Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of coverage. Please note that mine dewatering wastewater includes wet pit overflows caused solely by direct rainfall and groundwater seepage for this type of mining operation. Any discharge of this water (and any stormwater commingled with it) is subject to the effluent limitations in this permit. Additionally, new coastal stormwater management rules (SESSION LAW 2008-211) were irnpiemented October il, 2008. Due to these new rules you may be required to appiy for an additional Coastal State Stormwater Permit. (2) For a partnership or sole proprietorship: by a general partner or the proprietor, respectively; or (3) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected official. I hereby request coverage under the referenced General Permit. I understand that coverage under this permit will constitute the permit requirements for the discharge(s) and is enforceable in the same manner as an individual permit. We have met on -site with DWQ representative Linda Willis of the Wilmington Regional office in regards to our renewal of this mining permit. After a site review, Ms. Willis determined that we need a discharge permit for this pit because of an outlet pipe installed in the southern end of the settling pond. We do not dewater this pit and the only disch ge that may occur would be from an extreme rain event. Ross Jr., Secretary The Solid Waste Section reviewed your Land Clearing and Inert Debris (LCID) landfill notification for the above referenced address received on September 7, 2005. Based on the information submitted in the notification, approval for operation of a LCID landfill is hereby granted for disposal activities at the referenced property as long as the facility does not exceed 2 acres in size. Should your landfill disposal facility exceed 2 acres in size, you will need to make application to the Solid Waste Section for an individual LCID disposal permit. Please be aware that the application and siting requirements for an individual LCID permit are more stringent. Construction and operations involving sedimentation and erosion control activities greater than 1 acre typically require approval from the Land Quality Section. You should contact the Land Quality Section at (919) 733-4574 to determine whether your disposal activities require approval. 8) Ground water quality at this facility is subject to the classification and remedial action provisions of 15A NCAC 2L, Classifications and Water Quality Standards. Please contact the Water Quality Section ( 1 -877- 623-6748) for additional information. 9) Closure of the facility shall be in accordance with 15A NCAC 13B .0566(5), which requires a minimum of l foot of suitable soil cover sloped to allow surface water runoff in a controlled manner be applied within 120 calendar days after completion of any phase of disposal operations or upon revocation of a permit.
