ALL PERSONNEL SHALL WEAR IDENTIFYING MARKINGS ON HARD HATS TO CLEARLY IDENTIFY THE COMPANY FOR WHOM THE EMPLOYEE WORKS. 7. EMPLOYEES SHALL PARK PRIVATELY OWNED VEHICLES IN AN AREA DESIGNATED BY THE CONTRACTING OFFICER. THIS AREA WILL BE WITHIN REASONABLE WALKING DISTANCE OF THE CONSTRUCTION SITE. EMPLOYEE PARKING SHALL NOT INTERFERE WITH EXISTING AND ESTABLISHED PARKING REQUIREMENTS OF THE INSTALLATION. PARKING WILL BE LOCATED IN THE PARKING LOTS OFF OF EL SALVADOR. 8. ADMINISTRATIVE FIELD OFFICES: PROVIDE AND MAINTAIN ADMINISTRATIVE FIELD OFFICE FACILITIES WITHIN THE CONSTRUCTION AREA OF THE DESIGNATED SITE UNLESS APPROVED BY THE CONTRACTING OFFICER AND THE INSTALLATION. GOVERNMENT OFFICE AND WAREHOUSE FACILITIES ARE NOT AVAILABLE TO THE CONTRACTOR'S OR SUBCONTRACTORS' EMPLOYEES. 9. TRAILERS, EQUIPMENT, OR MATERIALS SHALL NOT BE OPEN TO PUBLIC VIEW WITH THE EXCEPTIONS OF THOSE ITEMS WHICH ARE IN SUPPORT OF ONGOING WORK ON ANY GIVEN DAY. DO NOT STOCKPILE MATERIALS OUTSIDE THE FENCE IN PREPARATION FOR THE NEXT DAY'S WORK. PARK MOBILE EQUIPMENT, SUCH AS TRACTORS, WHEELED LIFTING EQUIPMENT, CRANES, TRUCKS, AND LIKE EQUIPMENT WITHIN THE FENCED AREA AT THE END OF EACH WORK DAY. LOCATE CONSTRUCTION TRAILER(S) WITHIN LIMITS OF CONSTRUCTION, OR IN LOCATION AS APPROVED BY CONTRACTING OFFICER. LOCATE THE LAYDOWN YARD/STORAGE AREA WITHIN THE DESIGNATED AREAS APPROVED BY THE CONTRACTING OFFICER AND THE INSTALLATION. 10. ALL TEMPORARY UTILITIES (WATER, SEWER, ELECTRICAL, TELECOMMUNICATIONS, ETC) WILL BE AT THE CONTRACTOR'S EXPENSE AND SUBJECT TO FORT BRAGG REGULATIONS. CONTRACTOR MUST NEGOTIATE AND CONTRACT WITH THE PRIVATIZATION UTILITY DIRECTLY WITHOUT BENEFIT OF THE GOVERNMENT. 11. COORDINATE WITH ONUS FOR ANY TEMPORARY WATER AND SANITARY SEWER SERVICE. ONUS WILL PROVIDE ESTIMATE FOR CONNECTION COSTS. 12. COORDINATE WITH SANDHILLS UTILITIES SERVICES FOR ANY TEMPORARY ELECTRICAL SERVICES. SANDHILLS UTILITIES SERVICES WILL PROVIDE ESTIMATE FOR CONNECTION COSTS. 13. STORAGE EQUIPMENT AND FACILITIES USED FOR ADMINISTRATIVE OR MATERIAL STORAGE PURPOSES SHALL PRESENT A CLEAN AND NEAT EXTERIOR APPEARANCE AND SHALL BE IN A STATE OF GOOD REPAIR. TRAILERS, WHICH, IN THE OPINION OF THE CONTRACTING OFFICER, REQUIRE EXTERIOR PAINTING OR MAINTENANCE, WILL NOT BE ALLOWED ON THE INSTALLATION. 14. KEEP FENCING IN A STATE OF GOOD REPAIR AND PROPER ALIGNMENT. SHOULD THE CONTRACTOR ELECT TO TRAVERSE, WITH CONSTRUCTION EQUIPMENT OR OTHER VEHICLES, GRASSED OR UNPAVED AREAS, WHICH ARE NOT ESTABLISHED ROADWAYS, COVER SUCH AREAS WITH A LAYER OF GRAVEL AS NECESSARY TO PREVENT RUTTING AND THE TRACKING OF MUD ONTO PAVED OR ESTABLISHED ROADWAYS. GRAVEL GRADATION SHALL BE AT THE CONTRACTOR'S DISCRETION. MOW GRASS LOCATED WITHIN THE BOUNDARIES OF THE CONSTRUCTION SITE FOR THE DURATION OF THE PROJECT. TRIM GRASS AND VEGETATION ALONG FENCES, BUILDINGS, UNDER TRAILERS, AND IN AREAS NOT ACCESSIBLE TO MOWERS AND EDGE NEATLY. 