On December 15, 2015, Mike Mickey of this office met with Matt Seats and Michael Weir, ORC, to
perform a Compliance Evaluation Inspection of the NPDES wastewater discharge from the Sparks Road
Water Treatment Plant (WTP). The inspection findings are detailed below and an inspection checklist is
attached.
I. Permit
The NPDES permit expires next on May 31, 2019. Permit condition A.(3) required the submittal of
electronic discharge monitoring reports (eDMRs) on or before April 1, 2015. The first electronic DMR for
the Sparks Road facility was submitted for February 2015, well ahead of the above deadline.
H. Self -Monitoring Proeram
The inspection reviewed DMRs that were submitted for the period January 2015 through November 2015.
The Sparks Road WTP was found to be compliant with all effluent limits during this period (see data
summary attached), except for a daily maximum and monthly average TSS violation that occurred in
October. These violations will be addressed in subsequent correspondence from this office. With regards
to the permit monitoring requirements, it was noted that all sampling was being performed per the
frequencies specified in the permit.
III. Flow Measurement
The NPDES permit requires continuous flow recording and an annual calibration of the flow measuring
device. The effluent flow values reported on the DMRs are measured using an ultrasonic meter with
parshall flume. The meter was last calibrated by Fortech on August 17, 2015.
IV. Sludge Handling and Disposal
Solids collected in the settling lagoons are land applied under non -discharge permit No. WQ0028806. The
residuals were last removed by EMA Resources in 2010. Accumulated solids may be removed from one
lagoon in the spring of 2016.
V. Operations and Maintenance
The facility was competently operated and maintained. The only concern noted during the inspection
involved the erosion along the banks of the settling lagoons. The lack of vegetation was resulting in high
TSS and turbidity levels in the lagoon effluent after significant rainfall events. It is hoped that the banks
can be lined with matting and gravel in 2016.

VI. Facility Site Review
The treatment system components for the filter backwash and clarifier wastewater consist of the following:
two 785,000 gallon settling lagoons operated in parallel, tablet dechlorinator and flow meter.
VH. Records/Reports
A comparison of the November 2015 DMR with the operator's field records for flow, pH and total residual
chlorine along with the laboratory results from Statesville Analytical showed that all data was transcribed
onto the DMR correctly.
VM. Effluent/Receiving Waters
The system discharges into a ditch leading to a tributary of the Yadkin River, class "WS-IV" waters in the
Yadkin -Pee Dee River Basin. The effluent was clear on the day of inspection.

Comment: The NPDES permit was reissued August 1 2014 and expires next on May 31
2019. The
current permit required that eDMR submittal begin on or before April 1 2015.
Submittal
began before the deadline with the February 2015 DMR.
For the review period January 2015 through November 2015 the Sparks Road WTP was
found to be compliant with all effluent limits and monitoring requirements, except for violation
of the TSS daily maximum and monthly average limit in October. These exceedances were
due to erosion around the settling lagoons that after heavy rainfall events. Plans are in the
works to install matting and gravel to remedy the situation.
A comparison of the November 2015 DMR with the corresponding operator's records (for
flow, PH and total residual chlorine) and the laboratory data from Statesville Analytical
(remainder of the parameters) showed that all values were transcribed correctly.

Comment: The effluent is discharged to a ditch adjacent to the flow monitoring structure. The effluent
was crystal clear on the day of the inspection. The ditch eventually leads to a tributary of the
Yadkin River that is class "WS-W waters. The tributary loins with the river Oust downstream
of the intake structure.

Most recent renewal application (4/24/2014) listed Davie County Water System as the Owner. Permit renewal was
issued to Davie County Water System on 6/27/2014. 2009 renewal (11/13/2009) was issued to Davie County Public

NC0024872 — last ownership change application (5/26/2015) listed Davie County Public Utilities as the Owner. Permit
renewal was issued to Davie County Public Utilities on 6/4/2015.

The Davie County website refers to "Public Utilities — Water". The linked documents on the website don't address the
Water System vs. Public Utilities issue.
We need something from Davie County clarifying what Owner name they want to use. BIMS and the eDMR system can
be fixed once, hopefully for good.

