This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Collection System Permit No. WQCSD0557. A summary of the findings and comments noted during the inspection are provided in the enclosed copy of the inspection report. The Collection System Non -sampling inspection was conducted by Division of Water Resources staff from the Asheville Regional Office. The following .violation(s) were noted during the inspection: Inspection Area Description of Violation Pump Station One of the pumps was not working. Looking through the pump station log, it appears the pump has not run since June 15 at least, and that the pump was also not functioning March 17-May 22. In addition, the issues below must also be addressed: Compliance Issue(s): The Cranberry Pump Station control boxes are corroded. A manhole on Elk River Rd had cracked encasement concrete and was hidden by weeds. Maintenance records were missing. High priority lines need to be identified, and inspections of them need to be documented every six months. The Division of Water Resources may pursue enforcement action for this and any additio State law. I met with backup ORC, Otis Ward, who assisted with the inspection. Documentation was sufficient in some areas and lacking in others. Pump station inspection documentation could be more complete. Right of way maintenance and high priority line inspections should be documented. Sewer line cleaning, repair, and other maintenance should be documented in a log book. Pump station #1, Dark Hollow, had only one operating pump. Review of the log book revealed that this station has been operating with only one functional pump for almost 3 of the last 4 months. Otis said that a functional backup pump is on hand and ready to be installed. Please install that pump immediately, and going forward do not allow pump stations to run with only one functioning pump for more than 24 hours past discovery of the situation. Pump station #2, Cranberry, has a lot of rust on the control boxes. I recommend refurbishment to protect the controls. A canopy would offer some protection for the control boxes and perhaps reduce corrosion. One other recommendation is for a low float alarm. In the event that the shut off float for the pumps stops working, they could keep running even when the wet well is pumped down and the pumps are dry. This could damage the pumps. A low float alarm could alert an operator of this condition and perhaps save the pumps. A manhole Otis and I inspected along Elk River Rd needs the encasing concrete repaired. This manhole was hidden by tall weeds. Manholes need to be kept visible and accessible. Please keep records of maintaining the right of ways. The Mayor was also present for some of the inspection. System is, fairly new; 1996 with 204 customers in town and 18 out of town. The system is smoke tested yearly. Personnel stated right of ways are accessible if need be. Both pump stations need work immediately. The town will respond back to the inspector with documentation of the work completed. Documentation of work completed by the town was received in the Asheville Office on March 5, 2014. An itemized list of work as well as photo and video documentation were sent verifying work completed. Both pump/lift stations were cleaned and pumped. The alarm systems were repaired and now work. Both had simple repairs needed. It is suggested that these station be on a pumping schedule depending on fats/debris buildup. On 2/21/2014 the inspector emailed the ORC the Operational Log Package which consists of various forms related to collection systems. Overall the program is well managed. These were pump station maintenance issues that needed immediate attention; this will be an ongoing demand of the ORCs and towns time and resources. Another helpful ongoing maintenance practice of a collection system is cleaning; if the the town plans to clean their system they should consider the feasibility of purchasing a jetter system vs contracting this service out. In March 2000, the North Carolina Environmental Management Commission enacted rules related to sewage collection systems. These rules are now found in the North Carolina Administrative Code 15 NCAC 2T .0403 for deemed permitted facilities. They include facilities with a privately -owned collection system that serve condominiums, apartments, mobile home parks, subdivisions, shopping centers, colleges, etc. that are tributary to larger municipally owned systems, yet are not owned or managed by the municipality. If you are aware of any such privately -owned collection systems, we would also like to be informed of their existence. In addition, we would like to know where any privately -owned pump stations are located that are connected to your collection system. The type of information we are requesting is the facility name, location and contact person. If that system is in a state of disrepair or if you have received complaints about it previously, please note that as well. Your attention to this request is most appreciated. Over a period of time the manhole collected some grit & grease, therefore causing it to backup and over flow .There was about 1" of water running out of the manhole for approximately 2 hours. Water had run out down the road about 500 ft and into a ditch. I had been down to the plant earlier and there was no SSO at that time. Considering the time that it could have started and the time Rotor Rooter jetted the line, I could only estimate the amount of spill. Given the amount of time that had passed 600 gallons would be a high estimate, in my opinion. Since this occurred I have regularly checked this manhole for grease,grit,etc. I have also rodded the main from this manhole to the plant to reduce chance of blockage again. It had been about a year since I had smoke tested the system, which I do annually. At that time there were no problems with this manhole. The town does have a grease ordinance for grease traps. I have records when each one of these has been pumped. I have not sent out any information or literature concerning grease. I plan to put an ad in the local paper concerning FOG (Fats, Oils, Grease). It will appear in the local newspaper next week. In my previous response to this SSO I estimated the spill without using the estimation table. Noted below could be a more accurate figure using the estimation table chart. 