4a. Describe the existing conditions on the site and the general land use 1n the vicinity of the project at the time of this application:*
The proposed project area encompasses approximately 25.5 acres at the Lake Gaston Dam and tailwaters of Roanoke Rapids Lake immediately downstream of the dam. The dam and
side slopes are steep and either maintenance mowed or armored with riprap. The south abutment of the dam is occupied by the Gaston Hydro Power Station and a maintained day use
recreational area. The north dam abutment is steep, partially wooded, and partially cleared for an overhead power line. Shoreline areas along the toe of the dam and north shore of
Roanoke Rapids Lake are a mix of open and shrub -dominated areas, and contain several small wetland areas. A USGS topographic map showing the project location is provided as
Attachment 1. Soils mapping covering the project area is provided as Attachment 2.

4h. Explain the purpose of the proposed project:*
The purpose of the proposed project is to replace eAsting eel passage structures that were intended as temporary structures with new, upgraded, permanent eel passage structures.
The structures allow migrating American eels to scale the dam, be temporarily collected in traps, and ultimately released into Lake Gaston above the dam. Operation of the eel passage
structures is a condition of Dominion's current FERC hydropower operating license for the dam.
41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
Dominion is proposing to replace two eAsting temporary eel passage structures at the Lake Gaston Dam on the Roanoke River near Henrico, NC. The eAsting eel passage structure at
the north end of the dam was washed away in March 2019 during a high flood event. The replacement eel passage structures will be similar in location and configuration to the eAsting
structures, but will be upgraded, more permanent structures to accommodate increased numbers of eels observed in recent years. Principal project elements include neweel
conveyance pipes ("ladders"), holding traps, canopy structures, floating gangways and boat docks, and a new gravel access road to facilitate access to the north eel passage structure.
The proposed road will extend from a turnaround area at the north dam abutment, down steep slopes along the face of the dam, and then along the base of the dam to the new north
eel trap location. The new road is proposed to be constructed of crushed stone and be suitable for heavy truck access. Proposed jurisdictional "fill" activities include concrete footers for
the new eel ladders, additional stone fill and grout to create a stable foundation for footers, temporary cofferdams, and stone and rock fills associated with construction and bank
stabilization for the new access road. A total of 0.063 ac. of wetland impacts and 0.094 ac. of open water impacts are anticipated. Anticipated construction equipment includes
bulldozers, front end loaders, backhoes, dump trucks, concrete trucks, a vibratory or sheepsfoot compactor, and various portable power tools. Project plans are provided as Attachment
3.

The project has been designed to minimize permanent fills necessary to install the permanent eel passage structures and new access road. The eel
passage structures design maximizes use of floating elements (e.g. gangways, boat dock) for access in order to minimize fills.The access road to the
north structure will be unpaved, follows epsting topography, and is sited to minimize wetland impacts and shoreline stabilization fills.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
Temporary cofferdams and turbidity curtains will be employed to minimize temporary sedimentation impacts for in -water work on the eel passage
structures. Shoreline areas and non -impacted wetland areas will be protected using silt fence and turbidity curtains during construction of the new
access road.

IPaC and NCNHDB queries and field studies for Red -Cockaded Woodpecker (RCW) and Bald Eagle were conducted for the project area. No suitable
habitat for RCW was identified. No existing Bald Eagle nest sites or suitable nesting habitat were identified. Four transient Bald Eagle individuals were
observed perching near and flying over the Lake Gaston Dam during field investigations. No adverse impacts to these individuals or other federally
protected species are anticipated (see also Attachment 5).

Lake Gaston Eel Ladders Replacement Project
Pre -Construction Notification
Dear Mr. Philips and Ms. Stygar:
Virginia Power and Electric Company d/b/a Dominion Energy Virginia (Dominion) is planning to replace
existing temporary eel passage structures with new, upgraded, permanent eel passage structures at Lake
Gaston Dam located in Northampton and Halifax counties, NC. These structures will allow migrating
American eels to scale the dam. The eels will be trapped, and then released into Lake Gaston above the
dam. The operation of the eel passage structures is required by the facility's current FERC hydropower
operating license. The project is planned to be completed in 2020.
Dominion is requesting the authorization of the project under the USACE Nation Wide Permit 43. An
application and supporting documentation was prepared and is being submitted for your review and
processing.
