The plans must include a general location map, a plan view of the sewer extension, a profile of the sewer extension, and must show the proximity of the sewer extension to other utilities and natural features. Specifications may be omitted for delegated authorities. Each sheet of the plans and the first page of the specifications must be signed and sealed. Three copies of the existing permit if a renewal or modification. C. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5). d. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting the wastewater flow from this project is different from the applicant of the project, then a letter must be provided from the owner/authority of the WWTF specifying the volume of flow that will be accepted. The letter should be a recent letter and should refer to the project by the same name as that identified on the application and the plans/specifications. e. The following items must be addressed before we can complete our review: 1) On February 1, 1993, the Division adopted new regulations that require that the infiltration/exfiltration rate must not exceed 100 GPD per inch of pipe diameter -per mile of pipe. All sewer extension permit applications received on or after February 1, 1993, must conform to these new regulations. Please resubmit three revised, signed and sealed copies of page 4 of the application form and page 6 of your specifications reflecting these changes. Please be advised that the Division is in the process of revising the application forms to reflect the new regulations. 2) Page three of your application form specifies the use of an outside drop for MH #1. Please add an outside drop detail to your plans or send the Division three signed and sealed copies of an outside drop detail. Refer to the subject permit application number when providing the requested information. Please submit three copies of all information to my attention at the address below. Also, please note that failure to provide this additional information on or before April 11, 1993 will subject your application to being returned as incomplete, in accordance with 15A NCAC 2H .0208.
