Salem Glen Subdivision WWTP

On June 18, 2015, Ms. Jenifer Carter of the Winston-Salem Regional Office conducted a Compliance
Evaluation Inspection at Salem Glen Subdivision's wastewater treatment plant. Sam Pegram, ORC, was
present during the inspection. The inspection consisted of an on -site inspection of the treatment facilities
and a review of facility files and self -monitoring data. The following is a summary of findings during the
inspection:
A. Facility records (daily logs and maintenance records) are well organized and current. A review of
self -monitoring data showed that laboratory data correctly correlates with data submitted on the
monthly Discharge Monitoring Reports. Be sure to include time of day for meter calibrations.
B. Calibration of the flow meter was checked on July 18, 2014.
C. Sludge in the plant appeared old, and there were excessive solids in the clarifier. Mr. Pegram said
a pump had been replaced in October 2014, but pulled back out in March 2015. A new pump is
available, but staffing issues have prevented him from being able to replace the pump yet. He
says this would be done within a week, and believes this will correct the hydraulic overloading of
the plant.

service asap. Resulting hydraulic overload of plant has led to inability to waste as needed, leading to
old ML in aeration basin and excessive solids in the clarifier. Mr. Pegram says a pump is available, and
now that he is not having to back up at as many plants, he should have time to replace it soon.

Per the terms of your NPDES permit, you were required to register for and begin using the Division of
Water Resources' electronic Discharge Monitoring Report (eDMR) system by 1/26/2015. Our records indicate
that as of the date of this letter, the subject facility has not submitted any data using the eDMR system.
Failure to register and begin submitting reports electronically is a violation of the terms of your permit,
subjecting you to a possible Notice of Violation and/or the assessment of civil penalties. In order to reduce
the risk of receiving additional enforcement action, you must complete your registration within 30 days of
receipt of this notice.
The Division has prepared a website devoted to all aspects of eDMR, including registration for its use,
obtaining an eDMR user account, and answers to frequently asked questions. You are encouraged to visit

(WWTP) and the collection system serving the Salem Glen Subdivision in Davidson County, NC.
Currently, Aqua NC, Inc., holds both of the above permits. The WWTP at Salem Glen is properly
licensed by the NC Division of Water Resources under permit number NC0083925. The WWTP
was last inspected by this office on July 21, 2014. The wastewater collection system at Salem Glen
is properly licensed by the NC Division of Water Resources under permit number WQCSD0194.
The collection system was last inspected on July 11, 2013.
Please note that the North Carolina Utilities Commission should be contacted if you need a
Certificate of Public Convenience and Necessity. The Utility Commission contact is Ron Brown,

On July 16, 2014, Ms. Jenifer Carter of the Winston-Salem Regional Office conducted a Compliance Evaluation
Inspection at Salem Glen and Spring Creek Subdivision's wastewater treatment facilities. Sam Pegram, Back-up ORC,
was present during the inspections. Ryan Smith was also present during the inspection at Spring Creek. The inspections
consisted of an on -site inspection of the treatment facilities and a review of facility files and self -monitoring data. The
following is a summary of findings during the inspection:
A. Facility records (daily logs and maintenance records) are well organized and current. A review of self -monitoring data
showed that laboratory data correctly correlates with data submitted on the monthly Discharge Monitoring Reports.
B. The flow meters were calibrated in July 2014 and April 2014 respectively.
C. The sampler tubing at Spring Creek's needed to be replaced.
D. There were floating solids/ pin floc in the clarifiers of both plants. Mr. Pegram did not have a sludge judge on hand to
measure the sludge blanket levels at Salem Glen, though he says he uses settleability to determine wasting needs. Mr.
Smith measured the sludge blanket level at Spring Creek, which was at an acceptable level. The overflow at both plants
was clear.
E. Mr. Pegram and Mr. Smith said they check meter calibrations each morning that meters are used for compliance
monitoring. Make sure that calibrations and calibration checks are recorded as per laboratory certification
requirements.

