4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The existing Fifth Street Landing Boat Access Area is located on Roanoke Rapids Lake and has been in operation for over 30 years. The area is used primarly for recreation, and
includes the boat ramp, ADA parking, and paved parking for patrons of the boat ramp. The existing concrete ramps, tongue and groove groins, and fixed docks have reached the end
of their life cycle and are in need of replacement. NCWRC is proposing demolition of the existing structures and replacement with new concrete ramps, newgroins, and one (1) new
floating dock separating the concrete ramp bays.
4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past?*
r Yes r No r Unknown
4d. Attach an 8 1/2 X 11 excerpt from the most recent version of the USGS topographic map indicating the location of the project site. (for DWR)
4e. Attach an 8 1/2 X 11 excerpt from the most recent version of the published County NRCS Soil Survey map depicting the project site. (for DWR)
4f. List the total estimated acreage of all existing wetlands on the property:
0
4g. List the total estimated linear feet of all existing streams on the property:
0
4h. Explain the purpose of the proposed project:*
The eAsting concrete ramps, tongue and groove groins, and fixed docks have reached the end of their life cycle and are in need of replacement. NCWRC is proposing demolition of the
wasting structures and replacement with new concrete ramps, new groins, and one (1) newfloating dock separating the concrete ramp bays. This Will allowcontinued usage of the site
and provide a safer facility for the general public.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
NCWRC is proposing demolition of the epsting structures and replacement with new concrete ramps, new groins, and one (1) newfloating dock separating the concrete ramp bays.
Each concrete ramp would be approArnately 16' in width with approArnately 15' of usable space for off loading watercraft. A concrete abutment would be installed in between each ramp
for the floating dock. Two newsheet pile groins will be installed on both sides of the ramp. All work will take place in the existing ramp footprint.
Equipment will consist of excavators, bulldozers, and dump trucks. The ramp area will be closed during construction so indirect impacts will be at a minimum.
4j. Please upload project drawings for the proposed project.
5. Jurisdictional Determinations
5a. Have the wetlands or streams been delineated on the property or proposed impact areas?*
r Yes r No
Comments:
No work will takeplace outside the epsting footprint of the ramp
5b. If the Corps made a jurisdictional determination, what type of determination was made?*

SEEDING NOTES
AFTER AUG. 15, USE UNSCARIFIED SERICEA SEED.
WHERE PERIODIC MOWING IS PLANNED OR A NEAT APPEARANCE IS DESIRED, OMIT SERICEA
AND INCREASE KOBE LESPEDEZA TO 40 LB/ACRE.
TO EXTEND SPRING SEEDING DATES INTO JUNE, ADD 15 LB/ACRE HULLED BERMUDA GRASS,
HOWEVER, AFTER MID—APRIL IT IS PREFERABLE TO SEED TEMPORARY COVER.
NURSE PLANTS
BETWEEN MAY 1 AND AUG. 15, ADD 10 LB/ACRE GERMAN MILLET OR 15 LB.ACRE
SUDANGRASS. PRIOR TO MAY 1 OR AFTER AUG. 15, ADD 40 LB/ACRE RYE (GRAIN).
SEEDING DATES
BEST POSSIBLE
FALL: AUG. 25—SEPT. 15 AUG. 20 — OCT. 25
LATE WINTER: FEB. 15—MAR. 21 FEB 1. — APR. 15
FALL IS BEST FOR TALL FESCUE, AND LATE WINTER FOR LESPEDEZAS. OVERSEEDING OF
KOBE LESPEDEZA OVER FALL —SEEDED TALL FESCUE IS VERY EFFECTIVE.
SOIL AMENDMENTS
APPLY LIME AND FERTILIZER ACCORDING TO SOIL TESTS, OR APPLY 4,000 LB/ACRE GROUND
AGRICULTURAL LIMESTONE AND 1,000 LB/ACRE 10-10-10 FERTILIZER.
MULCH
APPLY 4,000 LB/ACRE GRAIN STRAW OR EQUIVALENT COVER OF ANOTHER SUITABLE MULCH.
ANCHOR STRAW BY TACKING WITH ASPHALT, NETTING, OR A MULCH ANCHORING TOOL. A
DISK WITH BLADES SET NEARLY STRAIGHT CAN BE USED AS A MULCH ANCHORING TOOL.
MAINTENANCE
REFERTILIZE IN THE SECOND YEAR UNLESS GROWTH IS FULLY ADEQUATE. MAY BE MOWED
ONCE OR TWICE A YEAR, BUT MOWING IS NOT NECESSARY. RESEED, FERTILIZE, AND MULCH
