Attached are three (3) copies of the NOI and supporting documents, which are our request for
coverage under General NPDES permit NCG200000. We have enclosed and a check for $ 100
(application fee.)
This is an existing facility that has not previously held a stormwater permit. In the course of
reviewing the facilities operations, it was determined that there are exposed materials'and
operations that necessitate our application for coverage under the general discharge permit for
our industry.
The primary industrial activity at this facility is the recycling/redistribution of alloy metals (i.e.
titanium alloys, nickel alloys, stainless steel alloys, etc.) The facility currently receives scrap
metal materials ( cuttings, turnings, grindings, etc.) which they clean, size, package, etc. for
outside sales to steel manufacturers for use as ingredients in producing steel alloys.

Attached are copies of the completed report forms with the results of the stormwater analytical
sampling and the visual analysis performed October 26, 2018. This was the late collection of
samples for the first half of 2018 and is noted on the DMR.
We regret that we lost track of our sampling schedule and forgot to collect the first sample before
the end of June. We have collected this ample as soon as an opportunity presented itself (i.e. rain
event) after discovering our mistake. We will collect a second sample before the end of this year
so that we will at least have collected two samples for the year.
