Dear Mr. Hardy:
The Division has reviewed and approved a request from Mark Matthews, ORC for your WWTP. Mr.
Matthews correctly noted that the installation of UV disinfection at the subject facility obviated the need for
mandatory monitoring of Total Residual Chlorine (TRC). The Winston-Salem Regional Office concurred with
his request to make TRC monitoring optional [required only if chlorine is used for disinfection] in the subject
permit. Therefore, this modification adds UV disinfection to the Supplement to Permit Cover Sheet, as well
as making TRC monitoring optional. No other changes to the permit have been made.
Please find enclosed the revised pages, which should be inserted into your permit. The old pages may
then be discarded. All other terms and conditions contained in the original permit remain unchanged and in
full effect. This permit modification is issued under the requirements of North Carolina General Statutes
143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental
Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit modification
are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be a written petition conforming to Chapter 150B
of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail Service
Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and
binding. If you have any questions concerning this permit modification, please contact Charles Weaver of

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked. As of this permit
issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive authority to operate
and discharge from this facility arises under the permit conditions, requirements, terms, and provisions included herein.
The Town of East Bend is hereby authorized to:
1. Continue to operate an existing 0.01 MGD wastewater treatment facility with the
following components:
♦ Extended aeration
♦ Settling
♦ Sludge holding
♦ UV disinfection
The facility is located in East Bend at the East Bend Industrial Park off Salem
Trail in Yadkin County.
2. Discharge treated. wastewater (via Outfall 001) from said facility into the unnamed
tributary to the Yadkin River, a Class WS-IV water in the Yadkin -Pee Dee River
Basin.

A. (1.) EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS
During the period beginning on the effective date of this permit and lasting until expiration,
the permittee is authorized to discharge from outfall 001. Such discharges shall be limited
and monitored by the Permittee as specified below:

1. U: Upstream at least 100 feet from the outfall. D: Downstream at least 300 feet from outfall.
2. The monthly average effluent BODs and Total Suspended Solids concentrations shall not exceed 15
percent of the respective monthly average influent value (85% removal).
3. TRC limit and monitoring requirements apply only if chlorine is added to the treatment system.
4. The daily average dissolved oxygen concentration in the effluent shall not fall below 5.0 mg/L.
There shall be no discharge of floating solids or visible foam in other than trace amounts

I would like to request a permit change due to a change in
operation of our wastewater plants. We are no longer using
chlorine as a disinfectant. We have replaced the chlorine with UV
systems at the following sites.

Starmount High 70459
I also operate a plant for the town of East Bend. We have gone to
UV disinfection there also. The plant ID 19=64726?
We have kept the original systems for tablets of chlorine and
dechlorine in case of a power outage.
