Good afternoon Lin,
I'm reviewing the Sassarixa Swamp 401 application, which was the first DIMS ePCN project submittal. I have one
correction request in order to continue to process the approval letter. Wildlands' contact information is listed both
under the applicant section B.3. and the consultant section B.4. DMS' contact information was not included, but should
be listed under the applicant section B.3. That way we have both co -applicants contact information as well as signatures.

Can you please submit a brief cover letter requesting an application modification/correction along with a revised contact
information page (original attached). The revision can be a simple text strikethrough of Wildlands' information and
hand-written or pdf text box with DIMS' contact information. The cover letter and revised application page can be
submitted using this link: https://edocs.deg.nc.gov/Forms/Supplemental-Information-Form. Click the "more information
