Dear Mr.. GT1arini:
• A letter of request for art Aut-horiznt.ion to Construct teas received .July 1.2,
1989, by the Division and final plans and specili.cations for the subject project
have been reviewed and found to be satisfactory. Authorization is hereby granted
for the coristr.uction of modifications to the Nutbush Creek Wastewater Treatment
Plant consisting of the demolition of sludge cirying beds and the construction of two
(2) flow spliter boxes, two (2) new 7.5-foot diameter clarifiers to operate in
series with the existing clarifiers, a new return sludge pump station with a 1440
GPM pump and a 300 GPM pump and high water alarms and a new ef:f:liient return pump
station with dual. 900 GPM minimum speed, two (2) speed pr-er.otation pumps with high
water alarms.
This Authorization to Construct .is :issued in accordance with Part TIT paragraph
B of the NPDES Permit No. NC0020559 _issued April 9, 1986, and shall be subject to
revocation tililess the wastewater treatment facilities are constructed in accordance
with the conditions and limitntions specified in Permit No. NC0020559.
The Permittee must employ a certified wastewater operator in accordance with
Part TTT paragraph C of the referenced permit,.
Thesludge generated from these treatment facilities must be disposed of in
accordance with G.S. 1.43-215,1 and in a manner app.rovable by the North Carolina
Division of Environmental Management.
The Raleigh Regional Office, telephone number 919/733-2314 shall be notified
at least twenty-four (24) hours i.n advance of operation of these facilities. Such
notification to the Regional Supervisor. shall he made during the normal office hoar
• from 8.00 a.m. nnti.l 5:00 p.m. on Monday thrangh Friday, excluding State Holidays.

In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional
wastewater treatment and disposal facilities.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications.
A copy of the approved plans and specifications shall be maintained on file by
the permittee for the life of the project from the date of issuance.
