There are areas of the program that need your attention. Deficiencies exist related to inspection and maintenance
of your collection system as well as the required documentation and record keeping. These areas are required in the permit

Inspection Summary:
Dan Arnold/ owner, Randy Roberson/ property manager and WWTP ORC Joe David Hall assisted in the inspection of Et
Glen's collection system. (Mr. Hall is the ORC for the WUVTP).
This mobile home park was purchased September 23, 2011, by Dan Arnold of Dry Ridge Properties, LLC.
No maps, records, emergency plan or logbooks were available from the owner or property manager. This inspector supplied
copies of the map to the MHP and the emergency plan (from the previous owner) which were on file in our office. A copy of
the 2T regulations for collection systems was also supplied.
This collection system consists of laterals, collection main lines, one high priority line, eight manholes and no pump
stations.
The inspection began with the manholes which were noted on the map. Each manhole was opened and inspected.
MH #1: located at home #20/21: It appeared to be intact and clean.
MH #2: located at home #27: The manhole lid and ring are "offset," probably from being hit by a car. The edge of
the manhole has been breached and is allowing I&I (Inflow & Infiltration).
This manhole lid and ring should be repaired or replaced.
MH #3: located at home #28: Though marked on the map, this MH was difficult to locate. It was buried and covered
with silt and grass and had to be dug out. This MH had been flooded, I was told, by the nearby stream. It contained
silt and rags and should be cleaned out.
MH #4: located at home #32: There was a small amount of silt inside.
MH #5: located at home #12: It appeared to be intact and clean.
MH #6: located at home #6: It contained a few rags which need to be cleaned out.
MH #7: located near the VVVVfP: This manhole was stopped up and contained dark water;.,, This was the last
stop before entering.the VVVVTP and prompted an inspection of the VVWTP and the effluent"
pipe. Several unsuccessful attempts were made to unclog the manhole.
MH #8 This manhole was missed on the map and therefore not inspected.
The High Priority Line was located over Little Flat Creek and was contained in a concrete conduit. The current owner had
also placed concrete barriers on either side of the creek to prevent washout. Its located between home #20 and home #12
(across the creek from each other.)

Information on reporting Sanitary Sewer Overflows (SSOs) was given to Mr. Arnold and Mr. Roberson along
with copies of the "FOG" (fats, oils, grease) flyer in English and Spanish (Provided by MSD in Buncombe
County).
It would be a_good idea to add a request to not throw rags down the sewer system to the "FOG" pamphlet due
to finding rags in some of the manholes.
All sewage spills are to be reported within 24 hours of first knowledge of the spill.
All flooding from the creek to the collection system and manholes is also to be reported within 24 hours.

These documents will help to prepare you for a collection system
inspection.
You will need to obtain a map of the collection system, indicating:
Construction drawings, Miles or feet of collection system, Approximate age
of sewer lines, Line size(s), Pipe material(s) and Flow Direction.
Educational materials are required to be provided to users, twice a year,
regarding the proper disposal of fats, oils, grease (FOG), as well as other items,
i.e. diapers, etc. The documentation of the distribution of these educational
materials must be provided.

a chance to review these documents and prepare for the inspection.

this MHP. The actual values are much lower, A
major water leak was located and repaired in
November of 2008. Prior to that flows were 8,000
to 9,000 gpd. Individual water meters were
installed at each mobile home, however, due to
drinking water quality issues, homes are no
longer being billed based on individual usage.
After the installation of a new 3-stage treatment
system to remove high levels of iron and
manganese, individual billings will resume (in late
2010 or 2011).

SUBCHAPTER 02T .0403 PERMITTING BY REGULATION
(a) Collection systems having an actual, permitted or Division approved average daily flow less than
200,000 gallons per day are deemed permitted pursuant to Rule _0113 of this Subchapter provided
the system meets the criteria in Rule .0113 of this Subchapter and all specific criteria required in this
Rule:
(1) The sewer system is effectively maintained and operated at all times to prevent discharge to
land or surface waters, and to prevent any contravention of groundwater standards or surface
water standards.
(2) A map of the sewer system has been developed and is actively maintained.
(3) An operation and maintenance plan including pump station inspection frequency, preventative
maintenance schedule, spare parts inventory and overflow response has been developed and
implemented.
(4) Pump stations that are not connected to a telemetry system (i.e., remote alarm system) are
inspected by the permittee or its representative every day (i.e., 365 days per year). Pump
stations that are connected to a telemetry system are inspected at least once per week.
(6) High -priority sewers are inspected by the perrnittee or its representative at least once every
six -months and inspections are documented.
(6) A general observation by the permittee or its representative of the entire sewer system is
conducted at least once per year.
(7) Overflows and bypasses are reported to the appropriate Division regional office in accordance
with 15A NCAC 02B .0506(a), and public notice is provided as required by G.S. 143-215.1 C.
(8) A Grease Control Program is in place as follows:
(A) For publicly owned collection systems, the Grease Control Program shall include at least
biannual distribution of educational materials for both commercial and residential users and the
legal means to require grease interceptors for new construction and retrofit, if necessary, of
grease interceptors at existing establishments. The plan shall also include legal means for
inspections of the grease interceptors, enforcement for violators and the legal means to control
grease entering the system from other public and private satellite sewer systems.
(B) For privately owned collection systems, the Grease Control Program shall include at least
biannual distribution of grease education materials to users of the collection system by the
permittee or its representative.
(C) Crease education materials shall be distributed more often than required in Parts (A) and
(B) of this Subparagraph if necessary to prevent grease -related sanitary sewer overflows-
(9) Right-of-ways and easements are maintained in the full easement width for personnel and
equipment accessibility.
(10) Documentation shall be kept for Subparagraphs (a)(1) through (a)(9) of this Rule for a
minimum of three years with exception of the map, which shall be maintained for the life of the
system.
(b) Private collection systems on a single property serving an industrial facility where the domestic
wastewater contribution is less than 200,000 gallons per day shall be deemed permitted.
(c) The Director may determine that a collection system should not be deemed to be permitted in
accordance with this Rule and Rule .0113 of this Subchapter. This determination shall be made in
accordance with Rule .0113(e) of this Subchapter.

