However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on July 17, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through D, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. Application: It is mentioned in section V.S. of the Wastewater Irrigation System application form (FORM: WWIS 06-16) that automatically activated standby power onsite capable of power all essential treatment units would be provided under design conditions in accordance with 15A NCAC 02T .0505(1). The cover letter mentions that this would be addressed with a backup generator, and the Power Reliability Plan in the preliminary Operation and Maintenance Manual states that a backup generator and automatic transfer switch is currently employed at the system. However, the current permit does not include this generator in the facility description, nor are the auxiliary power permit conditions included in the permit issued December 10, 2019. The equipment information in V.1 l.h. do not propose any specifications for an "Auxiliary Generator", the engineering plans do not depict this generator, and the specifications do not mention the installation of one. Please provide information regarding this backup generator that meets the requirements of 15A NCAC 02T .0505(1) so that it may be incorporated into the permit. 2. The WWIS 06-16 form was signed by Gary Gardner, the Engineering Section Chief. According to 15A NCAC 02T .0106(a)(4), the application shall be signed by either an executive officer, elected official in the highest level of elected office, or other authorized employee in the case of a municipal, state, or public entity. The attached signature authority delegation letter shall be signed in accordance with 15A NCAC 02T .0106(d) to authorize Mr. Gardner to sign this permit application on behalf of the North Carolina Wildlife Resources Commission. Alternatively, the WWIS 06-16 form shall be signed by a signature authority and resubmitted. On file, we have Melinda Patterson, the Center Director, as having signature authority. B. Engineering Plans: 1. See Item A. 1. Please provide location and detail of this generator and automatic transfer switch, and show the connections to treatment units that use it as an auxiliary power source. 2. The facility description of the previous permit mentions a 4,000 gallon baffled septic tank that is not depicted in the engineering plans. Please provide updated plans with the location of this septic tank and associating piping and appurtenances, or clarify its omission. 3. The facility description of the previous permit mentions a tablet chlorinator that is not depicted in the engineering plans. Please provide updated plans with the location of this tablet chlorinator and associating piping and appurtenances, or clarify its omission. C. Engineering Calculations: 1. Both the plans and the specifications reference an EZ-Treat Model 4L Filter media pod as the chosen fixed media pods. Sizing for these units appears to be based on manufacturer and NSF Certification data which specify removal rates of biological oxygen demand (BOD), total nitrogen (TN), and total suspended solids (TSS) at certain loading rates. However, I cannot find any manufacturer's information for the Model 4L filter which would support these removal rates. Please provide the manufacturer and NSF data that support these sizing and removal rate calculations. 2. Section 3.2 — "Media Pod Dosing System" of the specifications specify a Sta-Rite Model STEP 50 1.0 HP 3 stage 230 V pump. Please provide a pump performance curve for this pump. D. Power Reliability Plan: 1. See A. 1. Provide the technical specifications of the referenced generator.