1. The Town of Elk Park determines the volume of a spill by using 7.2 Flow Estimation Tables. A 24" Manhole Cover with 1" flow equals to 6 gpm & for 2 hours equals to 1,080 gals. Our Manhole Cover is 22" in Diameter; therefore my estimate would be between 600 & 800 gals spilled Maximum, in my opinion. 2. The Town of Elk Park has a Sewer Use Ordinance that includes a FOG clause. I have enclosed a copy that was sent out recently. 3. The line was last cleaned about a year ago. 4. Public education information for grease was last sent out on June 22, 2011.1 have also enclosed a copy of the information that was advertised in the local newspaper. Guinn Michael Smith Joel Whitley To Whom It May Concern: On May 4" we had a call that.water was running out of a manhole. This happens to be the last one going to the W.W.Plant. The manhole is the smallest one as in depth. Over a period of time the manhole collected some grit & grease, therefore causing it to backup and over flow .There was about V of water running out of the manhole for approximately 2 hours. Water had run out down the road about 500 ft and into a ditch. I had been down to the plant earlier and there was no SSO at that time. Considering the time that it could have started and the time Rotor Rooter jetted the line, I could only estimate the amount of spill. Given the amount of time that had passed 600 gallons would be a high estimate, in my opinion. Since this occurred I have regularly checked this manhole for grease,grit,etc. I have also rodded the main from this manhole to the plant to reduce chance of blockage again. It had been about a year since I had smoke tested the system, which I do annually. At that time there were no problems with this manhole. The town does have a grease ordinance for grease traps. I have records when each one of these has been pumped. I have not sent out any information or literature concerning grease. I plan to put an ad in the local paper concerning FOG.(Fats, Oils, Grease). It will appear in the local newspaper next week. This plan was developed to ensure that key personnel respond to every spill event within sixty (60) minutes of a reported event. INITIAL RESPONSE: Spills reported to or detected by collection system personnel require immediate attention. The Collection System ORC and / or Back-up Operator will make the preliminary assessment of the spill within 15 minutes after receiving report. That assessment includes visiting the spill site. The ORC or Back-up Operator will determine the appropriate response and request assistance from key personnel, as necessary. The contact list will be updated annually and include essential response equipment and personnel. Local contractors may be employed, as deemed necessary. Key personnel will, upon assessing the site, continue to document the event and determine the severity, magnitude and potential environmental impact. Every effort should be made to minimize the spill event consistent with the SOURCE CONTROL AND CONTAINMENT PROCEDURES section of this plan. The safety of all response personnel should never be compromised. Key personnel should implement recovery actions to eliminate the cause of the obstruction and / or system failure as outlined in this plan. POST SPILL RESPONSE (REPORTING): After containment is achieved, the Division of Water Quality, (DWQ) must be contacted as soon as possible but, in no case later than 24 hours after initial detection if; 1) the spill has reached surface waters, "waters of the State" or 2) the spill exceeds one thousand gallons, (1,000 gallons) Spills that are fully contained and removed quickly and are less than one thousand gallons are not required to be reported to DWQ. However, a full spill report form will be filled out and kept on file. All spills must be logged in, regardless of size, and include the incident date, time, duration, approximate volume and name of respondent(s). SOURCE CONTROL AND CONTAINMENT PROCEDURES: Depending on the nature of the spill or overflow, the initial assessment should indicate the priority of source control and/or containment. 1) When the source can be isolated through valves, plugs or other methods, the flow should be discontinued immediately or reduced to the least possible volume. 2) If the pumps on the location are not operating the control panel should be checked as follows: a. Is the Power on? b. Check the breaker. c. If the breaker is closed, check the thermal overloads (external button). d. Is the overload good? Check the fuses, if you are trained to do so. e. Is the pump straining or clogged with debris? f. Do you smell a burnt motor? (contact from attached list) g. Is an electrician needed? (contact from attached list) 3) Containment methods will typically include; a. Sand or dirt dikes and dams (for small streams & creeks). b. Holes or pits (to prevent run off to surface waters). If the spill event resulted from an area wide power outage, you should contact Power Company and identify your site as critical. (Use the account number on the attached list.) If the problem is mechanical in nature, retrieve parts from inventory and make repairs. Re -order parts as they are used. If the problem results from a sewer blockage, call for a jet-rodding machine (contact contractor from attached list). SPILL RESPONSE PROCEDURES FOR SPILLS OCCURRING: ON MAJOR INTERCEPTOR / RIGHT OF WAY 1. Put up warning tape ("DANGER / CAUTION - DO NOT ENTER") around the SSO site to restrict access. 2. In open areas, remove debris and lime site. Rotor -till, if needed. Re -seed and mulch with straw. 3. In areas where there is under growth and small trees, flush area with potable water until no waste remains. Wait twelve to twenty four hours and then lime the area. Be certain that lime does not flush to the receiving stream. NOTE: DO NOT LIME, IF RAIN IS FORECASTED. IN CITIZEN'S YARD 1. Till entire affected area, if possible. Apply lime and follow with seed and straw. 