15. PROVIDE ADEQUATE OUTSIDE SECURITY LIGHTING AT ALL TEMPORARY FACILITIES. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE SECURITY OF ITS OWN EQUIPMENT. NOTIFY THE APPROPRIATE LAW ENFORCEMENT AGENCY REQUESTING PERIODIC SECURITY CHECKS OF THE TEMPORARY PROJECT FIELD OFFICE. 16. AS SOON AS PRACTICAL, BUT NOT LATER THAN 15 DAYS AFTER THE DATE ESTABLISHED FOR COMMENCEMENT OF WORK, FURNISH AND ERECT TEMPORARY PROJECT SAFETY FENCING AROUND THE CONSTRUCTION SITE. THIS FENCING SHALL REMAIN THE PROPERTY OF THE CONTRACTOR. THE SAFETY FENCING SHALL BE 9 GAUGE CHAIN LINK FENCE, A MINIMUM OF 72 INCHES HIGH, SUPPORTED AND TIGHTLY SECURED TO STEEL POSTS LOCATED ON A MAXIMUM OF 10 FOOT CENTERS, CONSTRUCTED AT THE APPROVED LOCATION. MAINTAIN THE SAFETY FENCING DURING THE LIFE OF THE CONTRACT AND UPON COMPLETION AND ACCEPTANCE OF THE WORK REMOVE ALL FENCING FROM THE WORK SITE. PRIOR TO ERECTION OF ANY TEMPORARY PROJECT SAFETY FENCING, COORDINATE WITH FORT BRAGG DPW TRANSPORTATION ENGINEER, RAY GOFF; 910-907.1759 TO CHECK APPROPRIATE TRAFFIC SAFETY SIGHT LINES. INSTALLATION AND LOCATING OF PROJECT SAFETY FENCING SHALL CONSIDER SIGHT TRIANGLES AT INTERSECTIONS, CURVES, AND CONSTRUCTION ENTRANCES. 17. DEVELOP A SITE SAFETY AND HEALTH PLAN (SSHP) TO COVER THE SAFETY AND HEALTH ASPECTS OF THE SUBSURFACE CONTAMINATION DETAILED IN EM 385-1-1, WHICH MAY BE ENCOUNTERED DURING THE EXECUTION OF THIS PROJECT. IN SOME AREAS THE CONTRACTOR MAY ENCOUNTER CONTAMINATED GROUNDWATER WHICH MAY NEED TO BE DEWATERED TO EXECUTE CONSTRUCTION ACTIVITIES. ALTHOUGH THE CONTAMINATION IS IN THE SOIL, VOC VAPORS MAY ALSO BE RELEASED DURING EXCAVATION OF FOOTERS, UTILITY TRENCHES, AND OTHER SUBSURFACE DISTURBING ACTIVITIES. 18. THE WILMINGTON DISTRICT SAFETY AND OCCUPATIONAL HEALTH OFFICE ARE REQUIRED TO REVIEW AND ACCEPT THE SITE SAFETY AND HEALTH PLAN. IN ADDITION TO THE REQUIREMENTS DETAILED IN 385-1-128.A, SUBMIT THE FOLLOWING AS PART OF THE SSHP: CERTIFICATIONS OF HAZARD WASTE SITE TRAINING AND EXPERIENCE, MEDICAL CLEARANCES, AND LICENSES. DO NOT SUBMIT MATERIALS WITH FULL SOCIAL SECURITY NUMBERS OR PERSONAL MEDICAL DATA. BLACK OUT THIS INFORMATION. 19. SURVEY INFORMATION PROVIDED BY THE OWNER IS VERIFIED BY M.A.P.S. SURVEYING, INC., DATED AUGUST 28, 2012. SITE DEMOLITION NOTES: 1. CONTRACTOR SHALL COORDINATE WITH OWNER AND THE UTILITY PROVIDER PRIOR TO THE DISCONNECTING OR REMOVAL OF ANY UTILITY SERVICE TO THE EXISTING BUILDINGS. ALL UTILITIES TO BE REMOVED ARE TO BE CAPPED OR PLUGGED OR TERMINATED ACCORDING TO THE UTILITY OWNERS REQUIREMENTS. 2. ALL DEMOLITION AND CONSTRUCTION DEBRIS SHALL BE TRANSPORTED AND DISPOSED OF AT LEAST WEEKLY IN A LEGAL AND APPROVED MANNER. 6. ALL EXISTING PAVING, CURBS, HARDSCAPE, ETC. SHALL BE SAW CUT AT THE LIMITS OF REMOVAL IN ORDER TO PROVIDE A CLEAN EDGE. EXISTING PAVING AT EDGE SHALL BE MILLED BACK A MINIMUM OF 8" TO ENSURE SMOOTH TRANSITION. 