Charles,
Michele is trying to register a facility that appears to have an issue with the owner name in BIMS. There are 2 permits,
NCO028472 (Davie County Water System/Cooleemee WWTP) and NCO084212 (Davie County Public Utilities/Sparks Road
WTP, and appears they should be under the same owner. NCO084212 has already registered and now they want to
register NC0028472. Both permits should have registered when NCO084212 was registered but failed to do so due to
having different owner names in BIMS, Davie County Water System and Davie County Public Utilities.
The owner needs to reflect the same name for both in BIMS. Once this is corrected, we can manually add the other
permit to eDMR.
Please take a look at this and update one of the permits to reflect the correct owner name.

I'm in the process of registering Davie County Water System/Cooleemee WWTP (NC0024872) for eDMR. However, the
other facility NCO084212 is registered for eDMR but it's listed as Davie County Public Utilities in BIMS. Both facilities are
not showing up under Johnny Lambert's name under the verification report because of the difference in the names. Can

to perform a Compliance Evaluation Inspection of the NPDES wastewater discharge from the Sparks Road
Water Treatment Plant (WTP). The treatment system was found to be compliant with the NPDES permit.
The inspection findings are detailed below and an inspection checklist is attached for your records.

The NPDES permit was renewed on August 1, 2014. Permit condition A.(3) requires the submittal of
electronic discharge monitoring reports (eDMR) on or before April 1, 2015.

The monthly self -monitoring reports (DMRs) were reviewed for the period January 2014 through November
2014. The facility was found to be compliant with all permit effluent limits (see data summary attached).
In regards to the permit monitoring requirements, it was noted that all sampling was being performed per the
frequencies specified in the permit.

The Sparks Road WTP was found to be compliant will all effluent limits and monitoring_
requirements for the review period January 2014 through November 2014.
A review of the November 2014 DMR found that all values were transcribed correctly from
the operator records (for PH and total residual chlorine) and from the Statesville Analytical
data results (remainder of the permit parameters).
It was noted that the chronic toxicity test conducted in February 2014 was reported as a
"fail". Currently, the permit does just requires toxicity monitoring with no limit.

sedimentation, and filtration] to produce potable water at a system -design rate of 3.0 MGD. Previous permit
required installation of "continuous recording" by March 1, 2011. Filter -backwash discharge rates vary over
time, but reported for the past two years, average 1.241 MGD with a maximum at 2.355 MGD; minimum

notice in newspapers having circulation in the general Davie County area, as required by the NPDES

Changes for Renewal. The Division updated your permit in accord with current permitting strategies

your convenience [see Section A. (1)]. Please note that recent Whole Effluent Toxicity (WET) test
failures suggest that this effluent may be harmful to the aquatic environment. Therefore the Division
has included a Special Condition that may require additional action [see permit Section A. (4)].

A. (2.) CHRONIC TOXICITY PASS/FAIL MONITORING (QUARTERLY)
The Permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised December 2010, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90%. The testing shall be
performed as a Ceriodaphnia dubia 7-day pass/fail test. The tests will be performed during the months of
February, May, August and November. These months signify the first month of each three-month
toxicity -testing quarter assigned to the facility. Effluent sampling for this testing must be obtained during
representative effluent discharge and shall be performed at the NPDES permitted final effluent discharge
below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed, using the parameter code TGP3B.

This facility's history of Whole Effluent Toxicity (WET) monitoring indicates that discharge to surface

Sparks Road Water Treatment Plant
Sludge Handling Plan
The Sparks Road WTP has two parallel 785,000-gallon filter
backwash lagoons. The lagoons Provide long-term storage of sludge
developed during clarifier flushing and filter backwash operation
sl atdge is added to the lagoons the s. As chemical solids are settled by gravity.
Tla.e supernatant is discharged through the overflow -supernatant pipe to a
m, im made ditch that flows to an unnamed tributary of the Yadkin River.
Sludge depth is monitored in each lagoon and lagoons are designed to
laiiit up to 10 years at the expected plant sludge disposal rate before solids
ha+re to be removed. Once lagoons are classified as full, sludge in the
lal loons is dredged and land applied on neighboring farm fields. prior to
dri 4ging a land application permit is acquired and the sludge is analyzed to
as:iure it is safe to apply. The sludge removal is contracted out to a
spii,cialized private company to assure that all state and federal laws would
be followed though out the sludge removal process.
In summary the sludge handling at this facility happens only once
evEr 10 years and is contracted out for land application.