2. If waste/debris is not visible, apply lime and 2 inches of top soil and mix thoroughly. Apply lime lightly, seed and mulch. ON ASPHALT OR CONCRETE 1. Set up containment at or near the storm drainage system. Do not let the spill enter the storm drain system. 2. Put up warning tape ("DANGER / CAUTION - DO NOT ENTER") around the SSO area. 3. Spread lime on spillage and sand or apply other material to a. depth of one inch. Let this material absorb for one hour, then sweep up. Dispose of debris appropriately. If spill is still evident, repeat application. 4. After removing the contaminated material, wash down area with high pressure water and bleach. 5. Pump the clean up materials and wash water into sanitary sewer. IN A CREEK OR STREAM 1. Take conductivity readings of the receiving stream upstream and downstream to determine where a temporary dam should be placed. 2. The dam should be constructed near a manhole which is accessible to the jet rodder and vacuum truck. 3. The dam should be placed below the spill as indicated by low conductivity readings. 4. Construct a dam sized to contain the flow of water, plus any flush water added during the clean up. 5. Station pumps at the clean-up site, one to use, one for backup. 6. Have a back hoe at the site for dam repairs as needed. 7. Wash down all of the rip rap along the stream bank until no residue is evident. 8. Put up warning tape ("DANGER / CAUTION - DO NOT ENTER") at spill site and 400 feet downstream. 9. Take upstream and downstream samples for fecal coliform. 10. Repeat sampling until normal, background counts are 200 fecal coliform colony forming units/100 mi. 11. If settled sludge is evident, sweep bottom until clean. 12. If creek or stream is recreational waters (swimming), notify the affected community and local officials, as soon as the problem is discovered. This permit shall be effective only with respect to the nature and volume of wastes described in the application and other supporting data. The wastewater collection facilities shall be properly maintained and operated at all times. The Permittee shall maintain compliance with an individual system -wide collection system permit for the operation and maintenance of these facilities as required by 15A NCAC 2T .0403. If an individual permit is not required, the following performance criteria shall be met as provided in 15A NCAC 2T .0403: The sewer system shall be effectively maintained and operated at all times to prevent discharge to land or surface waters, and any contravention of the groundwater standards in 15A NCAC 2L .0200 or the surface water standards in 15A NCAC 213 .0200. b. A map of the sewer system shall be developed and shall be actively maintained. c. An operation and maintenance plan shall be developed and implemented. Pump stations that are not connected to a telemetry system shall be inspected every day (i.e. 365 days per year). Pump stations that are connected to a telemetry system shall be inspected at least once per week. e. High -priority sewer lines shall be inspected at least once per every six-month period of time. f. A general observation of the entire sewer system shall be conducted at least once per year. g. Inspection and maintenance records shall be maintained for a period of at least three years. Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC 213 .0506(a), and public notice shall be provided as required by North Carolina General Statute §143-215.1C. In the event there is a desire for the wastewater collection facilities to change ownership, or there is a name change of the Permittee, a formal permit request shall be submitted to the Division accompanied by documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request shall be considered on its merits and may or may not be approved. 5.. Construction of the gravity sewers, pump stations, and force mains shall be scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State. 6. Upon completion of construction and prior to operation of these permitted facilities, a certification, a copy of the construction record drawings, as well as supporting design calculations for any pump stations permitted as part of this project shall be received from a North Carolina -licensed Professional Engineer certifying that the facilities have been installed in, accordance with this permit; 15A NCAC 2T .0200; the Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Main adopted June 1, 2000 as applicable; and other supporting materials. If this project is to be completed in phases and partially certified, you shall retain the responsibility to track further construction approved under the same permit, and shall provide a final certificate of completion once the entire project has been completed. A copy of the construction record drawings, indicating the facilities constructed in the phase being certified, shall be submitted with each partial certification. Mail the Engineer's Certification, one copy of the "Construction Record Drawings," and one copy of the supporting design calculations to the Non -Discharge Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617. 7. A copy of the construction record drawings shall be maintained on file by the Permittee for the life of the wastewater collection facilities. 8. Failure to abide by the conditions and limitations contained in this permit; 15A NCAC 2T .0200; the Division's Gravity Sewer Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump Station and Force Mains adopted June 1, 2000 as applicable; and other supporting materials may subject the Permittee to an enforcement action by the Division, in accordance with North Carolina General Statutes §143- 215.6A through §143-215.6C. 9. In the event that the wastewater collection facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement facilities. 10. The issuance of this permit shall not exempt the Permittee from complying with any and all statutes, rules, regulations, or ordinances that may be imposed by other government agencies (local, state and federal) which have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 2B .0200, erosion and sedimentation control requirements in 15A NCAC Ch. 4 and under the Division's General Permit NCG010000, and any requirements pertaining to wetlands under 15A NCAC 213.0200 and 15A NCAC 2H .0500. 11.
