Dear Mr. Bollinger:
OIn accordance with your application received May 14, 1987, we are forwarding
herewith Permit No. 7647R, dated October 21, 1987, to Perdue, Inc., for the continued
operation of the subject non -discharge type waste treatment facilities.
This permit shall be effective from the date of issuance until October 1, 1992,
shall void Permit No. 7647, issued October 6, 1982, and shall be subject to the
conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must be
in the form of a written petition, conforming to Chapter 150E of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made, this
permit shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a 17,900 GPD non --discharge type wastewater treatment facility
consisting of two 8400-gallon capacity septic tanks in series, a 1200 gallon septic
tank, approximately 700 linear feet of 6-inch gravity sewer, a 1.5 MG treatment and
storage lagoon, a spray pump station with dual pumps, approximately 4000 linear feet
of 4-inch, 6-inch, and 8-inch force main and sprinkler piping, approximately 6.0
acres of spray irrigation area, and all associated piping, valves and appurtenances
to serve Perdue, Inc.'s Hatchery No. 9 with no discharge of wastewater to the surface
waters of the State, pursuant to the application received May 14, 1987, and in
conformity with the project plan, specifications, and other supporting data
subsequently filed and approved by the Department of Natural Resources and Community
Development and considered a part of this permit.
This permit shall be effective from the date of issuance until October 1, 1992,
shall void Permit No. 7647, issued October 6, 1982, and shall he subject to the
following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
. S. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes and may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will protect the assigned
water quality standards of the surface waters and groundwaters.
6. In event the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional or
replacement wastewater treatment or disposal facilities.
7. Solids, sludges, or other pollutants removed or resulting from the wastewater
storage facilities shall be contained and disposed of in such a manner as to
prevent any contamination of the surface or groundwater of the State.
8. The Permittee shall employ a certified wastewater treatment plant operator in
responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission.
9. Diversion or bypassing of the untreated wastewater from the treatment facilities
is prohibited.
10. A suitable year round vegetative cover shall be maintained.
11. Irrigation shall not be performed during inclement weather or when the ground
is in a condition that will caused runoff.
12. Dikes shall be constructed, as needed, so as to prevent surface water runoff
. from the spray field.
13. The application rates shall not exceed:
a. An annual average of 0.8 iniaciwk.
b. 400 lbs PAN/acryr on the grassed irrigation area
C. 100 lbs PAN/aciyr on the wooded irrigation area
14. An annual report shall be submitted to the Raleigh Regional office which
includes the following:
a. date of irrigation event
b. volume of wastewater irrigated
c. continuous weekly, monthly, and year-to-date hydraulic (inches per acres and
nutrient (pounds PAN per acre) loadings.
15. The sludge generated form these treatment facilities must be disposed of in
accordance with General Statute 143.215.1 and in a manner approvable by the
North Carolina Division of Environmental Management.
16. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation
of this facility.
17. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
• review the adequacy of the facilities described therein, and as indicated, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
18. Within 90 days of permit issuance, two (21 additional monitor wells, one (1)
upgradient and one (1) downgradient must be installed to monitor groundwater
quality at the spray irrigation site. The downgradient monitor well shall be
located no more than 50 feet from the perimeter of the wastewater disposal
system. The location and construction details for these wells must be approved
by the Raleigh Regional Office from which a well construction permit must be
obtained.
19. The two (2) additional monitor wells and the one existing well at the spray
irrigation system site shall be sampled every March, July and November for the
following parameters:

Fecal Coliforms
Water Level
Chloride
PH
BOD
Orthophosphate
NH4
The measurement of water level must be made prior to sampling for the remaining
parameters.
The results of the sampling and analysis shall be sent to the N. C. Division of
Environmental Management on Form GW-59 (Compliance Monitoring Report Form).
20. Any additional groundwater quality monitoring, as deemed necessary by this
• Division, shall be provided.