I would like to take this opportunity to discuss the requirements for sewerage collection
systems that were first established in 15 NCAC .02H .0200 in March 2000 and are now found in
15 NCAC 2T .0403, Waste Not Discharged to Surface Waters, which became effective
September 1, 2006. These Regulations place significant operation, maintenance and reporting
requirements on those entities that own or operate a wastewater collection system with average
daily flows of less than 200,000 gallons per day.
These regulations are applicable to your facility. This letter is provided as guidance
to assist you in complying with the new reporting and operations and maintenance (O&M)
requirements and to advise you that you are subject to system review, inspections and possible
enforcement, if the system is not in compliance with the regulation.
For your convenience and easy reference, a highlight of these requirements and the
following guidance are offered (see enclosed). You. may find the regulations using the following

The Asheville Regional Office will be increasing the level of oversight, compliance
activities and enforcement relating to collections systems, therefore, we wanted to be sure you
are aware of the requirements for these systems. We will be performing NPDES Wastewater
Collection System inspections sometime in the near future.

These values are not typical of the residents of
this MHP. The actual values are much lower. A
major water leak was located and repaired in
November of 2008. Prior to that flows were 8,000
to 9,000 gpd. Individual water meters were
installed at each mobile home, however, due to
drinking water quality issues, homes are no
longer being billed based on individual usage.
After the installation of a new 3-stage treatment
system to remove high levels of iron and
manganese, individual billings will resume (2011).

There are — 2800 linear feet of gravity collection
system lines, which are 8-inch pvc. There are no
lift stations. There is one aerial crossing (high
priority line). On 8-25-2011, a 300-400 feet of
lines were, tved, snaked and cleaned by
Haynes Plumbing (contractor).
Development: The park was built in the 1990s by Berry Boys
Inc. The park has 58 lots serving —100 residents.
(Note: There are 51 lots rented as of 6-24-2011.)

Development: The park was built in the 1990s and has 58 lots serving —100 residents. There are
currently — 50 lots rented.
There is approximately 2800 linear feet of 8-inch PVC lines.
There are no lift stations.
There is one aerial crossing (high priority line) which crosses Little Flat Creek and is
inspected routinely (> 2/yr).
MB Haynes (plumbing contractor) pumped the manholes, tved, snaked & cleaned — 300-400 feet
of the collection system (>10%) on August 25th, 2011. Next year, the aerial line will be
included in the cleaning schedule.
The collection system map was complete and recently updated in May 2011 by John Coxey, PE.
The FOG program educational materials are distributed to each resident twice a year and are in
English and Spanish.
There have been no SSOs. The owner and property management staff have the 2T regulations, the
24-hour and 5-day SSO guidelines, reporting forms and contact information, in the event of an
SSO.

