For coverage under NPDES General Permit NCG020000, the Division of Energy, Mineral and Land
Resources (DEMLR) is forwarding herewith the reissued Certificate of Coverage and
Stormwater/Wastewater General Permit. This permit is reissued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently
amended.) The following information is included with your permit package:
• New Certificate of Coverage
• Stormwater/Wastewater General Permit
• Technical Bulletin for the General Permit
• Discharge Monitoring Report (DMR) Forms
• Annual Discharge Monitoring Report Forms
• Qualitative Monitoring Report Forms
The General Permit authorizes discharges of stormwater and/or wastewater (as applicable to your site),
and it specifies your obligations with respect to discharge controls, management, monitoring and record
keeping. Please review the new permit to familiarize yourself with all changes in the reissued permit.
Significant changes to the General Permit are outlined in the Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DEMLR, This
permit does not affect the legal requirements to obtain other permits which may be required by the N.C.
Department of Environmental Quality, nor does it relieve the permittce from responsibility for
compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order,
judgment or decree.

Our office received your Notice of Intent (NOI) for coverage under the NPDES Stormwater Mining
permit (NCG020000) on May 1, 2013. After review of the application request and check of our files it
appears that this mining site currently holds coverage under the NPDES general permit Certificate of
Coverage Number NCG020544. This permit coverage is active and in effect through the end of December
2014. Because coverage is already in place, we are returning your application package and the attached
fee in the amount of $100.
Your application notes that mine dewatering activities will take place as part of your operation and
that the water will be directed to an onsite lake. You should work with staff in the Division of Water
Quality's Wilmington Regional Office to determine the need for an approved Pumping Operation and
Monitoring Plan for these activities. Information on the need for these plans can be found in Part III
Section E of your NPDES permit.
Coverage under NCG020544 is tied to coverage for your facility under the Division of Energy,
Minerals and Land Resources' (DEMLR) mining permit. Compliance with the DEMLR mining permit is a
condition of our stormwater NPDES permit. It is our understanding that your DEMLR mining permit has
expired and that you are working to get your facility repermitted. Mining activities should not begin again
at your site until you have received your mining permit from DEMLR.

Note: Wetlands must be CLEARLY DELINEATED on the site plan. Mine dewatering activities that have
the potential to drain wetlands or otherwise impact surface water or groundwater MUST develop and
implement a Pumping Operation and Monitoring (0&M) Plan that has been approved by the
Division.
For sites that may drain wetlands — approval of a Pumping 0&M plan maybe required prior to
coverage under this general permit. The Pumping O&M Plan should be submitted to the
appropriate Regional Office and must be approved prior to operation.
The plan shall include, but is not limited to. (1) Groundwater monitoring strategies to demonstrate the
effect of pumping and to establish any pumping regime necessary to reduce impacts, and (2) Detailed

employing more than 250 persons or having gross annual sales or expenditures exceeding 25 million (in second quarter 1980 dollars), if
authority to sign documents has been assigned or delegated to the manager in accordance with corporate procedures.
(2) For a partnership or sole proprietorship: by a general partner or the proprietor, respectively; or
(3) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected official.
I hereby request coverage under the referenced General Permit. I understand that coverage under this permit will
constitute the permit requirements for the discharge(s) and is enforceable in the same manner as an individual
permit.

In response to your renewal application for continued coverage under general permit NCG020000 the Division of Water
Quality (DWQ) is forwarding herewith the reissued stormwater general permit. Please review the new permit to familiarize
yourself with the changes in the reissued permit. The general permit authorizes discharges of stormwater and some types of
wastewater. You must meet the provisions of the permit for the types of discharges present at your facility. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
