2. Provide a detailed site plan showing property boundaries and proposed locations of
vegetation clearing, structures (buildings, retaining walls, docks, impervious surfaces,

etc.), rip rap, excavation or dredging below Full Pond/ Normal Water Level elevations,
and construction access corridors. You may use the diagram provided at the end of this
application form.
3. Attach a photograph of the shoreline/ buffer proposed to be stabilized. (Include a scale
of some sort- a yard stick, shovel handle, etc.)

8. Describe proposed work (include discussion as to how hardening of shoreline has been
avoided, or why it is necessary): I would like to propose a retaining wall to avoid
continuous erosion to the shoreline. It is necessary, as in the small period of time I have
owned the home, trees have fallen (and more are close to) fall into the Lake. Also, the
shoreline is continuously eroding around the front portion of the property due to wake,
and I would like to mitigate further erosion.

square feet or acres: approximately 225 feet of retaining wall to be erected (225 feet long
x 10" wide) — Total —188 so feet from too view.

50 feet landward to be impacted (number of trees, for instance): Approximately 5-6 trees

will need to be removed, as they are already sloping over water edge and impending fall
into water is inevitable. 3 trees have fallen into water (with land) over last several months.
