After reviewing the submitted documents, the Division has determined the application is incomplete and cannot be processed. The application is on hold until all of the following information is received: 1. Please provide the lengths of proposed impacts within the French Broad River for the boat ramp and the river bank boulder toe. These lengths should be measured parallel to river flow. [15A NCAC 02H .0502] Detail 2 on Sheet 6 (Typical Coffer Dam) on the updated plan sheets appears to depict a series of settling ponds constructed behind coffer dams within the French Broad River. The settling ponds are acceptable to treat turbid water pumped from the isolated area behind the coffer dam. However, they must be constructed in an upland area (i.e. not in the footprint of the river). Please note that Special Stilling Basins (Section 4.8.4 of the NCDOT Erosion and Sediment Control Design and Construction Manual) have also been shown to be effective in these situations. Please revise and re -submit Detail 2 on Sheet 6 without the series of settling ponds depicted within the river. [15A NCAC 02H .0506(b)] Pursuant to Title 15A NCAC 02H .0502(c), the applicant shall furnish all of the above requested information for the proper consideration of the application. Below are my comments/questions that need to be addressed before PCN is considered complete: 1. On Plan Sheet LO.4, there's a wetland impact that appears unaccounted for (see red circle on attached plan sheet). This will need to be included in the PCN if not already. 2. For the boat ramp, will need linear footage of impact to river bank as well as cubic yards of material excavated and/or filled below OHWM. Also, will need to provide information as to why ramp will be wider than 20-feet and please address what type of/expected boats will be using the ramp. This will need to be included in the PCN. 3. Construction sequencing details for boat ramp notes installation of coffer dam. Will need details submitted for the coffer dam and if below OHWM then will need to include in the PCN as temporary open water impact. 4. 1 will process the PCN under NWP #42 which should cover the wetland fill and the boat ramp — Andrew does this work for you? 5. Bedding material consisting of a well graded 3" to 4" cobble and sand mixture or approved native alluvium shall be placed to a depth no greater than 12 inches, on top of all installed filter fabric, prior to the placement of boulder so as to prevent tearing or ripping. Boulder A. B. C E F Individual stone boulders shall be dense, sound and free from cracks, seams and other defects conducive to accelerated weathering. The boulders shall be weathered to sub -rounded in shape. At a minimum exposed rock should have one flat surface and this should be the only exposed surface. Boulders shall be sound, hard, durable particles free from silt, clay, shale, sandstone, flaky particles, topsoil, organic matter, and other deleterious materials. a.The Contractor shall remove silt, clay, topsoil and other deleterious materials from the surfaces of the boulders in such a way as to prevent excess turbidity levels within the river or inhibit the bond with concrete, prior to transporting the boulders to the site. The Contractor shall stockpile the materials on -site in a manner that requires the minimum amount of material handling and that prevents the contamination of the boulders with underlying soils. a.Placement of a gravel bed may be necessary as directed by the Engineer. All boulder shall have the following properties: a.Bulk specific gravity (saturated surface -dry basis) not less than 2.5. b.Absorption not more than 2% by weight. c.Rock that fails to meet these requirements may be accepted only if similar rock from the same source has been demonstrated to be sound after five years or more of service under conditions of weather, wetting and drying, and erosive forces similar to those anticipated. Alternatively native or imported stone, already at the site and meeting the standards outlined above, may be used. d.The ratio of maximum to minimum boulder dimensions shall not exceed 3.0. The Contractor shall furnish all non -woven filter fabric as described in the Construction Documents (or an equivalent approved by the Engineer in writing). B. Filter fabrics shall be installed per manufacturer's recommendations. C. Filter fabrics shall be placed in direct contact with undisturbed bank materials. D. Filter Fabric shall be free of tears of rips prior to and following boulder installation. E. To protect filter fabrics during construction a layer of bedding material, with a 6 inch minimum depth, shall be placed over all filter fabrics prior to receiving boulder installations. CONSTRUCTION SEQUENCE: 1. Call 811 (North Carolina) OR 1-800-632-4949 from anywhere in the U.S for utility information. Call 2 business days in advance before you begin construction, grade or excavate for marking of underground member utilities. 2. Review and maintain all necessary construction permits. 