The facility was found to be compliant with the NPDES permit effluent limits. The inspection findings are detailed below and an inspection checklist is attached. I. Permit The NPDES permit required the installation of an influent composite sampler by January 1, 2016. The ISCO sampler was installed by the town in October 2015, well ahead of the deadline. Thank you for your attention to this matter. II. Self -Monitoring Program The inspection reviewed the monthly discharge monitoring reports (DMRs) that were submitted for the period December 2014 through September 2015. The Bermuda Run WWTP was found to be compliant with all effluent limits during this time period (see data summary attached). With regards to the monitoring requirements, it was noted that all sampling was performed per the frequencies specified in the permit. III. Flow Measurement The permit requires continuous flow recording and annual calibration of the metering device. The daily effluent flow values reported on the DMR are measured using an ultrasonic flow meter and v- notch weir. The meter was last calibrated on July 24, 2015 by Carolina Technical Services. Additionally, flow proportional composite samples are collected of the effluent as required by the permit. IV. Sludge Handling and Disposal Solids are stored in the two available digesters. Operations and Maintenance The facility appeared to be competently operated and maintained. No problems or concerns were noted. VI. Facility Site Review The existing 0.193 MGD treatment system consists of the following: influent pump station, flow splitter box with bar screen, package plant type system with dual aeration, clarification and digester units, followed by a bucket chlorinator, contact tanks, bucket dechlorinator and flow meter. VII. Records/Reports A comparison of the September 2015 monitoring report with the corresponding operator's records and the laboratory data sheets from Statesville Analytical Laboratory showed that all values were transcribed onto the DMR correctly. VUL Effluent/Receivina Waters The system discharges into the Yadkin River, class "WS-IV' waters in the Yadkin -Pee Dee River Basin. The effluent was observed to be clear. The treatment facility was found to be compliant with the effluent limits of the NPDES permit. The inspection findings are detailed below and an inspection checklist is attached for your records. I. Permit The NPDES permit was recently modified on December 8, 2014 to waive the requirement to collect influent samples with a composite sampler until January 1, 2016. II. Self -Monitoring Program The DMRs were reviewed for the period January 2014 through November 2014. The facility was found to be compliant with all NPDES effluent limits (see data summary attached). With regards to the permit monitoring requirements, it was noted that all sampling was performed per the frequencies specified in the permit. III. The treatment facility was found to be in consistent compliance with the effluent limits of the NPDES permit. The inspection findings are detailed below and an inspection checklist is attached for your records. I. Permit The NPDES permit was recently reissued with an effective date of February 1, 2014. Two changes to the permit involve the addition of total mercury monitoring to be performed once during the permit cycle (EPA Method 1631E) and the requirement to begin submitting the monthly discharge monitoring reports (DMRs) electronically no later than 90 days from the effective date of the permit. II. Self -Monitoring Program The DMRs were reviewed for the period January 2013 through December 2013. The facility was found to be compliant with the NPDES effluent limits, except for one weekly average fecal coliform violation that occurred on October 8, 2013 (see data summary attached). This violation was previously addressed by correspondence from our office. With regards to the permit monitoring requirements, it was noted that all sampling was performed per the frequencies specified in the permit. III. Flow Measurement The NPDES permit requires continuous flow recording and annual calibration of the metering device. The flow values reported on the DMR are measured using an ultrasonic flow meter and v-notch weir. The meter was last calibrated on July 18, 2013 by Carolina Technical Services. In addition, flow proportional composite samples are collected of the effluent as required by the permit. Sludge Handling and Disposal Solids are stored in the two available digesters. The digesters are pumped regularly by Forsyth Rooter Service with disposal occurring at the Winston-Salem/Forsyth County Utilities Commission treatment facilities. The digesters were last pumped on January 15, 2014. V. Operations and Maintenance The facility appeared to be competently operated and maintained. It was noted that the roof drains from the Golf Club House were removed from the sewer system in August 2013. This action, along with the ongoing collection system improvements, has reduced the inflow and infiltration problem. The