Inspection Summary:
On June 24, 2020, Zac Lentz and Paul Clark met with Kumar Letchumanan (Fibertex) and Paul Spangenberg (Mid Atlantic)
to inspect the Fibertex facility in order to evaluate the site following a General Permit NOI Application.The inspection
revealed no discrepancies with the application or housekeeping issues. The SWPPP was received via email on June 29,
2020. No discrepancies with the application, housekeeping issues,or shortcoming with the SWPPP have been observed. As
such, it has been recommended to the application reviewer that the application be approved.

accurate and adequate.
