Dear Mr. Johnson:
In accordance with your application for a discharge permit received on October 5, 2016, we arc
forwarding herewith the subject certificate of coverage to discharge under the subject state — NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection
Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Energy, Mineral, and Land Resources. The Division may require modification or revocation
and reissuance of the certificate of coverage.
This general permit allows the discharge of stormwater and certain types of wastewater from your
facility. In your application, you specified that only mine dewatering water would be discharged from
this facility. If separate point -source discharges of stormwater occur (i.e., stormwater not combined with
mine dewatering water), you will be required to adhere to the conditions in this permit for stormwater
discharges.
Also, you may be required to apply for a separate Post -Construction stormwater permit from the
State or local government (Winston-Salem in this case) if you are adding built -upon area at this site.

This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Energy, Mining, and Land Resources, or permits required by the Division of Water
Resources, Coastal Area Management Act, or any other federal or local governmental permit that may be
required. '

standards and regulations promulgated and adopted by the North Carolina, Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,

to receiving waters designated as Silas Creek, a class C water in the Yadkin -Pee Dee River
Basin, in accordance with the effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, Ill, IV, V, and VI of General Permit No. NCG020000 as
attached.
This certificate of coverage shall become effective February 3, 2017.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

We received the attached NOI from Vulcan Construction Materials, LLC for coverage under NCG02 for the 412 Quarry,
an inactive mine site in Winston-Salem (4900 Kester Mill Road) under mining permit 34-01. The quarry would dewater
to Silas Creek, class C waters in the Yadkin -Pee Dee River Basin. A copy of the NOI application is attached; it is the
revised application in response to an additional information request re: Question 38 earlier this month.
Could the WSRO advise us on whether to proceed with issuance of a COC for this site?