Dear Mr. Roberts:
On September 26, 2013, Ms. Jenifer Carter of the Winston-Salem Regional Office conducted a Compliance Evaluation
Inspection at Salem Glen and Spring Creek Subdivision's wastewater treatment facilities. Sam Pegram, Back-up ORC,
was present during the inspections. The inspections consisted of an on -site inspection of the treatment facilities and a
review of facility files and self -monitoring data.
A review of the submitted self -monitoring data revealed no permit limits violations at either facility. The following is a
summary of findings during the inspection, with deficiencies noted in bold print.
A) Copies of the permits, maintenance records and self -monitoring data were readily available. There were some
transcription errors, especially with time on site and temperature data. Compliance status was not affected.
B) The flow meters were calibrated in July 2013
C) The thermometer at Spring Creek's effluent sampler registered 7eC, and the thermometer had not been
checked against an NIST thermometer since 2008. Please have the thermometer checked annually and adjust
the temperature of the refrigerated sampler as necessary.
D) There were floating solids/pin floc in the clarifiers of both plants, especially at Salem Glen, though the effluent
leaving the clarifiers was clear. Mr. Pegram had not checked the sludge blanket levels since he started operating
these plants recently (due to the ORC being on medical leave). He promised to do so as soon as possible and take
action as warranted.
E) Mr. Pegram admitted to calibrating his meters only once weekly. Dieters should be calibrated within 24-hours
of use and the calibration checked at the end of each day, when used for compliance purposes.

SLUDGE MANAGEMENT PLAN

Aqua North Carolina, Inc.
No sludge will be treated on any wastewater treatment plant site operated
by Aqua N.C., Western Division
Where practical, sludge removed from a Aqua N.C., Western Division
facility will be transported via a contract hauler to another Aqua N.C.,
Western Division facility for the purpose of "seeding" a new or under
loaded plant.
Unusable (or "dead") sludge will be removed by a contract hauler and
properly disposed of in accordance with NCGS 143-215.1. Contract
haulers used by Aqua N.C., Western Division will be required to report the
quantity of sludge transported and identify the location of the proposed
disposal site if the sludge is not taken to an existing plant operated by Aqua
N.C., Western Division. Aqua N.C., Western Division has not entered into
any agreement to accept sludge into its facilities from plants not owned by
them.
Aqua N.C., Western Division will keep records on the quantity of sludge
removed from each facility, the name of the contract hauler, and the
destination of the sludge (whether used in another plant or disposed of).
The information will be kept on file and will be made available to any
regulatory agency having jurisdiction over sludge treatment or disposal.
Aqua N.C., Western Division includes all of the facilities under the
jurisdiction of the Winston-Salem Regional Office.

On November 4, 2010, Ms. Jenifer Carter of the Winston-Salem Regional Office conducted a Compliance
Evaluation Inspection at Salem Glen and Spring Creek Subdivision's wastewater treatment facilities. Mr. R. Mike
Barker, ORC, was present during the inspections. The inspections consisted of an on -site inspection of the
treatment facilities and a review of facility files and self -monitoring data.
A review of the submitted self -monitoring data for the period of September 2009 through August 2010 revealed no
permit limits violations at either facility. With respect to the administrative portion of the facility's self -monitoring
programs, no deficiencies were noted. The following is a summary of findings during the inspection.
A) Records were well organized and current for both facilities
B) While there is considerable rust on the exterior of the plants, the interiors seem to be in relatively okay
condition.
C) Copies of the permits, maintenance records and self -monitoring data were readily available.
D) Thermometers have been placed in the sampling units since the last inspection.
E) The sludge blank in the secondary clarifier at Spring Creek was 8 feet deep. Mr. Barker noted that some
wasting was in order. The sludge blanket was not measured at Salem Glen during the inspection, though
there were similar amounts of floating solids at both plants.
F) The effluent flowing from both plants was clear.
Overall, both plants appear to be sufficiently maintained and operated. Should you have any questions please

for April 2009. This is in violation of Part Il, condition D(2) of the NPDES permit, as well as 15A
NCAC 213 .506(a), which requires the submittal of Discharge Monitoring Reports no later than the last
calendar day following the reporting period. Failure to submit reports as required will subject the violator
to the assessment of a civil penalty of up to $25,000 per violation. You will be considered noncompliant
with the self -monitoring requirements of your NPDES permit until the report has been submitted.
To prevent further action, please submit said report within fifteen (15) days of receipt of this notice.
Additionally, this letter provides notice that this office will recommend the assessment of civil penalties if
future reports are not received within the required time frame during the next twelve (12) reporting

