The assistance and cooperation -of Chris Marion, Operator in Responsible Charge (ORC), was greatly appreciated. An inspection checklist is attached for your records and inspection findings are summarized below. General Information The WWTP is located on the west side of the school, in Winston-Salem, Forsyth County, NC, at approximately coordinates 36.205729°N,-80.389910°W. The permit authorizes the Winston-Salem/Forsyth County Board of Education to operate this 0.0064 MGD WWTP, which consists of!' : grease-trap;�ya 15,000 gallon septic tank, a 3,000 gallon dosing, chamber with twin 5-inch siphons, twin recirculating san`l'filters, a 10,000 gallon recirculation tank, a recirculation controller, twin 50 gallon per minute effluent pumps, ultraviolet disinfection and cascade aeration, and discharge the treated effluent via outfall 001 into an unnamed tributary (UT) to Fries Creek, which is currently classified as Class WS-IV waters in the Yadkin Pee -Dee river basin. . Site Review The plant was not in operation during the inspection because school was out and minimal wastewater was being produced. Mr. Marion appears to have done a good job of operating and maintaining the plant. However, the filter bed walls are badly deteriorated and look as though they may fail. The school may need to look at repairing the walls at some point in the near future. This was also noted during the previous inspection. No discrepancies were noted. Mr. Marion has very good records and all required records were available on site, complete, and well organized; this includes operations and visitation logs, discharge monitoring reports, laboratory and field laboratory records, and chains of custody, etc. A new operator designation form does need to be submitted to the Division as soon as possible to designate Mr. Marion as the new ORC, and other individual(s) as backup ORCs You are reminded that violations of the permit are subject to enforcement actions by the Division of Water Resources. Violations are subject to civil penalties not to exceed $25,000 per day, per violation. The following is a statement from R&A Labs, certified operator for the wastewater treatment system at Old Richmond Elementary School. Re: Forsyth County Schools (Old Richmond) — High Ammonia, February 2014 High ammonia due to freezing weather conditions, system had a high flow and was not able to recirculate the water properly. Forsyth,County Dear Mr. Smith: Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Resources (DWR or the Division) conducted a compliance evaluation inspection (CEI) of the Old Richmond Elementary School Wastewater Treatment Plant (WWTP) on July. 1st, 2014. The assistance and cooperation of Chris Marion, Operator in Responsible Charge (ORC), was greatly appreciated. An inspection checklist is attached for your records and inspection findings are summarized below. .General Information The WWTP is located on the west side of the school, in Winston-Salem, Forsyth County, NC, at approximately coordinates 36.205729°N,-80.3899100W. The permit authorizes the Winston-Salem/Forsyth County Board of Education to operate. this 0.0064 MGD WWTP, which consists of a grease trap, a 15,000 gallon septic tank, a 3,000 gallon dosing chamber with twin 5-inch siphons, twin recirculating sand filters, a 10,000 gallon recirculation tank, a recirculation controller, twin 50 gallon per minute effluent pumps, ultraviolet disinfection and cascade aeration, and discharge the treated effluent via outfall 001 into an unnamed tributary (UT) to Fries Creek, which is currently classified as Class WS-IV waters in the Yadkin Pee -Dee river basin. Site Review The plant was not in operation during the inspection because school was out and minimal wastewater was being produced. Mr. Marion appears to have done a good job of operating and maintaining the plant. However, the filter bed walls are badly deteriorated and look as though they may fail. The school may need to look at repairing the walls at some point in the near future. This was also noted during the previous inspection. The practice at this plant is to have two separate meters and have one installed while the other is being calibrated by the manufacturer. Mr. Marion explained that the manufacturer was taking a longer than normal time to calibrate the meter and was unsure of its current status. Because there is low or no flow at the school right now, this should suffice as long as the newly calibrated meter can be received and installed as flow starts to pick up and school begins in August. Please remember that a properly calibrated flow meter is a requirement of your permit. Documentation Review All documentation was reviewed. No discrepancies were noted. Mr. Marion has very good records and all required records were available on site, complete, and well organized; this includes operations and visitation logs, discharge monitoring reports, laboratory and field laboratory records, and chains of custody, etc. A