Dear Mr. van der Meyden:
A letter requesting authorization to install a flow measuring device was received July 27, 1994 by
the Division. The submitted information has been reviewed and found to be satisfactory. In accordance
with 15A NCAC 2B .0505(b), authorization is hereby granted for the installation of a standard 8"
Palmer-Bowlus flume at outfall 013 with discharge into the Yadkin River, classified Class "WS-IV and B
CA" waters, in the Yadkin River Basin.
The Mooresville Regional Office, telephone number 704-663-1699, shall be notified at least forty-
eight (48) hours in advance of operation of the installed device. Such notification to the regional
supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday
through Friday, excluding State Holidays.
A copy of the approved specifications shall be maintained on file by the Permittee for the life'of the
facility.
