Typically, for a Phase II, the prospective buyer would be the well owner and the environmental consultant would be the Contact Person on the form. Send all suspicious email as an attachment to report.spam@nc.gov Hi Andrew, I'd like to request monitoring well approval for a soil/groundwater investigation that is scheduled next Wednesday, July 8, at a vacant gas station that is under contract in Lake Toxaway, NC. The monitoring well application, site map, and well schematic is attached for your review. As mentioned on the phone, my client (the bonafide prospective purchaser) and I have requested the property owner's signature for the application, and I hope to have it later this week. Although we don't yet have the owner's signature for the form, I'd like to get the application into the system now so that the field investigation may proceed on 7/8.