EDEN GLEN MOBILE HOME PARK
HAZARD COMMUNICATION, ASSESSMENT,
SPILL RESPONSE PLAN AND PROCEDURES
PURPOSE:
This plan is to be implemented in the event of a Sanitary Sewer Overflow (SSO) or spill
within the Wastewater Collection System. This plan was developed to ensure that key
personnel respond to every spill event within sixty (60) minutes of a reported event.
INITIAL RESPONSE:
Spills reported to or detected by collection system personnel require immediate
attention. The Collection System ORC and / or Back-up Operator will make the
preliminary assessment of the spill within 15 minutes after receiving report. That
assessment includes visiting the spill site. The ORC or Back-up Operator will determine
the appropriate response and request assistance from key personnel, as necessary. The
contact list will be updated annually and include essential response equipment and
personnel. Local contractors may be employed, as deemed necessary.
Key personnel will, upon assessing the site, continue to document the event and
determine the severity, magnitude and potential environmental impact. Every effort
should be made to minimize the spill event consistent with the SOURCE CONTROL AND
CONTAINMENT PROCEDURES section of this plan. The safety of all response personnel
should never be compromised.
Key personnel should implement recovery actions to eliminate the cause of the
obstruction and / or system failure as outlined in this plan.
POST SPILL RESPONSE (REPORTING):
After containment is achieved, the Division of Water Quality, (DWQ) must be contacted as
soon as possible but, in no case later than 24 hours after initial detection if;
1) the spill has reached surface waters, "waters of the State" or
2) the spill exceeds one thousand gallons, (1,000 gallons)
Spills that are fully contained and removed quickly and are less than one thousand
gallons are not required to be reported to DWQ. However, a full spill report form will be
filled out and kept on file. All spills must be logged .in, regardless of size, and include the
incident date, time, duration, approximate volume and name of respondent(s).
SOURCE CONTROL AND CONTAINMENT PROCEDURES:
Depending on the nature of the spill or overflow, the initial assessment should indicate
the priority of source control and/or containment.
1) When the source can be isolated through valves, plugs or other methods, the
flow should be discontinued immediately or reduced to the least possible
volume.
2) If the pumps on the location are not operating the control panel should be
checked as follows:
a. Is the Power on?
b. Check the breaker.
c. If the breaker is closed, check the thermal overloads (external button).
d. Is the overload good? Check the fuses, if you are trained to do so.
e. Is the pump straining or clogged with debris?
f. Do you smell a burnt motor? (contact from attached list)
g. Is an electrician needed? (contact from attached list)
3) Containment methods will typically include;
a. Sand or dirt dikes and dams (for small streams & creeks).
b. Holes or pits (to prevent run off to surface waters).
If the spill event resulted from an area wide power outage, you should contact Power
Company and identify your site as critical. (Use the account number on the attached list.)
If the problem is mechanical in nature, retrieve parts from inventory and make repairs.
Re -order parts as they are used.
If the problem results from a sewer blockage, call for a jet-rodding machine (contact
contractor from attached list).
SPILL RESPONSE PROCEDURES FOR SPILLS OCCURRING:
ON MAJOR INTERCEPTOR / RIGHT OF WAY
1. Put up warning tape ("DANGER / CAUTION - DO NOT ENTER") around
the SSO site to restrict access.
2. In open areas, remove debris and lime site. Rotor -till, if needed.
Re -seed and mulch with straw.
3. In areas where there is under growth and small trees, flush area with
potable water until no waste remains. Wait twelve to twenty four
hours and then lime the area. Be certain that lime does not flush to
the receiving stream. NOTE: DO NOT LIME, IF RAIN IS FORECASTED.
IN CITIZEN'S YARD
1. Till entire affected area, if possible. Apply lime and follow with seed
and straw.
2. If waste/debris is not visible, apply lime and 2 inches of top soil and
mix thoroughly. Apply lime lightly, seed and mulch.
ON ASPHALT OR CONCRETE
Set up containment at or near the storm drainage system. Do not let
the spill enter the storm drain system.
2. Put up warning tape ("DANGER / CAUTION - DO NOT ENTER") around
the SSO area.
3. Spread lime on spillage and sand or apply other material to a depth of
one inch. Let this material absorb for one hour, then sweep up.
Dispose of debris appropriately. If spill is still evident, repeat
application.
4. After removing the contaminated material, wash down area with high
pressure water and bleach.
5. Pump the clean up materials and wash water into sanitary sewer.
IN A CREEK OR STREAM
1. Take conductivity readings of the receiving stream upstream and
downstream to determine where a temporary dam should be placed.
2. The dam should be constructed near a manhole which is accessible to
the jet rodder and vacuum truck.
3. The dam should be placed below the spill as indicated by low
conductivity readings.
4. Construct a dam sized to contain the flow of water, plus any flush
water added during the clean up.
5. Station pumps at the clean-up site, one to use, one for backup.
6. Have a back hoe at the site for dam repairs as needed.
7. Wash down all of the rip rap along the stream bank until no residue is
evident.
8. Put up warning tape ("DANGER / CAUTION - DO NOT ENTER") at spill
site and 400 feet downstream.
9. Take upstream and downstream samples for fecal coliform.
10. Repeat sampling until normal-, background counts are 200 fecal
coliform colony forming units/100 mi.
11. If settled sludge is evident, sweep bottom until clean.
12. If creek or stream is recreational waters (swimming), notify the
affected community and local officials, as soon as the problem is
discovered.

Summary of System Performance July 1, 2008 to June 30, 2009
July 10, 2008: Eden's Glenn Mobile Village received a notice of violation for failing to monitor
total residual chlorine two times a week of 2/2/08. Total residual chlorine was monitored and
the discharge monitoring report was amended. Analytical results indicate that no permitted
monthly limits were exceeded during the reporting period. In addition, no bypasses or
overflows occurred.