SUBJECT: Compliance Evaluation Inspection
Sparks Road WTP Discharge

Dear Mr. Lambert:
On January 30, 2014, Mike Mickey of this office met with Michael Weir, ORC, to perform a Compliance
Evaluation Inspection of the NPDES wastewater discharge from the Sparks Road Water Treatment Plant (WTP).
The treatment system was found tobe in consistent compliance with the effluent limits and monitoring requirements
of the NPDES permit. The inspection findings are detailed below and an inspection checklist is attached for your
records.
I. Permit
The NPDES permit expires on May 31, 2014. Per Part II.B(10) of the permit, a renewal request should have been
submitted to the Division at least 180 days prior to expiration. To date, a renewal request has not been received.
The application should be submitted to Raleigh on or before February 14, 2014.
H. Self -Monitoring Program
The monthly self -monitoring reports (DMRs) were reviewed for the period January 2013 through December 2013.
The facility was found to be compliant with all permit effluent limits (see data summary attached), except for one
daily maximum pH violation that occurred in May 2013. This violation was previously addressed in a Notice of
Deficiency letter from this office. In regards to the permit monitoring requirements, it was noted that all sampling
was being performed per the frequencies specified in the permit.
The review did note that the effluent failed the chronic toxicity test conducted in February 2013. It is our
understanding that excessive chlorine was the cause of the test failure. At present, the toxicity requirement in the
permit is monitoring only with no effluent limitation.
III. Flow Measurement
The NPDES permit requires continuous flow recording and annual calibration of the flow measuring device. The
daily effluent flow values reported on the DMRs are measured using an ultrasonic meter with parshall flume. The
meter was calibrated by Fortech on June 27, 2013.

IV., Sludge Handling and Disposal
Solids collected in the settling lagoons are land applied under non -discharge permit No. WQ0028806. The
residuals were last removed by EMA Resources in 2005 and 2010.
V. Operations and Maintenance
The facility was competently operated and maintained. No problems or concerns were noted.
VI. Facility Site Review
The treatment components for the filter backwash and clarifier wastewater consist of the following: two 785,000
gallon settling lagoons operated in parallel, two tablet dechlorinator tubes and an ultrasonic flow meter with
parshall flume.
VH. Records/Reports
A comparison of the November and December DMRs with the laboratory data from Statesville Analytical showed
that all data was transcribed onto the DMRs correctly, except for the December 17t` samples that were accidently
entered on the line for December 16t'. It was noted that the units of measurement (mg/L) for aluminum,
manganese, fluoride, zinc, copper, total nitrogen and total phosphorus were not being listed on the DMR. Please
include the unit of measurement for these parameters on the form.
VHI. EftluentlReceiving Waters
The system discharges into a tributary of the Yadkin River, class "WS-IV" waters in the Yadkin -Pee Dee River
Basin. The effluent was clear on the day of inspection.

Comment: A review of the monthly self -monitoring reports for the period January 2013

Davie County. It is our understanding that John Grey is currently working on the

Comment: A comparison of the November and December DMRs with the laboratory
data from Statesville Analytical (they perform analysis for all parameters except pH and
total residual chlorine) showed that all parameters were accurately transcribed onto the
DMR. It was noted that the samples collected on December 17th were accidently
entered on the line for December 16th. Additionally, it was noted that the units of
measurement (mg/L) need to be listed on the DMR for aluminum, manganese, fluoride,
zinc, copper, total nitrogen and total phosphorus.
The chronic toxicity test performed in February 2013 failed. It is our understanding that
this failure was due to the presence of excessive chlorine in the effluent. It should be
noted that the toxicity requirement in the permit is for monitoring only with no effluent
limit.

Stephen — Visited the WTP today and met with Mike Weir/ORC. They last applied residuals in 2005 (155 dry tons from
one settling lagoon) and again in 2010 (270 dry tons from both lagoons). Roy Whitaker with EMA uses a boat and
sludge judge to determine when solids need to be removed. No plans to pump in 2014 as storage in the two settling
lagoons is still adequate.
The facility has a single raw water storage lagoon. They pump to this lagoon from the intake structure at the Yadkin
River. They only have to pump from the river a few days a week in order to fill the lagoon. They try to pump at times
when the river is clearer, reducing the generation of excessive solids to the settling lagoons when the filters are
backwashed.

