Both bodies of water are in the Pasquotank Basin. DENHR has classified the Perquimans River as "C;Sw", indicating "Aquatic Life, Secondary Recreation, Fresh Water and Swamp Waters". Albemarle Sound is classified as "SB" indicating "Primary Recreation , Salt Water". All parts of the New Range "C" project are located more than 100 feet away from any down slope body of water. 1Wae STORMWATER MANAGEMENT NARRATIVE Harvey Point Defense Testing Activity HPDTA Administration Complex June 5, 2012 The drainage plan for this complex is very flat, with no deep, defined drainage ditches convenient to the site to which to drain. Since there is not adequate relief, the perimeter drainage swales are very flat in the longitudinal slope (0.25%). The surrounding swales serve to provide building security as well as act as a drainage conveyance and stormwater quality. Security requirements dictate that the swales be deeper, wider, and steeper than the need for a drainage swale. Most of the disturbed area (J-11.0 Ac) drains into the proposed wet detention basin. f 3.5 Ac of the disturbed area drain into the existing perimeter ditch on the north- west side of the property and + 1.6 Ac of the disturbed area drains into the Second Street storm structures which discharges to the perimeter channel on the north-east side. In general, the land near this site drains northeast toward Perquimans River, approximately 600 feet from the site. Stormwater management (90% TSS removal requirement) is achieved by the use of the extended wet detention basin (90% TSS removal). Detention Basin is located on the north corner of the property site and has 30' V-notch outfall weir that drain in to the perimeter ditch. This weir allows more than 48 hours draw down time for the volume of 1.5" across the contributing drainage area. Rv = 0.05+0.9*(2.83/13.16) = 0.24 V = 3630*1.5"*0.24*13.16 = 17,197 Cu.F.
