My last inspection did not close out the E&S portion of the permit, so yes. I will need to verify coverage under the new
E&S permit and send out the official closure inspection for the Demo activities. I will do that tomorrow morning as I will
not have access to the files today.

Thank you for submitting the NOT request for this project. Before we can approve it, there are a couple of things we
need:
-The NOT certification form was not complete (box needs to be checked). You can email me a revised copy.
-The original E&SC approval was issued out of our Winston-Salem Regional Office, but the close-out documentation was
prepared by a consultant. Do you have final inspection information from the Winston-Salem office E&SC program? This
is usually the supporting documentation we are looking for. (I have also copied two contacts in that office to see if they
can provide something, or can confirm that the consultant's documentation is the only one available.)
