Wilson, Environmental Engineer , i i Subject: USDA Rural Development—West Caldwell Health Council, Inc(Medical Facility) Project No. 11-0307 Caldwell County The proposal is for the construction of an approximately 6000 ft2 medical facility for primary health care services at 4330 Collettsville Rd., Collettsville,NC (PIN No. 2800739006). The following concerns related to surface water permitting may, or will likely, need to be addressed: 1. NPDES Construction Stormwater Permit NCGO10000—This permit is issued concurrently with an approved Sediment and Erosion Control plan to control stormwater discharges from construction activities. If greater than one acre is disturbed, a NCG010000 permit is required. 2. 401 Water Quality Certification—If a 404 permit is required by the Army Corp of Engineers, a 401 Water Quality Certification is necessary. Depending on the amount of impact to a.stream and/or wetland—written concurrence from this office may be required. The information provided in the scoping document does not indicate impact to surface waters or wetlands, however, this should be confirmed by the applicant or the consultant. 3. Caldwell County Phase 11 stormwater requirements- If they have not done so already,the applicant should ensure compliance with any Phase 11 or additional stormwater requirements. The ARO recommends that the applicant contact Mike Randall, Environmental Engineer, Stormwater Permitting Unit at(919) 807-6374 to determine the appropriate reviewing agency(if Phase II stormwater controls are required; if total disturbance is less than 1 acres, Phase I1 may not be applicable). Wilson, Environmental Engineer (Dew-'-pa- S t4'J';P) Subject: Dry Ponds Water Project(Amendment)—Town of Sawmills Project No. 10-0392 Caldwell County The Town of Sawmill proposes to replace/construct a 6 inch water line on Union Church Road and May Road (this is an addition to the original project). The following concerns related to surface water permitting may, or will likely, need to be addressed: 1. NPDES Construction Stormwater Permit NCGO 10000—This permit is issued concurrently with an approved Sediment and Erosion Control plan to control stormwater discharges from construction activities. If greater than one acre is disturbed, a NCG010000 permit is required. 2. 401 Water Quality Certification—If a 404 permit is required by the Army Corp of Engineers, a 401 Water Quality Certification is necessary. The scoping document indicates that the water line(s) will not impact surface waters or wetlands, therefore, impacts to surface waters may not be an issue.