7. ALL NONHAZARDOUS DEBRIS FROM SITE DEMOLITION SHALL BE DISPOSED OF AT FORT BRAGG'S LCID LANDFILL. THE SITE DEMOLITION, IF ANY, SHALL BE DISPOSED OF AT OFF -SITE LOCATIONS AT THE CONTRACTORS EXPENSE. IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TO SECURE ANY REQUIRED PERMITS OR APPROVALS PRIOR TO COMMENCEMENT OF DEMOLITION/REMOVAL OPERATIONS. 8. TOPSOIL SUITABLE FOR RE -USE SHALL BE STOCKPILED ON -SITE IN AN APPROVED LOCATION AS DIRECTED. UNSUITABLE OR EXCESS MATERIAL SHALL BE REMOVED FROM THE PROJECT SITE AT THE CONTRACTOR'S EXPENSE. REFER TO PROJECT SPECIFICATIONS FOR STOCKPILE MAINTENANCE REQUIREMENTS. 9. EROSION CONTROL MEASURES MUST BE IN PLACE PRIOR TO ANY DISTURBANCE. 10. CLEAR AND GRUB ALL VEGETATION WITHIN CONSTRUCTION LIMITS AND REMOVE TREES AS NECESSARY. 11. TREES SHALL BE REMOVED FROM THE SITE AND PROPERLY DISPOSED OF BY THE CONTRACTOR IN APPROVED DISPOSAL AREAS, 12. FULL DEPTH SAW CUT ALL EXISTING PAVEMENT TO BE REMOVED. 13. WATER AND SANITARY SEWER SERVICES ON FORT BRAGG ARE PRIVATIZED THROUGH OLD NORTH UTILITY SERVICES, INC. (ONUS). ONUS WILL ACT AS A PRIME CONTRACTOR (IN A SEPARATE CONTRACT BETWEEN THE GOVERNMENT AND ONUS) FOR DESIGN AND CONSTRUCTION OF EXTERIOR POTABLE WATER, FIRE WATER AND EXTERIOR SANITARY SEWER TO POINTS OF CONNECTION. 14. THE CONTRACTOR SHALL COORDINATE WITH THE BASE UTILITY COMPANY (SANDHILLS) PRIOR TO ANY DEMOLITION AND REMOVAL. THE CONTRACTOR SHALL PROVIDE SANDHILLS WITH A PHASING PLAN AND SCHEDULE IN ORDER TO DISCONNECT AND REMOVE APPLICABLE SERVICES AT THE PROPER TIMES. NO ELECTRICAL UTILITY LINES SHALL BE MODIFIED, REMOVED OR POWER DISCONNECTED TO ANY FACILITY BY THE CONTRACT AT ANY TIME. ALL FACILITIES TO BE REMOVED SHALL BE DISCONNECTED BY SANDHILLS PRIOR TO DEMOLITION IN ORDER TO INSURE POWER IS NOT ACCESSIBLE INSIDE THE FACILITY TO BE DEMOLISHED. THE CONTRACTOR SHALL ADVISE SANDHILLS OF WHICH FACILITIES ARE TO REMAIN IN OPERATION AND SERVICES PROTECTED AND/OR RE-ESTABLISHED. 15. ALL EXISTING IMPROVEMENTS WITHIN THE LIMITS OF CONSTRUCTION ARE TO BE REMOVED OR RELOCATED UNLESS SPECIFICALLY NOTED, "TO REMAIN". BUILDING MATERIAL WASTE HANDLING: 1. ALL WASTES COMPOSED OF BUILDING MATERIALS SHALL BE DISPOSED OF IN ACCORDANCE WITH NORTH CAROLINA GENERAL STATUTES, CHAPTER 130A, ARTICLE 9 - SOLID WASTE MANAGEMENT, AND RULES GOVERNING THE DISPOSAL OF SOLID WASTE (NORTH CAROLINA ADMINISTRATIVE CODE SECTION 15A NCAC 13B). 2. LOCATE AREAS DEDICATED FOR MANAGEMENT OF LAND CLEARING AND DEMOLITION DEBRIS, CONSTRUCTION AND DOMESTIC WASTE, AND HAZARDOUS OR TOXIC WASTE. THIS LOCATION SHALL BE AT LEAST 50 FEET AWAY FROM STORM DRAIN INLETS AND SURFACE WATERS UNLESS IT CAN BE SHOWN THAT NO OTHER ALTERNATIVES ARE REASONABLE AVAILABLE. 3. DUMPING OF PAINT AND OTHER LIQUID BUILDING MATERIAL WASTES IN STORM DRAINS IS PROHIBITED. 