Dear Mr. Holmes:
The final plans and specifications for the subject project have been reviewed
and found to be satisfactory. Authorization is hereby granted for the construction
of interim wastewater treatment facility, additions to the Nutbush Creak Wastewater
Treatment Plant, to consist of a 12,000 gallon bulk alum storage tank, one 53 gph
alum feed pump, 500 gallon polymer feed and ageing tanks and two 2 gpm polymer
feed pumps.
This is a Class III Wastewater Treatment Plant which requires that the person
in responsible charge hold a valid Grade III Certificate.
This Authorization to Construct shall be subject to revocation unless the
wastewater treatment facilities and additions are constructed in accordance with
the conditions and limitations specified in Permit No. NC0020559.
One (1) set of approved plans and specifications is being forwarded to you.
