Dear Mr. Bayer:
On May 30, 2014 a compliance inspection was conducted for the tract/project known as
Patriot Park, located at 404 Miller Boulevard in Havelock, NC. State Stormwater permit
SW7111108 was issued for the construction and operation of a wet detention pond that will treat
stormwater runoff from impervious surfaces on site for the life of the project. A file review noted
that we have not received a designer's certification for this project. Condition number II. 10. states "
Upon completion of construction, prior to issuance of a certificate of occupancy, and prior to
operation of this permitted facility, a certification must be received from an appropriate designer for
the system installed certifying that the permitted facility has been installed in accordance with this
permit, the approved plans and specifications, and other supporting documentation. Any deviations
from the approved plans and specifications must be noted on the Certification. A modification may
be required for those deviations."
Please be advised that you are required to comply with the terms, conditions and limitations of your
Stormwater Management Permit under Title 15A North Carolina Administrative Code 21-I .1003
and North Carolina General Statute 143-214.7. Failure to comply with the terms of your permit may
result in enforcement proceedings. A designer's certification should be submitted within 30 days of
receipt of this letter.

Craven County
On July 10, 2013, I conducted a site inspection for the tract/project known as
Patriot Park, located at 404 Miller Boulevard in Havelock, NC. The site visit and file
review revealed that the subject project has an approved Erosion and Sedimentation
Control Plan and is covered by Stormwater General PermitNCGO10000 and State
Stormwater permit SW7111108.
A Construction Stormwater General Permit (NCGO10000) is issued upon the
approval of an Erosion and Sedimentation Control Plan. This permit applies to projects
that receive a Division of Land Resources (DLR) (or local delegated program) Erosion
and Sedimentation Control Plan for land disturbance of l acre or greater. Specifically,
the General Permit (NCGO10000) authorizes the discharge of stormwater under the
National Pollutant Discharge Elimination System ("DES) in accordance with Title 15A
North Carolina Administrative Code 2H.0100 during the construction phase of the
project. The inspection for this permit revealed that construction appeared to be partially
complete and that no construction activities are currently on going at the site. No houses
have been constructed and the site appeared to be mostly stable with only some minor
erosion. Please keep in mind that until the project has been released by the Division of
Land Resources you must continue to comply with the construction stormwater permit.
Among other things the permit requires that a rain gauge be maintained on site and that
weekly inspections be conducted with the results recorded and maintained for inspection.

Re: Soil Evaluation for Stormwater Facility Design

On August 11, 2011, a hand augured soil boring was made by me to a depth of 60 inches
below ground level (22.0 ft +/-) in natural soils in the area of the proposed stormwater
wet detention pond for the referenced project for the purpose of determining the seasonal
high water table. This information is needed for the design of the permanent pool level
elevation of proposed stormwater wet detention pond for this project. The soil type
encountered was Goldsboro Urban land complex (GuA), which is moderately well
drained. This soil type also coincides with the mapped soil for this area as shown in the
"Soil Survey of Craven County, North Carolina" as prepared by the US Department of
Agriculture -Soil Conservation Service. The bore hole was left open and day to day
monitoring indicates the current water table to be at a depth of greater than or equal to
3.75 feet below the existing ground surface. Therefore, for the purpose of this
application, we find that the seasonal high water table is at approximate elevation of
18.25 feet.

project. Enclosed items include an (1) original and one (1) copy of the application form,
two (2) wet detention basin supplements, two (2) notarized operation and maintenance
agreements, two (2) copies of the deeds, two (2) Department of the Secretary of State
printout, two (2) copies of the Engineering plans, one (1) copy of the Engineering report,
two (2) copies of the SHWT Report, and one (1) check for $505.00 (Check No. 1001) for
the application processing fee.
This project is site grading, improvements, water line, sewer line, pump station, and
stormwater installation for a proposed 15 lot residential subdivision. Total site area is
4.31 acres. This is a high -density project and permanent stormwater will travel through
the proposed wet detention basin and discharge into an offsite adjacent ditch.