only concern, which has been noted in previous inspections, involves the continued use of buckets as chlorination and dechlorination devices. The relocation and installation of new tablet chlorinator/dechlorinator units was discussed with the operator during the inspection. VI. Facility Site Review The existing 0.193 MGD treatment system consists of the following: influent pump station, flow splitter box with new bar screen, package plant type system with dual aeration, clarification and digester units, followed by a bucket chlorinator, contact tanks, bucket dechlorinator and flow meter. VII. Records/Reports A comparison of the December 2013 self -monitoring report with the corresponding operator's records and the laboratory data sheets from Statesville Analytical Laboratory showed that all values were transcribed onto the DMR correctly. VIII. Effluent/Receiving Waters The system discharges into the Yadkin River, class "WS-IV' waters in the Yadkin -Pee Dee River Basin. The effluent was observed to be clear. Nick Slogick, ORC was present for the inspection. The inspection consisted of two parts: an on -site inspection of the treatment facility and a file review. The following are the findings from the subject inspection. I. Permit The NPDES permit for the Bermuda Run WWTP became effective February 1, 2009 and will expire on January 31, 2014. Please be aware that the permit renewal application must be received by Division of Water Quality 180 days prior to the permit expiration date. The permitted components of the 0.193 MGD wastewater facility are influent pump station with high level alarm, bar screen, flow splitter box, two extended aeration tanks in parallel, two aerobic sludge digesters, chlorinator and chlorine contact tank. II. Records/Reports A review of the laboratory reports and Discharge Monitoring Reports (DMRs) for the Bermuda Run WWTP showed no reporting violations during that period. The files and all required log books were on - site and in order. III. Facility Site Review The facility site review indicated that the 0.193 MGD treatment works is consistent with the permitted components. The actual treatment system consists of the abovementioned components. The flow meter had been calibrated on 1/15/13. Temperature in the composite sampler was 7 degrees at the time of the inspection. This should be kept at 4 degrees or below. It is understood that this is a new unit and there is still fine tuning going on. Please just be aware and keep watch on it so samples are not compromised. Overall the plant was clean and appeared to be well run. Discharge flows in the Yadkin River which at that location is a WS-IV CA Waterbody in subbasin 03-07-02 of the Yadkin -Pee Dee River Basin. IV. Laboratory All of the sample analyses are conducted by Statesville Analytical Labs. The laboratory was not reviewed at the time of the subject inspection. This inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities, equipment and the capability to meet effluent limits and terms and conditions of the NPDES permit. The attached EPA inspection form notes the eight (8) areas that were evaluated. The findings and observations are outlined below: I. Permit The NPDES permit was reissued on February 1, 2009. A copy of the current permit was available for review. II. Self -Monitoring Program This inspection involved a review of the monthly self -monitoring reports (DMR's) for January 2010 through December 2010. The Bermuda Run wastewater treatment system was found to be in compliance with all NPDES effluent limits and monitoring requirements for the above period (see attached data summary). III. Flow Measurement Continuous flow measurement at the plant is performed using a V-notch weir and an effluent Prosonic flow meter. The flow measuring device was last calibrated by Carolina Technical Services on July 7, 2010. In addition, flow proportional composite samples are collected from the effluent as required by the permit. IV. Sludge Handling and Disposal Solids returned from the secondary clarifiers are stored in the two available digesters. Forsyth Rooter regularly pumps the digesters with disposal at the Winston-Salem/Forsyth County Utility Commissions treatment facilities. Operations and Maintenance Observations indicate that the facility is competently operated and maintained. Our office appreciates that town's ongoing efforts to locate and repair inflow sources to the collection system and to enact a grease ordinance. It is our understanding that additional sewer line cleaning will occur in 2011 and that removal of the inflow sources discovered at the club house will be addressed in the near future. Additionally, progress is being made to replace the existing chlorination and dechlorination devices with new reliable units. An authorization to construct was issued for the new dechlorination unit by DWQ on 1/7/11. VI. Facility Site Review Grey Engineering is working on the following repairs/upgrades for the 0.193 MGD facility: install new tablet dechlorination and chlorination