The North Carolina Division of Water Resources conducted a file review inspection of the 490 Shell Hill Rd SFR
on 6/02/2020. This inspection was conducted to verify that the facility is operating in compliance with the
conditions and limitations specified in Non -discharge Permit No. WQ0015234. The findings and
comments noted during this inspection are provided in the enclosed copy of the inspection report
entitled "Compliance Inspection Report". There were no significant issues or findings noted during the
inspection and therefore, a response to this inspection report is not required.

Inspection Summary:
This inspection report is being prepared for the renewal of SFR Permit W00015234. Due to COVID-19 and restrictions place
on WIRO staff, only a file review and phone interview with the Permittee, Donna Billings, was performed in place of a site
visit.
The property, 490 Shell Hill Road, Sealevel, is a vacation property and is not occupied year around. There have been no
violations issued for this facility. The Permittee stated that Spectrum Environmental inspects and completes any repairs
needed to the SFR wastewater treatment and drip irrigation system once a year. A log book is kept on site to record when
the system is used. A spare bulb for the single UV light unit is on site.
According to the Permittee, there have been no changes to the SFR wastewater treatment or drip irrigation systems, and
property lines have not changed since the last renewal.
