We figured that Jamie Applequist would be signing it and his title is Executive Vice -President. If this is not correct it needs to be modified in Section B. The document needs to be signed and initialed. Section C: 2WEVED JAN 2 3 200 1. Include check. 2. Suggest some documentation confirming Mr. Applequist's title and authority be included in possible. 3. This should be Not Applicable as we would think all documents have already been submitted at this point. 4. 0&M Agreements; We do not have one in our files, but hopefully SECU has a copy. If not then you can ask for a copy to be sent to you. 5. Designer Certifications: We do not have a copy of the final certification in our files. Hopefully, this was previously submitted (this would be the first selection). If you do not know we can contact Roger Thorpe at the DEQ office and ask him. 6. You should be able to submit the attached Inspection Report we completed in December 2019 for his item. 7. SECU is a Non -Profit Corporation and as such may not be required to provide this information, but we suggest you include some documentation. We checked the Secretary of State's website and the officers are not listed. Maybe you have some internal document you can provide. Let me know if you need additional assistance.
