Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDI-S discharge permit. This permit is issued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on April 28, 2004.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
You, you have the tight to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be

Cleveland
Rowan County
to receiving waters designated as an unnamed tributary to Beaverdam Creek in the Yadkin -Pee Dee River
Basin in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts 1, 11, 111, and IV hereof.
The permit shall become effective August 1, 2004.
This permit and the authorization to discharge shall expireat midnight on March 31, 2009

The Permittee shall obtain approval from the Division's Aquatic Toxicology Unit: prior to the use of any biocide (not
previously approved by the Division) in the effluent discharged under this permit. Approval for use of any biocide
not previously approved should be requested at least 90 days in advance of any planned usage.

No changes are proposed. TRC limit was not added since chlorine was never
detected in the discharge.
Summary
The facility has requested a permit renewal with no modifications.
The following notes were taken during the review of the application package:
• 303(d) List Review: Beaver Dam Creek is not listed on North Carolina's
1998 303(d) List.
• Review of Basin Plan: No information was reported for Beaver Dam Creek
in the Basin Plan.
• Staff Report (prepared 2/8/99): Region did not provide specific comments
in reference to the subject permit. Region recommends issuance of permit.
Staff Report (prepared 6/30/93: Facility discharges contact cooling water
from one outfall (Outfall 001) and air compressor condensate from a second
outfall (Outfall 002). The discharges from the facility do not enter the
receiving stream by a direct conveyance, rather they empty onto the ground
behind the building and meander through a wooded area owned by the
company. Most of the contact cooling water used by the autoclaves is
recycled. When a discharge is necessary, it is usually on an infrequent basis
(3-5 times per month). Most of the water discharged either soaks into the
ground or is lost to evaporation before reaching the receiving stream.

The facility has a good compliance record, during the review period, only
one Notice of Deficiency was issued as a result of compliance evaluation
inspection due to minor deficiencies in Laboratory, Self -Monitoring, and
Flow Measurement.

the Town of Cleveland, travel south on SR 1741 approx. 1.0 miles (The road changes
name from Kesler Rd. to Redmon Rd.). The entrance to RDH Tire and Retread Co. is on
the left (south) side of Redmon Road via a dirt access road.

C. Describe receiving stream features and pertinent downstream uses: Flow was
observed in the receiving at the time of the site inspection. The stream had a well
defined channel and was approx. 4 feet wide and 2-6 inches deep. The
surrounding area is rural with agriculture being the primary use. Downstream
users are unknown.

PART IV - EVALUATION AND RECOMMENDATIONS
The permittee requests renewal of the subject Permit which authorizes the discharge of
contact cooling water from two (2) autoclaves used for curing retreaded tires and an air
compressor unit.
The discharges from this facility do not enter the receiving stream by a direct conveyance,
rather they empty onto the ground behind the building and meander through a wooded area
owned by the company. Most of the contact cooling water used by the autoclaves is recycled.
When a discharge is necessary, it is usually on an infrequent basis (3-5 times per month). Most of
the water discharged either soaks into the ground or is lost to evaporation before reaching the
receiving stream.
It is recommended that the Permit be renewed as requested.