units, replace the existing bar screen unit, relocate the effluent sampler, rotate the effluent flow meter, install new walkways and grating and remove unused piping. VII. Records/Reports A review of the operator's files indicates that all records are being maintained according to the permit. A review of chain of custody records and certified laboratory data sheets shows that all information is being transcribed to the DMR's correctly. VIII. Effluent/Receiving Waters The system discharges into the Yadkin River, Class "WS-IV" waters in the Yadkin -Pee Dee River Basin. The effluent was observed to be clear on the day of inspection. The following minor repairs/upgrades are planned: new tablet chlorination and dechlorination units are to be installed to replace the 5 gallons bucket devices, (Authorization to construct for the dechlorination unit was issued by DWQ on 1 /7/11), the bar screen unit will be replaced, the effluent sampler relocated, the flow meter position rotated and walkways will be added/improved. MLSS levels in both aeration basins are currently analyzed by Tritest 2/month for solids inventory. In regards the collection system, an inflow/infiltration study was started in the summer of 2010 to clean various sewer lines and note deficiencies that need repair. Additional work is to be completed in 2011. One of the main problems located to date invovles the club house facility that has stormwater connections that need to be eliminated. This type of inspection consists primarily of two parts: an in office file review and an on -site inspection of the treatment facility. The attached EPA inspection form notes the eight (8) areas that were evaluated. The findings and observations are outlined below: I. Permit The NPDES permit was recently reissued with an effective date of February 1, 2009. II. Self -Monitoring Program This inspection involved a review of the monthly self -monitoring reports (DMR's) for October 2008 through September 2009. The Bermuda Run facility was found to be in compliance with all final NPDES effluent limits for the above period (see attached data summary). III. Flow Measurement A Prosonic FMU 861 meter is used to report effluent flow on the DMR's. Regulations require that flow meters used for reporting be calibrated annually. The facility's flow meter was last calibrated by Carolina Technical Services on July 14, 2009. In addition, flow proportional composite samples are collected from the effluent as required by the permit. IV. Sludge Handling and Disposal Solids from the secondary clarifiers are stored in the two available digesters. Sludge wasting from the digesters has been increased to reduce the loss of solids from the treatment system during rainfall events. Solids removed from the digesters are disposed of at the Winston-Salem/Forsyth County Utility Commissions treatment facilities. Solids from this basin can be transferred to the other tank. Land application of the solids is proposed. The big question is if they may be able to discharge the treated wastewater at this location after dechlorination. I told him that I would pass this question to Mike for evaluation and response. The second basin will primarily receive the settling basin solids from the water purification process. 2-Spillman pump station. This system is now documented on plans that are 75% complete. He gave me a copy of them (on your desk) and will send an e-mail to discuss the parts that may need variances from the MDC. 3-Bermuda Run consideration of an expansion and relocation of WWTP to Carter Creek area which will discharge below Idols Dam. Carters Creek is "C" and the Yadkin at this point is "WS-V". We discussed the review of the project by PWS staff. This could be considered as a slight improvement in the location of the discharge, even though they will be going from the current 0.19 MGD to an initial 1.0 MGD. Expansion to 2 MGD would be planned for that site in the future. This is still on the QT for the near future. Bermuda Run WWTP's permit contains a definition of a `composite sample', along with four methods for performing such sampling. Bermuda Run WWTP's composite samples are obtained using the `constant time/ constant volume' method, which can only be used in situations where effluent flow rates vary less than 15% and with prior approval from the Director. Records show that the flow at Bermuda Run WWTP varies more than 15%, and this office feels that this will continue to be the case in the foreseeable future. Therefore, this office requires Bermuda Run to begin using one of the other three, flow -proportional methods for pulling composite samples in order to comply with the permit. The deadline for implementing an approved composite sampling method is December 1, 2006. If you cannot meet the deadline, please call the regional office as soon as possible to discuss the possibility of extending the deadline. Failure to implement an approved method may result in a violation, and possible enforcement action by the Division of Water Quality.
