Subject: Return Rocky River Quarry discharge point relocation request, Cabarrus County
Dear Mr. North:
The Division received your NOI and request to modify the subject facility's stormwater
permit to relocate your permitted discharge outfall today. We are returning your request
unprocessed on the following bases.
a) You request modification to DWQ permit NCG020096, but this permit number is
assigned to the Salem Stone facility at 4572 High Point Road in Forsyth County, not
the Rocky River facility in Cabarrus County.
b) Under a General Permit, such as NCG020000, DWQ cannot provide a permit
modification to accommodate the relocation of outfalls. We can provide a letter of
approval for relocating the discharge point based on our review of a more detailed
site plan that better illustrates the physical arrangement of the outfalls, the site
features, and the flows of stormwater and wastewater.
We are returning your two submittal packages, each containing a cover letter, an NOI, a
copy of a Certified Mail receipt, and a photocopy from a partial 1:24,000 USGS quad sheet.

Quality (DWQ) is forwarding herewith the reissued stormwater general permit. Please review the new permit to familiarize
yourself with the changes in the reissued permit. The general permit authorizes discharges of stormwater and some types of
wastewater. You must meet the provisions of the permit for the types of discharges present at your facility. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.

The Winston-Salem Regional Office staff reviewed your request for a determination that Stormwater
Discharge Outfall (SDO) 001 for Land Disturbance and Process Areas be granted representative outfall
status. Based on the information and map provided, and our site visit on :tune 5, 2008 by Steve Tedder,
Rose Pruitt, and George Smith, all of this office, xve are making the following ROS determination.
In accordance with 40 CFR §122.21(g)(7), you are authorized to sample outfall number 001 as a
representative outfall. This approval is effective with the next sampling event. We. also want to remind
you that the permit still requires Qualitative Monitoring be performed at all SDOs, regardless of
representative outfall status.
Please annend this letter to vour Stormwater Permit to document that representative outfall status has
been approved. If changes in drainage areas, structures, processes, storage practices, or other activities
occur that significantly alter the basis of this approval, notify this office for an updated ROS

A letter of request for an Authorization to Construct was received October 28, 1996 by the Division
and final plans and specifications for the subject project have been reviewed and found to be satisfactory.
Authorization is hereby granted for the construction of two new settling cells with discharge of treated
wastewater into Salem Stone Quarry's Recycle System.
This Authorization to Construct is issued in accordance with Part I, Paragraph 1. of NPDES
Permit No. NCG020000 issued June 28, 1993 and shall be subject to revocation unless the wastewater
treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No.
NCG020000,
The sludge generated from these treatment facilities must be disposed of in accordance with G.S.
143-215.1 and in a manner approved by the North Carolina Division of Water Quality.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by this
Division, such as the construction of additional or replacement wastewater treatment or disposal facilities.
The Winston-Salem Regional Office, telephone number [phone] shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from 8.00
a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed in
accordance with the NPDES Permit, this Authorization to Construct and the approved plans and

Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina

The issuance of this Authorization to Construct does not preclude the Permittee from complying
with any and all statutes, rules, regulations, or ordinances which may be imposed by other government