new operator designation, form does need to be submitted to the Division as soon as possible to designate Mr. Marion as the new ORC, and other individual(s) as backup ORCs You are reminded that violations of the permit are subject to enforcement actions by the Division of Water Resources. Violations are subject to civil penalties not to exceed $25,000 per day, per violation. Please respond to this letter in writing within 30 days of receiving it. Your reply should explain what you plan to do about the filter walls and keeping the flow meter properly calibrated, as well as address the outdated ORC designation form. The assistance and cooperation of Ryan Smith, Operator in Responsible Charge (ORC), was greatly appreciated. General Information The WWTP is located on the west side of the school, in Winston-Salem, Forsyth County,.NC, at approximately coordinates 36.205729°N,-80.389910°W. The permit authorizes the Winston-Salem/Forsyth County Board of Education to operate this 0.0064 MGD WWTP, which consists of a grease trap, a 15,000 gallon septic tank, a 3,000 gallon dosing chamber with twin 5-inch siphons, twin recirculating sand filters, a 10,000 gallon recirculation tank, a recirculation controller, twin 50 gallon per minute effluent pumps, ultraviolet disinfection and cascade aeration, and discharge the treated effluent via outfall 001 into an unnamed tributary (UT) to Fries Creek, which is currently classified as Class WS-IV waters in the Yadkin Pee -Dee river basin. Site Review Mr. Smith has done an excellent job of operating and maintaining the plant. It is in very good condition and Mr. Boone noted no discrepancies. General Information The WWTP is located on the west side of the school, in Winston-Salem, Forsyth County, NC, at approximately coordinates 36.205729°N,-80.3899100W. The permit authorizes the Winston-Salem/Forsyth County Board of Education to operate this 0.0064 MGD WWTP, which consists of a grease trap, a 15,000 gallon septic tank, a 3,000 gallon dosing chamber with twin 5-inch siphons, twin recirculating sand filters, a 10,000 gallon recirculation tank, a recirculation controller, twin 50 gallon per minute effluent pumps, ultraviolet disinfection and cascade aeration, and discharge the treated effluent via outfall 001 into an unnamed tributary (UT) to Fries Creek, which is currently classified as Class WS-IV waters in the Yadkin Pee Dee River basin. Site Review Mr. Marion appears to have done an excellent job of operating and maintaining the plant. It is in very good condition and Mr. Boone noted no discrepancies. The sand filter beds were in good condition and nearly all spray heads were fully operational., Mr. Marion knew the system very well and was able to explain its operation in detail. Dear Mr. Smith: Enclosed please find a copy of the Compliance Evaluation Inspection form from the inspection conducted on April 22, 2009. The Compliance Evaluation Inspection was conducted by Rose Pruitt of the Winston-Salem Regional Office. The facility was found to be in Compliance with permit NC0034827. Kevin Booe, ORC was present for the inspection. The inspection consisted of two parts: an on -site, inspection of the treatment facility . and a file review. The following are the findings from the subject inspection. I. Permit The NPDES permit for the Old Richmond Elementary School WWTP became effective February 1, 2009 and expires on January 31, 2014. The permitted components of the 0.0064 MGD wastewater treatment plant include: Grease trap, 15,000 gallon septic tank, 3,000 gallon dosing chamber with twin 5", siphons, twin recirculating sand filters, 10,000 gallon recirculation tank, ultraviolet (UV) disinfection; and cascade re -aeration. The facility is located near Tobaccoville at the, Old Richmond Elementary School off Tobaccoville Rd at Highway 67 in Forsyth County.: " H. Records/Reports A review of the laboratory reports and Discharge Monitoring Reports (DMRs) for the Old Richmond Elementary School WWTP for the period January 2008 through December 2008 revealed that the facility had no limit violations. Operations records include all sample analyses and process control tests that are performed. III. Facility Site Review The facility site review indicated that the 0.0064 MGD treatment works is consistent with the permitted components. The actual treatment system consists of Grease trap,15,000 gallon septic tank, 3,000 gallon dosing chamber with twin 5" siphons, twin recirculating sand filters, 10,000 gallon recirculation tank, ultraviolet (UV) disinfection, and cascade reaeration. IV. Effluent / Receiving Stream The WWTP discharges to an unnamed tributary to Fries Creek (WS-IV waters in the Yadkin — Pee Dee River Basin). The effluent was clear and free of visible solids on the date of inspection. Flow Measurement Effluent flow is measured with a Master Meter. The meter was calibrated at the factory on September 25, 2008. This meter is switched out annually with another meter