Dear Ms. Dirks:
Sue White of the Winston-Salem Regional Office performed a Compliance Evaluation
Inspection (CEI) on the Sparks Road Water Treatment Plant on February 13, 2013. Michael
Weir, ORC and Johnny Lambert, Director of Public Utilities were present for the inspection. The
inspection consisted of two parts: an on -site inspection of the treatment facility and a file
review. The following are the findings from the subject inspection:
I. Permit
The NPDES permit for the Spark Rd WTP became effective December 1, 2009 and will expire
on May 31, 2014. Please be aware that the permit renewal application must be received by
Division of Water Quality 180 days prior to the permit expiration date. The permitted
components of the water treatment facility are two filter backwash lagoons with dechlorination.
Solids from the settling lagoons are removed as necessary for land application under non -
discharge permit number WQ0010528.
II. Records/Reports
A review of the laboratory reports and Discharge Monitoring Reports (DMRs) for the Sparks
Road WTP showed no reporting violations during that period. The files and all required log
books were on -site and in order.
III. Facility Site Review
The facility site review indicated that the treatment works is consistent with the permitted
components. The actual treatment system consists of the abovementioned components. The
filter backwash flow is estimated using a v-notch weir.

NPDES permit is issued to Davie County at the Sparks Road Water
Treatment Plant and covers the operation of two parallel 785,000 gallon
filter backwash lagoons. Many new parameters have been added to our new
permit so 2010 will be exciting for us as we look forward to the challenge of
all the extra testing envolved.
During 2009, Davie County had no violations and we met all or
monitoring data and sampling requirements for the year. A chart of our
2009 annual averages and annual max for our monitoring data is found
below.

Mike — Saw your 2010 annual report come through. Thought I should let you know that this summer the
legislature amended the law so that annual reports are now only required from municipal/domestic facilities
with an average annual flow greater that 200,000 gpd. Since the WTP does not treat municipal or domestic
wastewater, you should be done with annual reports. See cite below. Mike.
§ 143-215.1C. Report to wastewater system customers on system performance; publication of notice of discharge
of untreated wastewater and waste.
(a) Report to Wastewater System Customers. — The owner or operator of any wastewater collection or
treatment works, the operation of which is primarily to collect or treat municipal or domestic wastewater and for
which a permit is issued under this Part and having an average annual flow greater than 200,000 gallons per day,
shall provide to the users or customers of the collection system or treatment works and to the Department an
annual report that summarizes the performance of the collection system or treatment works and the extent to
which the collection system or treatment works has violated the permit or federal or State laws, regulations, or
rules related to the protection of water quality. The report shall be prepared on either a calendar or fiscal year
basis and shall be provided no later than 60 days after the end of the calendar or fiscal year.

On March 23, 2010, Mike Mickey of this office met with Michael Weir, ORC,' to perform a
Compliance Evaluation Inspection on the NPDES wastewater discharge from the Sparks Road water
treatment plant. This type of inspection consists of two basic parts: an in office file review and an on -site
inspection of the treatment facility. The attached EPA inspection form notes the seven (7) areas that were
evaluated for this inspection. The findings and observations are outlined below:
I. Permit
The NPDES permit was reissued on December 1, 2009. Footnote "1" on the Effluent Limitations page of
the new permit states that continuous flow monitoring becomes effective on March 1, 2011. It is our
understanding that plans are already underway to install a flow meter at the start of the next fiscal year.
II. Self -Monitoring_ Program
Monthly self -monitoring reports were reviewed for the period January 2009 through December 2009.
The facility was found to be in complete compliance with all effluent limits for this time period (see data
summary attached). In addition, all monitoring was performed per the frequencies specified in the
permit. Field parameter certification was recently renewed for on -site testing of total residual chlorine.
Approval for on -site pH testing is pending.
III. Flow Measurement
Flow from the settling lagoons is currently estimated using the v-notch weir on the end of the discharge
pipe.

IV. Facility Site Review
Wastewater generated from clarifier and filter backwash is routed to two 785,000 gallon settling lagoons
in parallel. The supernatant overflow is dechlorinated prior to discharge. The facility plans to remove
solids from one settling lagoon this year for land application under permit number WQ0028806.
V. Operations & Maintenance
The operation and maintenance of the facility appeared to be excellent. No concerns were noted.
VI. Records/Reports
A review of the facility files showed that all records are being maintained according to the permit and that
all monitoring was performed per the frequencies listed in the NPDES permit. In addition, the 2009
annual report was submitted as required.
VII. Effluent/Receiving Waters
The system discharges into an unnamed tributary to the Yadkin River, Class "WS-IV" waters. The
effluent was crystal clear on the day of the inspection.

