In accordance with your application for a discharge permit received on May-24, 2006,
and the issuance of your Division of Land Resources mining permit on October 23, 2009, we are
• forwarding herewith the subject certificate of coverage to discharge under the subject state—
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina
General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
US Environmental Protection Agency dated October 15; 2007 (or as subsequently amended).
This general permit allows the discharge of stormwater from your facility. In your'•
application, you specified that only stormwater would be discharged from this facility. If •
separate point-source discharges of wastewater occur(i.e:, mine dewatering water or other
process wastewater), you will be required to seek a modification to this permit. Any.
• modification to add a wastewater discharge for this facility would have to be reviewed and •
issued prior to that discharge occurring at your facility.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land
Resources, Coastal Area Management Act or any other federal, state or local governmental
permit that may be required.

Dear Mr. McMillan:
Engineering & Environmental Science Company (E2S) has completed the mine map for
the above-referenced site. The subject site is located in Stokes County (see Figure
attached, and provided mine maps-Sheets 1, 2, and 3).
Proposed for the mine are 60 sediment basins. Only exposed ground due to mining
activities will be conducted on the subject property.
It is proposed to sample two (2) discharge points at any given time with the sample points
represented the largest and smallest representative exposed water shed.

1. I agree to abide by the approved Mining Permit for this mining activity. (A copy of the valid
mining permit must be attached to this request.
2. I agree to not discharge any sanitary wastewater from this mining activity except under the
provisions of another NPDES permit specifically issued therefore.
3. I agree that bulk storage of petroleum products and other chemicals shall have adequate
protection so as to contain all spills on the site.
4, I agree that solid wastes will be disposed of in accordance with N.C.statutes and rules
governing solid waste disposal.
5. I agree that maintenance activities for vehicles and heavy equipment will be performed so as to
not result in contamination of the surface or ground waters.
