The Division of Water Resources (Division) acknowledges receipt of your 2019 annual report. A
review of this report was conducted by Division staff person Patrick Mitchell. Review of the
subject annual report reflects compliance with Permit No. WQ0003796.
As a reminder, Permit Condition III. 3. requires that the Winston-Salem Regional Office (WSRO)
shall be notified at least 24 hours in advance of any land application event. Please ensure that the
WSRO is notified prior to the next land application event.
An announced routine facility compliance inspection is planned to be scheduled within the next