This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for the
analysis of environmental samples.
II. GENERAL COMMENTS:
The records were well organized and concise. The facility has all the equipment necessary to perform the
analyses. A few additional quality control procedures need to be implemented.
Mr. Weir has requested the deletion of Settleable Residue — Standard Methods, 18th Edition, 2540 F from
his certificate immediately, and is requesting the addition of pH — Standard Methods, 18th Edition, 4500 H+
B in 2010. Certification for pH will be granted when an acceptable performance testing sample is submitted
to this office directly from an approved vendor.

IV. PAPER TRAIL INVESTIGATION:
The paper trail consisted of comparing field testing records and contract lab reports to Discharge
Monitoring Reports (DMRs) submitted to the North Carolina Division of Water Quality. Data were
reviewed for Davie Co. Water - Sparks Rd. WTP for the months of August, September, and November,
2009. No transcription errors were detected. The facility appears to be doing a good job of accurately
transcribing data.
V. CONCLUSIONS:
Correcting the above -cited findings will help this lab to produce quality data and meet certification
requirements. The inspector would like to thank the staff for its assistance in the inspection and data

This topic was briefly discussed at the SWPS breakout on 10/1/2009. NPDES has since updated the Water Treatment
Plant permitting strategy to remove flow limits on all WTPs (flow monitoring requirement will remain). Unlike WWTPs,
flow data was not critical for treatment design purposes, and NPDES still has use of DMR flow data to conduct RPA
analysis. Also, the WTP strategy still precludes any new or expanding RO/IE plants to freshwater streams.
Given the number of WTPs with existing flow limits (approx 115) and potential cost for public notice to remove limit,
NPDES is proposing to remove existing WTP effluent flow limits upon permit renewal. It is recommended that Regions
use BPJ for any flow violations associated with water treatment plants, until the limits are eventually removed at permit

Dear Mr. Sledge:
Upon reviewing our draft NPDES permit NC0084212 we have some
concerns regarding the changes to our monitoring requirements. We
acknowledge the changes but are requesting some of the limits and
monitoring requirements be adjusted to better suit our facility.
Our first concern pertains to the monthly average flow limit of 0.023
MGD for our facility. We believe that this limit is a typo and should
actually be 0.230 MGD. Our previous permit allowed us to take weekly
instantaneous flow readings from our 45-degree v-notch weir. With the
monitoring requirement changing to continuous we will have to upgrade our
facility with a more accurate flow -measuring device that can communicate
to our scada system for data collection. After reviewing historical plant data
we have on the amount of plant discharge to our lagoons and with the
installation of a more accurate flow -measuring device we feel that during
some months we will not be able to meet the 0.230 MGD monthly average
flow limit. With these concerns we are asking that this monthly average
flow limit be increased to 0.360 MGD.
I have attached a simple excel spreadsheet that shows our monthly
data taken from the v-notch weir versus the data we have in the plant
pertaining to backwash sent to the lagoons. This data does not take into
account filter wasting that may take place during plant operations. The
highlighted data indicates months that we would not meet our proposed now
limit requirements of .230 average mgd.
Another concern is with the increased amount of effluent
characteristics being sampled and monitoring frequencies. We would like to
see footnote 4 be added to Aluminum, Total Zinc, Total Copper, Total Iron,
and Whole Effluent Toxicity. After sufficient data has been collected we
would like the opportunity to request a review of reasonable potential for
monitoring and effluent limitations as stated in footnote 4.
Davie County Public Utilities strives to protect our watershed and we
will continue to do everything in our power to meet our NPDES permit
requirements.

The draft permit contains a number of changes in the monitoring requirements from those found in your
current permit. These changes were made as part of the NPDES strategies for wastewater discharges
from potable water treatment plants, which are being implemented throughout the state as permits are
issued and/or renewed. Some of these changes are:
• The facility has been given a flow limit based on the highest reported monthly average flow that
occurred over the past three years. Flow monitoring has been changed to continuous, based upon the
reported occurrence of discharge. The permit provides a one year compliance schedule to allow the
facility time to purchase and install a suitable flow metering device.
• Monitoring requirements for Settleable Solids and Turbidity have been removed from the permit.
• Monitoring requirements for Calcium, Magnesium, Manganese, Fluoride, Copper, and pH have been
added based upon, studies of discharges from conventional water treatment facilities. Please note the
permit contains provision for reduction or removal of monitoring for particular parameters if it can
be shown there is no reasonable potential for their presence in the effluent.
• A monitoring requirement for Total Zinc has been added to the draft permit because the facility uses
zinc orthophosphate as a corrosion inhibitor.
• The monitoring frequency for Aluminum has been reduced to quarterly; monitoring for Total Iron
has been reduced to monthly.