Enclosed with this letter is a copy of the draft permit for your facility. Please review the draft very carefully to ensure
thorough understanding of the conditions and requirements it contains. The following procedure has been recently
implemented by DWO: Total residual chlorine (TRC) compliance level changed to 50ug/L. Effective March 1, 2008, the
Division received EPA approval to allow 50 ug/L TRC compliance level. This change is due to analytical difficulties with TRC
measurements. Facilities will be still be required to report actual results on their monthly discharge monitoring report (DMR)
submittals, but for compliance purposes, all TRC values below 50 ug/L will be treated as zero. A footnote regarding this
change has been added to the effluent limitations page(s) in the draft permit.
Regarding the receiving stream of the WWTP discharge, this section of the Yadkin is listed as an impaired stream on
North Carolina's 303(d) list. This means that this river section does not meet all water quality standards. The Division is
working hard toward the improvement of water quality in this section of the Yadkin River. If the current changes to permits
discharging to this section of the Yadkin River do not result in improved water quality, removal of discharges to this section of
the Yadkin River may be necessary for the health of the river.
If noncompliance with your permit's effluent limits is shown to be a direct cause of stream degradation, the Division
will recommend removal of your facility's discharge. You should be continually evaluating all wastewater disposal alternatives
and pursue the most environmentally sound alternative of the reasonably cost effective alternatives to surface water
discharge. Such an evaluation may help you find an alternative to surface water discharge that is more appropriate for your
facility.
Concurrent with this transmittal, the Division will solicit public comment on this draft as required by the EPA by
publishing a notice in newspapers having circulation in the general Davidson County area. Following the 30-day public
comment period, the Division will review all pertinent comments and take appropriate action prior to issuing a final NPDES
permit.

Jenifer Carter of the Winston-Salem Regional Office (WSRO) performed a Compliance Evaluation
Inspection at Salem Glen Subdivision's Wastewater Treatment system on October 8, 2008. Mr. Marc
Stokes, (WSRO Technical Assistance), Mr. Tony Parker (Aqua's Area Manager), Mr. Samuel Pegram
(ORC), and Mr. Robert `Mike' Barker (back-up ORC), were also present during the inspection. The
inspection consisted of two parts: an on -site inspection of the treatment facility and a review of facility
files and self -monitoring data. The following are the findings from the subject inspection:
Permit
The current permit became effective May 1, 2004 and expires on December 31, 2008.
Less than three weeks before this inspection, Aqua took over operations of Salem Glen from Atlantic
Utilities. It was apparent that Mr. Pegram and Mr. Barker were still in the process of familiarizing
themselves with the plant. If not already done anew ORC designation form should be submitted
immediately.
Operations and Maintenance
Overall housekeeping is acceptable, though rust has formed in several areas on the plant itself. A good
paint job may be needed to prevent the rust from becoming a serious problem.
Records/Reports
A review of laboratory reports and Discharge Monitoring Reports (DMR) revealed no discrepancy
between the generated data and the data submitted to DWQ. A copy of the laboratory data is kept on
file. A daily visitation logbook and separate maintenance records are available for review.
Self -Monitoring Program
A review of the DMRs for the period of March 2007 through July 2008 did not reveal any violations.
Facility Site Review
When asked about the cycling of the blowers, Mr. Pegram stated that the blowers are always on and
never cycled off. The DO in the aeration basin was around 0.98 mg/L on both sides of the plant during

the inspection, which is on the low side. Mr. Pe ram agreed that adjustments could be made to bring
the DO up.
Floating solids and grease were observed in both secondary clarifiers, though none was observed at the
discharge. The sludge blanket level was 10 feet, with 4 feet clear. Adjustments may be needed to keep
the solids and rg ease from zettinZ worse. Mr. Pegram thought adjustments to the skimmers might help.
Though the refrigerated effluent sampler felt cold inside, there was no thermometer to determine the
actual temperature. The thermometer was the property of the previous operator and was therefore
removed. Mr. Parker said new thermometers have been ordered for all of Atlantic Utility's previous
facilities.
Flow Measurement
The flow at the plant is measured with a 90-degree V-notch weir and a model 61R Stevens Flow Meter.
The flow -measuring device was last calibrated on July 26, 2008.
Effluent / Receiving Waters
The effluent at the outfall was clear and free of grease or solids.
Sludge Handling & Disposal
Forsyth Rooter handles the removal of residuals.