3. Notify Owner's Representative and Engineer of anticipated start date. 4. Perform all specified Site Documentation. 5. Prepare and submit all specified Submittals; 6. Prepare and post all site safety and construction protocols-, 7. Identify construction access, staging, stockpiling, and oiling areas for Engineers approval; 8. Install BMPs, barrier fencing, signs, and safety protocols; 9. Locate and clearly mark, all onsite utilities; 10. Protect in Place all permanent site features (i.e. trees, infrastructure, buildings, utilities, etc.); 11. Locate and install all applicable temporary facilities; 12. Establish all necessary Survey Control, locations to be approved by the Engineer prior to installation-, and 13. Use approved automatically controlled grading and paving equipment to produce final subgrade and base surfaces meeting the lines, grades and cross sections required by the plans or as directed. When in the judgment of the Engineer the use of such equipment is impractical, this requirement will be waived. - Dampen the surface of the base at the time the concrete is placed. Sprinkle the base when necessary to provide a damp surface. Ensure that no free water or ponding is present at the time of concrete placement. INSTALL FOOTER STRUCTURES -Stone boulders shall be placed as shown on the drawings, without any gaps, so that each boulder is in direct contact with all surrounding boulder. -Boulder to be placed on subgrade compacted to 95% of maximum as determined by the standard proctor method (ASTM d-698-66t or AASHTO t 99) or undisturbed native alluvium. -Non-woven 8oz filter fabric (Mirafi 180n or equivalent) shall be placed in intimate contact with undisturbed bank materials and overlain with a minimum depth of 6 inches of bedding material prior to the placement of all boulder within the active river channel. PLACING CONCRETE (A) General Note: No wet concrete is to come in contact with surface water. See sheet WW-6.0 for dewatering notes. Use a slip form paver to place concrete except where impractical due to irregular areas or areas of existing pavement adjacent to the proposed pavement. Handle concrete so as to prevent segregation and keep free from mud, soil or any other foreign matter. Do not pave when any of the following conditions exist: (1) A descending air temperature at the location of the concrete paving operation and away from artificial heat reaches 35°F. Paving may resume when the weather forecast is projected to reach a high of 40°F on that day's operation and the morning ambient temperature is above 32°F. (2) The subgrade or base course is frozen. (3) Aggregates to be used in the mix contain frozen particles. (4) Air temperature in the shade is 90°F and rising or the concrete temperature is greater than 95°F. Where additional pavement, aggregate or soil must be placed adjacent to new pavement by machine methods, do not place it until the concrete has attained a compressive strength of at least 3,000 psi. Construction equipment or hauling equipment will not be allowed over the pavement until the concrete has attained a compressive strength of 3,000 psi. (B)Grout: Mixing and Transporting Concrete Use mixers capable of producing concrete that is thoroughly mixed, of a uniform mass, and discharging the concrete such that the concrete mixture and aggregates are uniformly distributed. (C)Preparation and Placement of Concrete Completely discharge and place concrete in place no later than 90 minutes after the cement has been mixed with water or aggregates. Spread the concrete uniformly over the entire area without segregation. Perform the spreading with a mechanical spreader independent of the paver except where hand methods are necessary due to pavement design, equipment breakdown or other emergencies. Place the concrete in continuous operation until the unit section is completed. Use a concrete placing rate that ensures that each layer is placed while the previous layer is soft or plastic, the two layers become monolithic by penetration of the vibrators, and cold joints are not produced.Place boulder concrete such that the concrete penetrates and fills the voids between the boulders. place boulder concrete for each pour after the boulders are in their proper position and of a significant size to create a monolith of significant proportions (greater than 5 boulder widths or 1500 square feet). Limit the lateral extent of each pour such that the concrete is completely contained within the boulder mass. Remove and replace any concrete that extends beyond the boulder mass and interferes with the proper placement of adjacent boulders or work.Place boulder concrete to the specified depth. Uniformly and thoroughly compact concrete as it is being placed. Use vibrators of the proper size, frequency and amplitude, supplemented by hand spading and tamping when required, to secure a dense, homogeneous concrete and a good bond with the reinforcement, embedded items and boulders.