2. The Division shall consider all effluent TRC values reported below 50 µg/L to be in compliance
with the permit. However, the Permittee shall continue to record and submit all values reported by

4. - After sufficient data have been collected (8 —12 data points over at least one year), the Permittee

A. (2.) CHRONIC TOXICITY MONITORING (QUARTERLY)
The permittee shall conduct uq arterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90 %. The testing
shall be performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during
the months of February, May, August and November. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGP3B. Additionally, DWQ Form AT-1(original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is.
required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test
form indicating the facility name, permit number, pipe number, county, and the month/year of the
report with the notation of "No Flow" in the comment area of the form. The report shall be submitted
to the Environmental Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately. Upon submission of a valid test, this monthly test
requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re-
opened and modified to include alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT
Forms submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to be completed no later
than the last day of the month following the month of the initial monitoring.

Bob - Below are my comments on the above draft permit for the Sparks Road WTP:
1) The monthly average flow limit of 0.023 MGD is too low. Is this a decimal error? The cover letter states that the limit
is based on the highest monthly average flow reported over the last three years. I pulled just the 2008 monitoring
reports from our files and the highest month was 0.177 MGD in October 2008. 1 would suggest that the limit be set
higher than any value they have reported to date. With anticipated growth in the county the WTP output will surely
increase causing higher backwash flows than previously reported. Perhaps the NPDES flow limit could be based on
maximum backwash flow at maximum WTP output so we don't create a noncompliance issue.
2) Suggest a grace period of 18 months instead of 12 months to have flow meter installed. Spoke with Mike Weir, ORC,
on Friday and he indicated that they are working to find funds for an open channel flow meter with sonar attached to
their SCADA system.
3) Recommend that footnote (4) also apply to Whole Effluent Toxicity testing. If the effluent does not exhibit toxicity
then monitoring could be discontinued.

Hey Mike, this is Michael Weir with Davie County Water, NPDES permit number NC0084212. I
have a few questions about our NPDES permit draft that you had faxed me today. Wondering if
you could maybe stop by or give me a ring at your convenience. Significant changes to say
the least, the one that I have the most questions about and the biggest concern is the flow.
I have already turned in my budget for next year and will have to add some funds to cover
this addition to our facility very quickly. There is a few different ways we can achieve
this and I wanted to run them by you or get a better feel for what the state is wanting or

1. The requirement for continuous flow monitoring will become effective 12 months following the
effective date of the permit.
2. The Division shall consider all effluent TRC values reported below 50 µg/L to be in compliance
with the permit. However, the Permittee shall continue to record and submit all values reported by
a North Carolina certified laboratory (including field certified), even if these values fall below 50
n/L.
3. The pH shall not be less than 6.0 standard units, nor greater than 9.0 standard units.
4. After sufficient data have been collected (8 —12 data points over at least one year), the Permittee
may request a review of reasonable potential for monitoring and effluent limitations.
5. Monitoring should be performed in conjunction with toxicity testing.
6. See Condition A. (2.) for toxicity testing requirements. Toxicity sampling should coincide with
sampling for parameters covered by footnote 5.
There shall be no floating solids or foam in other than trace amounts.

A. (2.) CHRONIC TOXICITY MONITORING (QUARTERLY)
The permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90 %. The testing
shall be performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during
the months of February, May, August and November. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGP3B. Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test
form indicating the facility name, permit number, pipe number, county, and the month/year of the
report with the notation of "No Flow" in the comment area of the form. The report shall be submitted
to the Environmental Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately. Upon submission of a valid test, this monthly test
requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re-
opened and modified to include alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT
Forms submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to be completed no later
than the last day of the month following the month of the initial monitoring.

In 2008 our facility showed exceptional performance in meeting all
laws, regulations and rules pertaining to water quality. Our current NPDES
permit # NCO084212 became effective 8/1/2004 and expires on 5/31/2009.
We are currently seeking renewal of our NPDES permit and are awaiting
response from the DWQ. The NPDES permit is issued to Davie County at
the Sparks Road Water Treatment Plant and covers the operation of two
parallel 785,000 gallon filter backwash lagoons.
During 2008, Davie County had no violations and we met all or
monitoring data and sampling requirements for the year. A chart of our
2008 annual averages for our monitoring data is found below.