and sent to the factory for calibration. VI. Self -Monitoring Program A review of the discharge monitoring reports (DMRs) for the time period of January 2008 through December 2008 demonstrated that the Old Richmond Elementary School WWTP had no effluent violations. VII. Laboratory All of the sample analyses are conducted by R & A Labs, Certification 934. The laboratory was not reviewed at the time of the subject inspection. The field parameter certification 45299 for this facility is current but decertification has been requested by the facility as contract operations will begin May 1, 2009. VIII.Operation and Maintenance Operation and maintenance at the time of the subject inspection was deemed satisfactory. The effluent was clear and free of visible solids. The inspector notes that Kevin Booe the current ORC will be replaced by a contract lab (R&A) on May 1, 2009 IX. Sludge Utilization/Disposal Solids are removed from the WWTP as necessary by a licensed contract hauler and disposed of properly. The septic tank had last been pumped down by Transou Septic tank Service on August 18, 2008. The Division of Water Quality greatly appreciates your continued oversight at this facility. Kevin Booe, ORC was present for the inspection. The inspection consisted of two parts: an on - site inspection of the treatment facility and a file review. The following are the findings from the subject inspection. I. Permit The NPDES permit for the Old Richmond Elementary School WWTP became effective July 1, 2004 and expires on January 31, 2009. The permitted components of the 0.0064 MGD wastewater treatment plant include: Grease trap, 15,000 gallon septic tank, 3,000 gallon dosing chamber with twin 5" siphons, twin recirculating sand filters, 10,000 gallon recirculation tank, ultraviolet (UV) disinfection, and cascade re -aeration. . II. Records/Reports A review of the laboratory reports and Discharge Monitoring Reports (DMRs) for the Old Richmond Elementary School WWTP for the period January 2007 through December 2007 revealed that the facility had two limit violations, a monthly average violation of NH3 as N and a daily pH violation. Operations records include all sample analyses and process control tests that are performed. The electronic flow meter was calibrated on September 17, 2007 by Master Meter. The actual treatment system consists of Grease trap, 15,000 gallon septic tank, 3,000 gallon dosing chamber with twin 5" siphons, twin recirculating sand filters, 10,000 gallon recirculation tank, ultraviolet (UV) disinfection, and cascade reaeration. IV. Effluent / Receiving Stream The WWTP discharges to an unnamed tributary to Fries Creek (WS-IV waters in the Yadkin — Pee Dee River Basin). The effluent was clear and free of visible solids on the date of inspection. The receiving stream was free of solids, foam and debris. V. Flow Measurement Effluent flow is measured with a Master Meter. The meter was calibrated at the factory on September 17, 2007. This meter is switched out annually with another meter and sent to the factory for calibration. VI. Self -Monitoring Program A review of the discharge monitoring reports (DMRs) for the time period of January 2007 through December 2007 demonstrated that the Old Richmond Elementary School WWTP had two effluent violations. The first violation was for exceeding the monthly average in February 2007 for NH3 as N, an enforcement action was initiated for this violation. The second violation received no further action and was a daily violation of pH in August 2007. VII. Compliance Schedules No compliance schedules to evaluate. VIII. Laboratory All of the sample analyses are conducted by R & A Labs, Certification #34. The laboratory was not reviewed at the time of the subject inspection. The field parameter certification #5299 for this facility is current. IX. Operation and Maintenance Operation and maintenance at the time of the subject inspection was deemed satisfactory. The effluent was clear and free of visible solids. X. Sludge Utilization/Disposal Solids are removed from the WWTP as necessary by a licensed contract hauler and disposed of properly. The septic tank had last been pumped down by Transou in July 2004. XI. Pretreatment Not evaluated during this inspection. No pretreatment program required. II. GENERAL COMMENTS: The laboratory is spacious and well equipped. All facilities and instrumentation are well maintained. III. FINDINGS, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS: No findings were noted during the inspection. IV. PAPER TRAIL INVESTIGATION: The paper trail consisted of comparing field testing records and contract lab reports to Discharge Monitoring Reports (DMRs) submitted to the North Carolina Division of Water Quality for the months of January, February and March 2008. No transcription errors were detected. The facility appears to be doing a good job of accurately transcribing data. V. CONCLUSIONS: No findings were noted during the inspection.
