This application is for sewer extensions involving gravity sewers. pump stations and force mains, or any combination that has been
certified by a professional engineer and the applicant that the project meets the requirements of 1 SA tifAC 61 and the Division's
Minimum Design Criteria and that plans, specifications and supporting documents have been prepared in accordance with, I5a

While no upfront engineering design documents are required for submittal, in accordance with 15A NCAC 02T ,0305(b), design
documents must be prepared prior to submittal of a fast track permit application to the Division. This would include plans, design
calculations, and project specifications referenced in 15A NC %C 021 .0305 and die applicable minimum design criteria_ These
documents shall be available upon request by die Division
Projects that are deemed permitted (do not require a permit from the Division) are explained in I iA NC,yL 02TPRO
Projects, not eligible for review via the fast track process (must be submitted for full technical review):
Projects that require an environmental assessment in accordance with 15A NCAC 1 C .0100;
I- Projects that do not meet any part of the minimum design criteria (MDC) document:
:- Projects that involve a variance from the requirements of 15A NCAC 2T;
Pressure sewer systems utilizing septic tank: -effluent pumps (STEPS) or simplex grinder pumps;
i STEP or simplex grinder pumps connecting to pressurized systems (e.g. farce mains);

General When submitting an application, please use the following instructions as a checklist in order to ensure all required items
are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and
reduce the amount of requested additional information. Failure to submit all required items v ill necessitate additional
processing and review time, and may result in return of the application. Unless otherwise noted, the Applicant shall submit
one original and one copy of the application and supporting documentation.

appurtenances and personnel are available for distribution and operation of this pump station,"

stations' storage capacities and the rotation schedule of the portable power source or pump, including travel
timeframes, shall be pro%ided in the case of a multiple station power outage. (Required at time of certification)

I Chris Doherty - Director certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to

The proposed Cedar Creek Subdivision is located 500 feet south of the intersection of Hicks Road
and Cedar Creek Road in Franklin County, NC. The proposed modification includes the elimination
of the pump station and force main to serve the subdivision. We are increasing the amount of 8 inch
gravity sewer by 29 feet. We are seeking a permit modification to install a public sanitary sewer
extension to serve Fifty (50) proposed single-family residences as part of Phase 1.
The proposed sanitary sewer extension consists of 2,787 LF of 8" PVC extending off of an existing
sanitary sewer manhole constructed as part of permit WQ0041265 (hadleigh Subdivision phase 1).
Our office will be sending in a certification for a portion of this permit that was constructed to service
the Cedar Point Subdivision. The proposed sanitary sewer main will be public and maintained by
Franklin County.

Future sewer extension applications should be made using the reduced flow amounts stated {
above. All other aspects of the permitting process remain unchanged, and all applications
must be in compliance with the statutes, rules, regulations and minimum design criteria as
certified by the owner and engineer. Without regard to this or any other flow reduction either
requested or granted, at no time shall the flow to this wastewater treatment facility
exceed the limits defined in the NPDES permit for this facility, nor exceed the capacity
of the sewers downstream of any new sewer extension or service connection.

Project Narrative:
The proposed Cedar Point subdivision is located on Cedar Creek Road approximately
500 feet South of the intersection with Hicks Road in Franklin County. The Subdivision
has been approved for 63 single family lots, however the County has only granted sewer
allocation for 50 single family lots.
This permit application is a modification to permit WQ0038697 and consists of
increasing 8 inch sanitary sewer in the project from 2,758 LF to 2,787 LF. The
application no longer has a pump station and force main. This pump station was remove
in lieu of connecting to the adjacent Hadleigh phase 1 Subdivision with permit number
WQ0040297.

We're starting review on the modification for this project.
This modification was to remove a PS and FM, as well as add 29 feet of 8" line (2787 Total).
The original permit was for 38 homes and 9,500 GPD. The new application shows 50 homes and 12,500 GPD. We
wanted to confirm this revision as well.

We're following -up on the email I sent on April V. Please see below. The Certification that was received certified that
the project was built according to what was in the permit. But the narrative that accompanied the certification stated
that a pump station and forcemain that were in the permit were eliminated from the project, and additional gravity was
added. A permit mod is needed (at least one). The project should not be in operation until a permit mod is requested,
issued and certified. See my earlier email for more details.

We're just getting around to reviewing the Engineering Certification for the referenced project that we received back in
October (sorry for the delay, with everything we have going on, these ECs sometimes have to sit on the back -burner for
a while).
The EC had a narrative explaining that the pump station and forcemain described in permit WQ0038697 had been
eliminated from the design and replaced with gravity line. I've attached a screenshot of the narrative.
As described in the Standards Conditions of the issued permit (Section 1.7), a modification to the permit is required if
there are changes to the project, (such as eliminating a PS/FM and adding gravity), and "After -construction requests are
discouraged by the Division and may not be approved, thus requiring replacement or repair prior to certification &
activation. [15A NCAC 02T .0116)"
Please ro ose a solution to this which will at least require a modification to permit WQ0038697, and may require a
mod to WQ0040297 (Hadleigh at Cedar Creek, referenced in the narrative). (we don't have a record of receiving a
certification for 40297 yet).