4. LITTER AND SANITARY WASTE - THE PERMITTEE SHALL CONTROL THE MANAGEMENT AND DISPOSAL OF LITTER AND SANITARY WASTE FROM THE SITE. 5. LOCATION OF STOCK PILES - LOCATE EARTHEN -MATERIAL STOCK PILE AREAS AT LEAST 50 FEET AWAY FROM STORM DRAIN INLETS AND SURFACE WATERS UNLESS IT CAN BE SHOWN THAT NO OTHER ALTERNATIVES ARE REASONABLE AVAILABLE. 6.HANDLING OF CONCRETE - CONCRETE MATERIALS ONSITE, INCLUDING EXCESS CONCRETE, MUST BE CONTROLLED AND MANAGED TO AVOID CONTACT WITH SURFACE WATERS, WETLANDS OR BUFFERS. NO CONCRETE OR CEMENT SLURRY SHALL BE DISCHARGED FROM THE SITE. (NOTE THAT DISCHARGES FROM ONSITE CONCRETE PLANTS REQUIRE COVERAGE UNDER A SEPARATE NPDES PERMIT - NCG140000.) ANY HARDENED CONCRETE RESIDUE WILL BE DISPOSED OF, OR RECYCLED ON SITE, IN ACCORDANCE WITH LOCAL AND STATE SOLID WASTE 'REGULATIONS. LAYOUT NOTES: 1. THE CONTRACTOR SHALL TAKE ALL NECESSARY MEASURES TO PROTECT ADJACENT PROPERTIES AND IS RESPONSIBLE FOR ANY DAMAGE TO EXISTING IMPROVEMENTS ON OR OFF SITE DUE TO THE CONSTRUCTION OF THIS PROJECT. ANY DAMAGE WILL BE REPAIRED AT THE CONTRACTOR'S EXPENSE. 2. CONTRACTOR SHALL VERIFY SITE BOUNDARY AND EXISTING TOPOGRAPHY. NOTIFY THE ARCHITECT OF ANY DISCREPANCIES PRIOR TO SUBMITTING PRICES OR ORDERING MATERIALS. 3. IT IS THE CONTRACTOR'S RESPONSIBILITY TO PROTECT ALL BENCHMARKS AND PROPERTY CORNERS. ANY REPLACEMENT WILL BE AT THE CONTRACTOR'S EXPENSE. 4. IT IS THE RESPONSIBILITY OF THE CONTRACTOR TO OBTAIN ALL NECESSARY PERMITS REQUIRED TO CONSTRUCT THIS PROJECT AND PAY ALL PERMIT FEES. ALL PERMITS MUST BE IN -HAND PRIOR TO CONSTRUCTION. 5. ALL ACCESSIBLE RAMPS, SIGNS, SYMBOLS, AND PAINTED ISLANDS AND ACCESS ROUTES MUST CONFORM TO THE LATEST ABA REQUIREMENTS. 6. THE MAXIMUM SLOPE INACCESSIBLE PARKING AREAS SHALL NOT EXCEED 2.0% GRADE IN ANY DIRECTION. SLOPE IN THE DIRECTION OF TRAVEL IN ALL HANDICAP ACCESS ROUTES SHALL NOT EXCEED 5.0% GRADE AND 2.0% CROSS SLOPE. 7. ALL DIMENSIONS AND COORDINATES SHOWN ARE TO THE OUTSIDE FACE OF BUILDING, TO THE BACK OF CURB, OR TO THE EDGE OF SURFACING UNLESS OTHERWISE NOTED. THIS SITE CONSTRUCTION TRAFFIC CONTROL PLAN SHALL BE IN ADDITION TO ANY TRAFFIC CONTROL PLAN PROVIDED IN THE PLAN SET FOR ROADWAY IMPROVEMENTS. 10. CONTRACTOR IS RESPONSIBLE FOR ADJUSTING ELEVATIONS OF ALL AT -GRADE STRUCTURES AND UTILITIES TO REMAIN (VALVE BOXES, MANHOLES, INLETS, VAULTS, ETC) TO MATCH PROPOSED FINISHED GRADES. 11. HINGE POINTS OF BOTH BUTTRESSES SHALL EACH CONTAIN A LOCKING PIN WITH PADLOCK ACCEPTANCE FOR SECURING THE CRASH GATE IN THE CLOSED POSITION. THE CRASH GATE SHALL WITHSTAND A 10,000 POUND VEHICLE AT IMPACT SPEED OF 50 MPH, WITH MAXIMUM GATE DEFLECTION OR VEHICLE PENETRATION OF 10 FEET, OR CERTIFIED AT A K12 MINIMUM RATING. 13. BARRIER ARM SURFACE SHALL HAVE A RUST INHIBITING PAINTED SURFACE AND SHALL BE FURNISHED WITH 4-INCH WIDE REFLECTIVE PAINT SPACED AT EVERY 20 INCHES. 14.