Overall, it was apparent that the current ORC and back-up ORC were still in the process of familiarizing
themselves with the plant, which they began operating less than three weeks prior to the inspection. Please
note the following items needing attention:
DO in Aeration Basins on the low side. Adjustments will be made.
Floating solids and grease observed in the clarifiers, though not passing over the weirs during the inspection.
Rust was observed in serveral spots on the plant, which may need to be painted

There were some apparent minor discrepancies between the various documents and
drawings submitted in support of this application, as follow.
(1) Though both the drawings and specs. size the aeration blowers @ 15 H.P., page
1 of the treatment capability calculations states in its narrative that they are 10
H.P.
(2) Page 4 of the calculations states in its narrative that sludge handling is aerobic
digestion followed by vacuum assisted drying beds. The plans and specs make
no mention of drying beds.
The Region recommends that, upon a favorable engineering review by Central Office
staff, the A. to C. be issued.

.0004 DEFINITIONS
The following definitions shall apply throughout this Subchapter.
(1) Activated Carbon Beds. A physical/chemical method for reducing soluble organic material from wastewater effluent; The column -type beds used in this
method will have a flow rate varying from two to eight gallons per minute per square foot and may be either upfiow or downflow carbon beds. Carbon may or
may not be regenerated on the wastewater treatment plant site;
(2) Aerated Lagoons. A basin in which all solids are maintained in suspension and by which biological oxidation or organic matter is reduced through artificially
accelerated transfer of oxygen on a flow -through basis;
(3) Aeration. A process of bringing about intimate contact between air or high purity oxygen in a liquid by spraying, agitation or diffusion;(3a) Extended
Aeration. An activated sludge process utilizing a minimum hydraulic detention time of 18 hours.
(4) Agriculturally managed site. Any site on which a crop is produced, managed, and harvested (Crop includes grasses, grains, trees, etc.);
(5) Air Stripping. A process by which the ammonium Ion is first converted to dissolved ammonia (pH adjustment) with the ammonia then released to the
atmosphere by physical means; or other similar processes which remove petroleum products such as benzene, toluene, and xylene;
(6) Carbon Regeneration. The regeneration of exhausted carbon by the use of a, furnace to provide extremely high temperatures which volatilize and oxidize the
absorbed impurities;
(7) Carbonaceous Stage. A sbige of wastewater treatment designed to achieve 'secondary' effluent Ilmits;
(8) Centrifuge. A mechanical device in which centrifugal force is used to separate solids from liquids or to separate liquids of different densNas;
(9) Chemical Addition Systems- The addition of chemicals) to wastewater at an application point for purposes of Improving solids removal, pH adjustment,
alkalinity control, etc.; the capability to experiment with different chemicals and different application points to achieve a specific result will be considered one
system; the capability to add chemical(s) to dual units will be rated as one system; capability to add a chemical at a different application points for different
purposes will result in the systems being rated as separate systems;
(10) Chemical Sludge Conditioning. The addition of a chemical compound such as lime, ferric chloride, or a polymer to wet sludge to coalesce the mass prior to
its application to a dewatering device;
(11) Closed Cycle Systems. Use of holding ponds or holding tanks for containment of wastewater containing inorganic, non -toxic materials from sand, gravel,
crushed stone or other similar operations. Such systems shall carry a maximum of two points regardless of pumping facilities or any other appurtenances;
(12) Combined Removal of Carbonaceous BOD and Nitrogenous Removal by Nitrification- A single stage system required to achieve permit eKluentt limits on BOD
and ammonia nitrogen within the same biological reactor,
(13) Dechlorinatlon. The partial or complete reduction of residual chlorine to a liquid by any chemical or physical process;
(14) Denitrification Process. The conversion of nitrate -nitrogen to nitrogen gas;

PART IV - EVALUATION, CONCERNS AND RECOMMENDATIONS
PART IV -a EVALUATION
On January 25 & 27, 1994, Mr. Abner Braddy and myself
investigated the proposed subdivision site. During this site
evaluation, it was noted that several roads had been constructed,
several golf holes graded, and a small number of lots had been
prepared.
No apparent problems were observed with the discharge point
on the Yadkin River other than the location of a small
recreational boat landing approximately 1/4 mile downstream.
However, this area should not be adversely affected by the

discharge.
PART IV-b CONCERNS
First, no county zoning form was submitted with this
application. A form for both Davidson and Forsyth counties
should be submitted.
Second, a more complete soils evaluation should be
completed. The main area of concern is the Forsyth County
portion of the proposed subdivision. This area should be tested
for possible subsurface disposal.
PART IV-c RECOMMENDATIONS
WSRO recommends handling this application in accordance with
Division policy.
