14, 2009 for the above referenced project. The DWQ has determined that your application was
incomplete and/or provided inaccurate information as discussed below. Please provide the following so
that we may review and approve your project.
Additional Information Requested:
® PCN failed to reflect any recorded impact on pages 4 and/or 5. Due to lack of impacts check
#042068 for $240 is being returned until cumulative impacts are received and verified. Please
supply one (1) original and four (4) copies.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please
be aware that any impacts requested within your application are not authorized (at this time) by the DWQ.
